This Addendum shall be included in the contract for the above referenced project. All General,
Supplementary and Special Conditions, etc., as originally specified or as modified below shall apply to these

This Addendum consists of (46) pages. Attached documents include: (2) Sketches dated 07-26-16, (2)
Specifications Sections, Electrical, Mechanical, and Plumbing Addendum No. 2, and (14) Structural
Drawings and (1) Architectural Drawing with a Revision Date of July 26, 2016.

Wainscoting as shown in elevation 15/A4-41 and sketch SKA-03 as issued with Addendum
No. 1, to continue along wall in lieu of wood paneling for base bid. Second Floor Wood
Paneling shown, has been moved into Alternate No. 10. See changes to Proposal Form and
Alternates sections.

The undersigned, as bidder, hereby declares that the only person or persons interested in this proposal as principal or

satisfaction of Onslow County, and Smith Sinnett Architecture with a definite understanding that no money will
be allowed for extra work except as set forth in the General Conditions and the contract documents.
The low Bidder will be determined by the total cost of the Contract with the lump sum prices of the alternates
accepted being added to or deducted from the Base Bid to give the total cost of the Contract. Bidders are required to
give a price for Base Bid, all Alternates, and all Unit Prices as applicable to their Contract. All Bidders are required
to be licensed and in good standing with their respective North Carolina Licensing Board.

MINORITY BUSINESS PARTICIPATION REQUIREMENTS
Provide with the bid - Under GS 143-128.2(c) the undersigned bidder shall identify on its bid (Identification of
Minority Business Participation Form) the minority businesses that it will use on the project with the total dollar
value of the bids that will be performed by the minority businesses. Also list the good faith efforts (Affidavit A)
made to solicit minority participation in the bid effort.
NOTE: A contractor that performs all of the work with its own workforce may submit an Affidavit (B) to that
effect in lieu of Affidavit (A) required above. The MB Participation Form must still be submitted even if there is
zero participation.
After the bid opening - The Owner will consider all bids and alternates and determine the lowest responsible,
responsive bidder. Upon notification of being the apparent low bidder, the bidder shall then file within 72 hours of
the notification of being the apparent lowest bidder, the following:
An Affidavit (C) that includes a description of the portion of work to be executed by minority businesses, expressed
as a percentage of the total contract price, which is equal to or more than the 10% goal established. This affidavit

testing manufacturer's standard units in assemblies similar to those indicated for this Project:
Provide glazed aluminum curtain wall system that has the following capabilities based on
preconstruction testing:

Withstands loads and thermal and structural movement requirements indicated without failure.
Failure includes the following:

Air infiltration and water penetration exceeding specified limits.
b.
Framing members transferring stresses, including those caused by thermal and structural
movement, to glazing units.
Glazing is physically and thermally isolated from framing members.
System is pressure equalized at its interior face.
System is reglazable from the exterior.
Wind Loads: Provide glazed aluminum curtain wall system, including anchorage, capable of
withstanding wind-load design pressures calculated according to requirements of authorities having
jurisdiction or the American Society of Civil Engineers' ASCE 7-98, "Minimum Design Loads for
Buildings and Other Structures," 6.4.2, "Analytical Procedure," whichever are more stringent.
Basic Design Wind Velocity at the site is 125 mph. Height of window units above grade are indicated on
drawings. Minimum Design pressure is 40psf inward and outward.

Provide a minimum 1/8-inch clearance between members and top of fixed panels, glazing, or other fixed
part immediately below. Provide a minimum 1/16-inch clearance between members and operable
windows and doors.
Live Loads: Provide glazed aluminum curtain wall system, including anchorage, that accommodates
supporting structure's deflection from uniformly distributed and concentrated live loads indicated without
failure of materials or permanent deformation.
Air Infiltration: Provide glazed aluminum curtain wall system with permanent resistance to air leakage
through system of not more than 0.06 cfm/sq. ft. of fixed wall area when tested according to

maximum change (range) in ambient and surface temperatures without buckling, damaging stresses on
glazing, failure of joint sealants, damaging loads on fasteners, noise or vibration, and other detrimental
effects.

Testing Agency Qualifications: To qualify for approval, an independent testing agency must
demonstrate to Architect's satisfaction, based on evaluation of agency-submitted criteria conforming to
ASTM E 699, that it has the experience and capability to satisfactorily conduct the testing indicated
without delaying the Work.
Professional Engineer Qualifications: A professional engineer who is legally qualified to practice in
the State of North Carolina and who is experienced in providing engineering services of the kind
indicated. Engineering services are defined as those performed for installations of glazed aluminum
curtain wall systems that are similar to those indicated for this Project in material, design, and extent.
Installer Qualifications: Engage an experienced installer to assume engineering responsibility and
perform work of this Section who has specialized in installing glazed aluminum curtain wall systems
similar to those required for this Project and who is acceptable to manufacturer.

Engineering Responsibility: Engage a qualified professional engineer to prepare or supervise the
preparation of data for glazed aluminum curtain wall systems, including drawings, testing program

Do not modify intended aesthetic effects, as judged solely by Architect, except with Architect's
