That’s why we partner with Anthem BlueCross BlueShield (Anthem) to provide your medical benefits. Anthem gives us a broad network of participating doctors across the U.S. Anthem also helps people manage their conditions, stay on top of their treatment plans, and find great doctors. Future Moms ConditionCare Program Future Moms is a support program for expecting mothers. When you or a covered family member becomes pregnant, sign up for this program to be paired with a nurse who will help answer questions about pregnancy and support the expecting mother as she follows her healthcare provider’s care plan. Getting involved in this program is a great first step to a healthy pregnancy and safe delivery. Earn up to $400 (depending on when you enroll) in your HSA bank account and a Graco product for participating in this program. The ConditionCare Program gives you and your covered family members access to nurses, pharmacists, dietitians, doctors and other healthcare professionals to help manage long-term or chronic medical conditions like asthma, diabetes, chronic obstructive pulmonary disease (COPD), heart failure and coronary artery disease (CAD). You’ll see a link to the “I Got My Physical” form right on the home page. Remember, if you are covering a spouse/domestic partner, he/she will need to complete the form too. STAY HEALTHY REWARD Newell Rubbermaid will reduce your medical rates by $50 each month in 2016 for getting your physical — that means up to $600 more in your paycheck over the year! PRICE For example, let’s assume you hurt your knee while exercising and need to get an MRI.* For some medical procedures, like MRIs, there are no real quality differences—but there can be significant cost differences. Newell Rubbermaid gives every employee who enrolls in a medical plan up to $250 per year in his or her HSA. Here’s more information on the benefits of an HSA. HSA Features What is it? Who contributes to the HSA? Who cannot contribute? What can the money be used for? Can the money be invested? A bank account called a Health Savings Account (HSA) offered through HealthEquity, Anthem’s HSA partner ADVANTAGE If you are age 65 or older and enrolled in Medicare, or if you have health coverage under another medical plan that is not a high deductible plan, you can enroll in one of our medical plans, but neither you nor Newell Rubbermaid can contribute money into the HSA bank account tied to the plan. You’ll first have to meet the annual medical deductible associated with the plan you choose, then Newell Rubbermaid will pay coinsurance for these drugs (percentage shown below is what you pay; the Company pays the rest). Once you reach your out-of-pocket maximum, Newell Rubbermaid will cover 100% of your eligible prescriptions. FIRST you pay 100% for medications until your medical deductible is met. YOU PAY THEN you and Newell Rubbermaid pay coinsurance for these drugs (percentage shown is what you pay; Newell Rubbermaid pays the rest). Generic 30% Preferred brand 40% Non-preferred brand 50% Specialty Drugs (Oral or Injectables) << Back 30% (up to $250 per fill at retail) TAKE NOTE! GENERICS REQUIRED. Generic medications are a safe and easy way for you to save on healthcare. Generics use the same active ingredients and have the same effectiveness as name-brand medications, but at a fraction of the cost. Newell Rubbermaid covers many generics at 100%. If you choose a brand name drug with a generic alternative, in most cases you’ll be required to pay the difference in price – and that extra cost won’t count toward your deductible or out-of-pocket maximum.
