NEARLY ME® breast forms come in several shapes, weights and softnesses to help you match your
customer’s natural breast as closely as possible. Because of these variations, we have created distinctive
form “profiles” in our symmetrical triangle and symmetrical oval forms to help in the fitting process
selection. Reference these styles and their recommended profiles when determining a form style for
your customer:

Back profiles...
•A Curved Back is found on many triangle and classic style forms. These forms contour to the chest wall

more remaining tissue under the arm
or on the front of the
chest after surgery, or
who have small rounded
natural breasts.

•A Flat Back with a slight concavity in the
middle is found on the oval, butterfly and
certain triangle style forms. This back works
well for flat surgical surfaces or where the surgical site does
not curve under the arm.
Also consider:
• The softness, the weight and drape of the remaining natural breast.
• The remaining natural tissue left on the surgical side.
• The type of surgery that the woman has had.
Remember, fitting a prosthesis is “more art than science” and the ultimate goal is to match the remaining breast as closely as possible.

dependent on the measured bra size but the indicated sizes are ONLY A GUIDELINE.

General Company Information
MINIMUM ORDERS: Must have an opening order of $500 and
a $50 minimum reorder. Must provide tax ID number and credit
information.

CREDIT TERMS: Standard payment terms with approved credit
are NET 30 DAYS from invoice date. All merchandise shipped is
invoiced on the date of shipment.
All merchandise has been inspected prior to shipment; however, if you find shipping damage that was not obvious upon
receipt or error in product received, call your Customer Service
Representative at 800-887-3370. All claims for damage and shortage (container lost in transit) MUST BE MADE WITHIN 15 DAYS
FROM DATE OF SHIPMENT. If you have any questions, call our
Customer Service Dept.
SPECIAL TERMS: May be available on introductory offers and
special promotions. These should be discussed with your Sales
Representative.
DISCOUNTS: If offered, will be indicated on the invoice and calculated from the INVOICE DATE. Discounts taken from the receiving
date cannot be honored and will be charged back to your account.
FINANCE CHARGES: A 2% finance charge is applied to any overdue balance on a monthly basis.
CONSIGNMENT: We do not offer consignment terms on any of
our products or in conjunction with any special or seasonal promotions.
PREPAYMENT/COD: Customers purchasing on COD or checkwith-order basis must pay by cashiers check, money order or credit
card. Any freight or handling charges assessed by carrier will be
included in the total. Nearly Me Technologies, LLC. (NMT) does
not accept COD refusals.
PURCHASE ORDERS: Any statement contained on any purchase
order or similar document that is not specifically approved or
acknowledged by NMT will not be considered as part of the agreement between the parties.
All purchasers shall furnish a TAX EXEMPTION or RESALE
CERTIFICATE to NMT as directed by Multistate Tax Commission
regulations. Future purchases may be delayed if this information is
not furnished within 10 days of your opening order.
GENERAL EXCHANGE POLICY & PROCEDURE: NMT has an
authorized exchange policy of 60 days. Any authorized exchange,
unless product has been shipped in error or is a defective product,
will be charged a 15% restocking fee if returned within 60 days from
ship date. Any unauthorized return will be charged a 50% restocking fee if returned within 60 days from ship date. NMT accepts no
returns, authorized or unauthorized, after 60 days unless it is a
defective product.
All returns initially must be pre-paid. If the item was shipped in
error by NMT, or is defective product, your account will be credited
for the return postage/freight. Exchange Authorization Numbers
must be clearly marked on the outside of all packages.

(800-887-3370) or fax (254-662-1761). Have available: the date of
purchase, invoice number, and the reason you wish to return the
item. Discontinued product cannot be returned.
No refund checks will be issued, merchandise credit ONLY. All
product remains the property of NMT until invoice is paid in full.
Special Notes: Since we have an ongoing goal of product improvement, we reserve the right to improve or alter product design and
specifications without notice.

WARRANTIED PRODUCTS: If you replace a defective breast form
from your stock, remove the warranty card from the box before giving the replacement form to the customer. You will need to place
the warranty card in the new form that you receive from NMT.
NMT will not include a warranty card with the replacement form.
GENERAL SHIPMENT POLICY & PROCEDURE: Please examine
container(s) on receipt. Note any damage on the delivery bill and
