Persons other than you and your health care providers must have permission to view this booklet. TELEMONITORING Our Technology With the help of hospital-‐grade in-‐home monitors, you can check your vital signs daily. These vital signs are then transmitted to a central monitoring station where our nurses review and contact you in the event of any abnormal readings. This technology paired with our clinician’s home visits is our way of caring for you every day. Telehealth Benefits Telemonitoring also assists us in providing disease management services to you if you have a chronic health condition such as diabetes, heart and/or lung disease. We monitor your condition daily and can detect changes quickly. These changes are communicated to your physician. He/she is able to make changes in your care to prevent trips to the hospital or emergency room. Types of Monitors THA Group uses 3 types of telemonitors: Cardiocom, Philips, and Honeywell/HomMed. Your nurse or therapist will explain in detail how to use your telemonitor. The following pages provide instructions for your particular telemonitor. Please remember if you need help, you can call 1.888.842.4663 for additional assistance.
