The IPS family of highly esthetic all-ceramic restorative alternatives has grown to include several prosthetics
with successful, predictable and proven clinical results. These include Empress Esthetic, e.max Press &
e.max ZirCAD.
With almost 2 decades of proven clinical success, Empress Esthetic’s leucite-reinforced pressed ceramic is
virtually unmatched in the all-ceramic realm. With a flexural strength of 160 Mpa, Empress Esthetic must be a
bonded restoration, as bonding adds 40% more strength to the crowns or veneers.
IPS e.max Press (lithium disilicate glass-ceramic) is an ideal alternative for higher strength crown and bridge
applications in the anterior esthetic zone and premolar regions. A dual-cure bonding material must be used
for e.max Press (400 Mpa) restorations.
IPS e.max ZirCAD was developed to address the demand for maximum strength in the e.max family
of prosthetics. Because of the inherent strength of Zirconia (>900 Mpa), these restorations are best
conventionally cemented and indicated for crowns up to 6-unit bridges.

The Defender mouthguards are prescribed for both professional and amateur athletes because they offer the highest
level of protection during sports activity. When worn properly, they can prevent dental trauma, such as fractured teeth

New Image Dental Laboratory works with UCLA abutments to make cast custom abutments or can use stock abutments from numerous implant companies including: Biomet 3i, Nobel Biocare, Zimmer, Astra Tech, Straumann,
Dentsply Friadent, BioHorizons, Lifecore, Sybron Innova, BTI and Thommen.

New Image uses a variety of implant systems to scan and design custom abutments. Using these systems allows
us to match the appropriate abutments to your implants in order to retain your warranty. Manufacturers require their
abutments to be placed on their implants. We can design and mill custom CAD/CAM abutments from Biomet 3i(*,**),
Nobel Biocare - Procera(***), Zimmer and Astra Tech/Atlantis.

Implants - CSI (Complete Simplified Implant)
From your impression a soft tissue master model is fabricated and scanned. We subsequently design and mill your
abutment from your choice of material. The analog, soft tissue model, screw and placement jig are included with your
CSI. The New Image CSI is available with Biomet 3i(*,**), Nobel Biocare - Procera (***), Zimmer and Astra/Atlantis
abutments. Choose fixed crown and bridge from the menu above to complete your case.

Implant Components
Componentry can be ordered by the laboratory for your convenience. However, be advised any rush shipping and
handling fees will be added to the final cost of the components when invoiced. The laboratory would be happy to
provide you with a list of parts should you want to order them directly from the implant manufacturer.
Policies
INFECTION CONTROL - OCCUPATIONAL SAFETY AND HEALTH ADMINISTRATION (OSHA)
It has been determined that many tasks within the dental laboratory are classified as Category I
or Category II hazards as defined by OSHA. We are committed to a proactive role in providing our
employees, dentists and their patients with protection from the dangers of cross-contamination. A
charge of $1.75 is added to each case to help defray the costs of our comprehensive compliance
program.
PAYMENT TERMS
All work done by Lab is due in full upon receipt of each monthly statement. Lab offers a two percent
(2%) discount for payment in full by check within ten (10) days of statement date. Unpaid balances
over thirty (30) days from statement will be charged an additional 1.5% interest per month (18%
annually). Customer agrees to provide a valid credit card on account with Lab at all times and any
unpaid balances after 61 days will be automatically be charged to Customer’s credit card. If Customer
declines to provide a valid credit card on account, or if the credit card on file is not valid and payment
is rejected, all accounts will be classified C.O.D. and Customer agrees to pay all balances on C.O.D.
basis plus outstanding balances, if any.
Credit Cards: Visa, Mastercard or American Express are accepted by New Image Dental Laboratory.
C.O.D.: $10.50 Shipping Charge, $35.00 Charge on all returned checks.
PRODUCT REMAKE POLICY
All New Image products carry a 100% satisfaction guarantee for the appropriate term of each product.
This covers the full replacement cost of all materials and workmanship of the prosthetic produced
according to the dentist’s written prescription. When a removable product fits the model produced from
the dentists’ impression and does not fit intraorally, New Image shall not be responsible for the full
cost of refabrication. In this case, if all models and product are returned to New Image, the charge for
product remake will be 50% of the original price. If models are not returned to the laboratory, the cost of
remake will be 100% of the original price.

In order to give you quality service, New Image needs adequate time for fabricating, inspecting and delivering
your work. The schedule refers to full working days in lab Monday thru Friday. Pick-up and delivery days are not
considered working days. Pick-up and delivery service is available throughout metro Atlanta for a $4.00 fee per case.
Out of town cases should be sent to us by either Priority Mail and UPS Ground (at no charge) or United Parcel Service
(UPS) airpack (Second Day Service - Blue - $7.95, Next Day Service - Red - $9.95). All out of town cases are returned
via United Parcel Service (UPS) for a $6.00 per case fee.
