While there is no registration fee for the walk, all participants are encouraged to fundraise. Because Highmark Blue Shield underwrites the cost of the walk, 100 percent of money raised goes directly to the organization of your choice. LifeSpan, Inc. invites you to support our involvement in this year’s Highmark Walk for a Healthy Community Saturday, May 16th at Stage AE in Pittsburgh. LifeSpan’s mission is to offer resources, programs and services that promote healthy and dignified aging. As the aging population grows, government and private funding sources are decreasing. LifeSpan, a 501 (c) (3) not-for-profit agency, must currently raise more than $500,000 annually to supplement the program funding received from the Allegheny County Department of Human Services Area Agency on Aging. TUESDAY, JUNE 16, 2015 FARMERS MARKET VOUCHER DISTRIBUTION ALLEGHENY COUNTY SENIOR FARMERS MARKET NUTRITION PROGRAM—SFMNP Seniors must be 60 or older by December 31st of the program year and meet the income eligibility guidelines. Income eligibility is based on 185% of the federal poverty income guidelines. The guidelines are published each year by the United States Department of Agriculture. How the Program Works for Recipients: Care Management Center Services Family Caregiver Support Program Meals on Wheels/Home Delivered Meals Information and Assistance Options Program Community Outreach Senior Companion Program Volunteer Program Eligible Senior recipients receive the SFMNP by going to a distribution site in their county. Distribution site information is available by calling the county aging office in the county the senior resides. Recipients are given a list of participating farmers and farmers' markets when they receive their checks. These checks are redeemed for fresh fruits and vegetables grown in Pennsylvania. Each eligible recipient receives 4 $5.00 SFMNP checks to redeem at a qualified farmers' market or roadside stand. There are over 1,000 FMNP participating farmers at 800 farm stands and 190 farmers' markets in Pennsylvania. Proof of age and county residency required to receive vouchers. Meals on Wheels, Free Rides for Seniors shuttle drivers and drivers who can transport seniors to medical appointments in the volunteer’s own vehicle are all needed. For more information on the program locations, or to register as a volunteer with OYHS, please call 412-307-0071 or email [email protected]. Spring’s approach prompts seniors to seek yard help The turn of the calendar and warming weather has prompted a number of seniors countywide to call Open Your Heart to a Senior seeking help with light yard work. Volunteers are needed in all neighborhoods of Allegheny County to help clear yard debris, plant flowers, and mow seniors’ lawns if possible come late spring or summer. This is a great volunteer opportunity for teens and groups. To register as a volunteer with OYHS, call 412-307-0071 or email [email protected]. Open Your Heart to a Senior needs volunteers year round and countywide Volunteers are needed countywide to help seniors remain independent and stay safe in their homes. United Way’s Open Your Heart to a Senior initiative now has several nonprofit partners in Allegheny County to provide volunteer services to seniors. Volunteers can deliver meals, take seniors to appointments or the grocery store, help at senior centers and much more. Balance due by September 4th. SPRING 2015 DIALOGUE PAGE 5 Older Americans Month May 2015 Older adults are a vital part of our society. Since 1963, communities across the country have shown their gratitude by celebrating Older Americans Month each May. The theme of this year’s celebration is “Get into the Act,” to focus on how older adults are taking charge of their health, getting engaged in their communities, and making a positive impact in the lives of others. The theme also reflects on the 50th anniversary of the Older Americans Act. President Lyndon B. Johnson signed the Older Americans Act into law in July 1965. Since that time, the Act has provided a nationwide aging services network and funding that helps older adults live with dignity in the communities of their choice for as long as possible. These services include home-delivered and congregate meals, caregiver support, community-based assistance, preventive health services, elder abuse prevention, and much more. By promoting and engaging in activity, wellness, and inclusivity, more Americans than ever before can “Get into the Act.” While LifeSpan, Inc. provides resources and services to older adults year-round, Older Americans Month offers an opportunity to emphasize how older adults can access the home- and community-based services they need to live independently in their communities. It is also an occasion to highlight how older adults are engaging with and making a difference in those communities. Throughout the month of May, LifeSpan will be conducting activities and providing tips on how to access resources and services designed to promote healthy and dignified aging. It is LifeSpan’s vision to advocate for and provide innovative community resources, programs and services for the changing needs of older adults and those who care for them. Be sure to participate in our Center activities in May, i.e., Picnic in the Park—May 29th; Knoxville Center Banquet May 20th; Hillsdale Center—Primary Colors Art Guild 13th Annual Spring Exhibit & Sale, May 15th & 16th; Highmark Walk for a Healthy Community— LifeSpan Fundraiser May 16th. Donations, which are taxdeductible to the extent allowed by the law, support programs and services offered to older adults living in the community. Consider a donation to LifeSpan in lieu of flowers for bereavement or “In Memory of a Loved One”. These donations are separated from funding received by the Allegheny County Area Agency on Aging. SPRING 2015 DIALOGUE PAGE 10 UPMC Health Plan and LifeSpan team up to help older adults lead healthy, active lives UPMC Health Plan is partnering with LifeSpan, Inc., “Resources and Services for Seniors,” to provide older adults in Allegheny County with a variety of services and exercise equipment designed to help them lead healthier and more active lives. The partnership will help LifeSpan improve the health of its community through lifestyle changes that include fitness, nutrition management, health education, and health screenings. Join us at the location nearest you to participate in fun and healthy activities. UPMC Health Plan members can take advantage of the following health screenings: BMI/BP Spirometry Hemoglobin A1c Diabetic Eye Exam Diabetic Kidney Test Mail in Colorectal kits BMD (bone mineral density) ________________________________________________________ UPMC for Life has a contract with Medicare to provide HMO and PPO plans. Enrollment in UPMC for Life depends on contract renewal. UPMC for Life is a product of and operated by UPMC Health Plan, Inc., UPMC Health Network, Inc., and UPMC Health Benefits, Inc. UPMC for You Advantage is an HMO SNP plan with a Medicare contract and a contract with the Pennsylvania Medical Assistance (Medicaid) program. UPMC for Life Options and UPMC Community Care are HMO SNP plans with a Medicare contract. Enrollment in UPMC for You Advantage, UPMC for Life Options, and UPMC Community Care depends on contract renewal. Non-members and members visiting the LifeSpan facility can take advantage of: Health and wellness tips from a UPMC Health Plan health coach. Snacks, giveaways, games, and participate in a raffle.
