Health Snapshot: District 10 Commissioner Javier Souto The Office of Countywide Healthcare Planning (OCHP) is the health planning agency tasked with finding real solutions to critical health problems facing Miami-Dade County. This snapshot correlates OCHPs key initiatives to the issues and priorities facing residents of District 10 [includes District 10 zip codes: 33144, 33155, 33165, 33172, 33173, 33174, 33175, 33183] Achieving a Network of Integrated Community-Based Medical Homes OCHP is leading a MDC-wide comprehensive planning process to develop a roadmap that will become the guide for leveraging both additional capital and operating dollars to expand our health delivery system. The Goal is to turn Community Health Systems into full spectrum portals to comprehensive health and services. That is 14% of Miami-Dade 2 County’s uninsured– See Map • 16% of residents of District 10 are enrolled in 1,5 Medicaid • Median Household Income – Median income for District 10 residents was estimated at $43,364, ranging from $32,258 (zip 33144) to $51,445 (zip 33173). 5 Office of Countywide Healthcare Planning – August 2012 ER Diversion & Chronic Disease Management Dual initiatives utilizing evidence-based approaches will assist individuals seeking primary care in receiving an immediate appointment – while still in the ER – for a primary care visit in their neighborhood. For individuals living with chronic diseases, the initiative focuses electronic-enabled mechanisms for the identification of conditions, case management by licensed professionals, and guided treatment. 4 Sampling of Chronic Disease Indicators from District 10 : • Influenza & Pneumonia – 18% (40) of the County’s deaths due to influenza and/or pneumonia were District 10 residents. o • Zip code 33165 accounted for 33% (13) of District 10’s deaths due to influenza and/or pneumonia. Breast Cancer – 17% (52) of the County’s deaths due to breast cancer were residents of District 10. o Zip code 33165 accounted for 25% (13) of District 10’s deaths due to breast cancer. • Cardiovascular Disease – 16% of the County’s 5,916 deaths due to cardiovascular disease were District 10 residents. • Diabetes – 15% of the County’s 637 deaths due to diabetes were residents of District 10. o Zip code 33175 accounted for 28% (27) of District 10’s deaths due to diabetes. 3 Emergency Room Visits from District 10 Residents : • Over 93,000 visits to Miami-Dade County Emergency Departments were made by District 10 residents in 2011. • 12% of the total number of ER visits by Miami-Dade County residents was made by residents of District 10. Health Statistics at Zip Code and District Levels OCHP has developed critical relationships with State and local data sources to build a comprehensive database of nationally-comparative health and demographic data, as well as, health utilization and health system infrastructure. OCHP is able to present this data in baseline, trend analysis, projection and GIS formats.
