Program Information
We will accept registration forms by email, phone, fax, or in person Monday through Thursday 8:30am to5:00pm and
Friday8:30am to 4pm. Registration will remain open until the week prior to each event. Payments will be accepted in the
form of cash, money order, personal check, Master Card, Visa, or Discover. Registration is done on a first come, first served
basis. If the program is full, you may request to be put on a waiting list. If the program has less than the set minimum number
of participants at the deadline time, then the program will be cancelled. All group home residents need to be registered
together.
Photo Policy
IRVSRA will occasionally take pictures during special events. We sometimes use these pictures for our brochures, pamphlets,
flyers, publications, videos, etc. By signing this agreement the pictures of the participant may be used for the previously stated.
Annual Information Forms
This form is important to us because it’s valuable information such as emergency contact, typical participant behaviors,
medications, allergies, etc. This form needs to be updated annually or when there is a change in any information. It is the
parent/guardian and participant’s responsibility to inform us of any medication the participant is taking. This information is
beneficial and will remain confidential.
Liability Waiver
The IRVSRA liability insurance carrier requires that prior to participation in any of the IRVSRA program; the legal authority
and participant must sign the waiver named, “Waiver, Release of All Claims, and Hold Harmless Agreement for Illinois River
Valley Special Recreation Association”. PARTICIPANT WILL BE DENIED if the signature of the adult participant or
parent/guardian, and date are not on the liability waiver.
Dispensing Medication
Participants who need medication during a program will need to fill out a medication dispensing form before the start of the
program. Forms need to be signed by the participant’s guardian. To obtain a medical form, visit the website or email the
Program Supervisor. Please note that all medication should be in its original container, clearly labeled. For further questions
contact the Program Supervisor.
Resident/Non-Resident Policy
The Illinois River Valley Special Recreation Association is collaboration between the Fon du Lac Park District and Pekin Park
District. Any individual who pays taxes to one of these districts are considered residents, and therefore will pay a few dollars
less than Non-Resident fees. Each program description will indicate an “R” for Resident or “NR” Nonresident fees.
Inclusion Services
Fon du Lac Park District recognizes that there are some individuals who need assistance and support for successful
participation in our programs. Support may include training with Park District staff, consultation with the child and the family.
We provide all inclusion services at no cost to the individual.
Cancellation Policy
Programs will be cancelled due to the following:
There are not enough participants signed up, unexpected staffing concern, certain weather conditions including temperature
being below 15 degrees F or above 95 degrees F, wind chill is below 20 degrees or heat index above 100 degrees F, a winter
weather advisory is in effect or heat advisory warning is in effect.

Programs & Events
Come be a part of the Fon du Lac IRVSRA Track and Field team. Practices will be every Thursday starting
March 12th and run until April 30th. On May 3rd, athletes will participate in the 50 meter run and softball throw
at the Spring Track and Field meet held at Normal West High School in Normal, IL. Athletes who earn a gold
medal are eligible for the Summer Games on June 12th -14th. Additional practices will be offered to those
advancing to State. Athletes wanting to participate in track and field must have or be able to obtain, a current
medical application by the end of March. All practices will be held at EastSide Centre in East Peoria unless
otherwise stated.

Reminder:
Pick up and drop off will always be at the Fon du Lac Park District Administration Center, unless otherwise
stated. Please connect with a staff member before leaving your participate. If your participant needs
medication administrated please see a staff member at each program to fill out a form.
Registration will begin on March 6 and continue after this date. Please remember the longer you wait to
register, the less likely there is to get into a certain program. We will accept registration forms by email,
phone, fax, or in person.

Aktion Club
This club meets on the first Tuesday of every
month. Participants get a chance to give back to
the community as well as enjoy entertainment and
a snack. Aktion Club has been in existence for
several years, if you don’t know what it’s about,
come to a meeting.

Enabling Gardens
This class if for anyone who likes to get their
hands dirty. Join Pam as she creates beautiful
master pieces for the spring season and gets the
garden ready for planting. Please be specific with
dates and times when registering for this program.

“Lights Up Sound Down” gives families with children who have special needs
the opportunity to enjoy their favorite movies in a comfortable, sensory-friendly
setting with the lights turned up and the volume down in the auditorium. This is
a free event to the first 100 registrations and their family. Please visit

you are signing up for the Peoria movie. The movie choice will be announced
about two weeks prior to the showing date.
