2010 Report to the Community Building a Better Future I n the four decades of our not-for-profit agency’s history, we have gone through many stages of development to enable Home Aides of Central New York to thrive in a changing world. Much of our success is due to the wisdom and vision of our founders who, in 1966, saw the need to give frail, aging and disabled individuals the support they needed to remain at home. Over time, we have been able to build upon that solid foundation to create a structure that has held up to the waves of change to continuously meet the needs of our community. Each decade, the agency has met the challenges of a new world: first to an era of increased government regulations; then to a period of changing organizational structures and payment systems; and now to a time where there are not enough resources to meet the needs of a rapidly growing aging population. To meet these latest challenges, our agency is moving into a world of automation – into a world where we need to be able to do more with less, to be even more efficient – while maintaining our commitment to quality and customer service. Once again, we will build our future on the solid foundation of our past to enable aging, ill and frail individuals to remain at home with dignity and independence. As our history has proven, the future success of our agency will come from our dedicated team of home health aides, staff, volunteers, and donors working together to make the vision of our founders a reality today. Martin President Home Aides of Central New York Eldercare Foundation Building a Solid Infrastructure M ajor projects abounded throughout all of 2010. In June, we underwent a construction project to repair the foundation of the 160-year-old home that houses our organization. With funding provided by a Community Development Block Grant from the City of Syracuse, the underground sewer line was replaced, and the crumbling foundation was repaired. The project allowed us to save the basement of the home, preserving an historic room in the building and expanding important storage space. Meanwhile, inside the agency, major technological initiatives were underway. After recognizing the inefficiency of a paper based system, we applied for several grants to fund the transition to an electronic patient recordkeeping system. In the fall, the first of two project phases began with the creation of an electronic nursing record. This record contains the patient Project: Sewer and Foundation Repair Funder: Syracuse Community Development Block Grant assessment and facilitates the generation of each person’s individualized care plan. The process of converting all of our nursing department’s clinical files from paper to digital format took approximately six months to complete. Laptop computers were purchased for the nurses, allowing them to replace a 38 page assessment form with a digital record. As a part of this project, we hired the agency’s first full-time Information Technology professional, a clear indication of a new digital age for the agency. Beyond the electronic nursing record system, a new web-based payroll system and new fundraising software were also incorporated into the agency, allowing for increased efficiencies in those areas, as well. “When I first decided to get an aide for my husband, I was very concerned about having a stranger in my house. We hire “only the best” – less than 10% of those that apply are accepted for employment. We also: • Check work history and references, conduct drug and alcohol screening and perform criminal background checks by fingerprinting all home health aides; • Observe and monitor their progress as they are trained in our 75-hour certified home health aide training program approved by the New York State Department of Health. T he recruitment and retention of home health aides is a top priority for our agency. As a result of an increase in the State’s aging population, the New York State Department of Labor lists personal and home care aides as the fastest-growing occupation in the Central New York region. At the same time, however, statistics show that the traditional care-giving workforce (women aged 25 to 44) is decreasing. Our agency recruits new home health aides every day of the week by holding Open House events, visiting employment agencies and attending job fairs at local organizations. Our “Neighbors Caring for Neighbors” program travels throughout the community to recruit home health aides. The mobile recruitment office is a place for interested candidates to learn more about home health aide work, submit an application and be interviewed. While recruiting new home health aides is always the norm, locally as well as nationally, retention of home health aides is a major issue. Due to a number of issues including low wages and limited benefits, the turnover rate for home health aides is much higher than the overall labor force. Typical turnover rates of home health aides are between 40 and 60 percent. In 2010, our staff employed fresh strategies to retain home health aides, including more immediate employee recognition for good work, easier access to a nurse when an aide calls, and increased followup on recent training graduates to ensure a smooth transition from training to field work. By implementing a number of these retention strategies, we lowered our turnover rate to less than 35% in 2010. Recognizing and presenting opportunities for our aides to feel like a part of our “team” are also important activities at the agency. At the Annual Aide Recognition Dinner we announce our Aide of the Year honoree. In November, we plan a Home Care Month “Thank You” Reception which is hosted by the office staff. Knowing he was cared for by the best helps me through my journey.” client’s wife Better Future T he future of Home Aides of Central New York is dependent upon increasing efficiencies and mastering new skills. To best serve all the individuals and their family members who need our help, we must dedicate all of our limited resources to finding, recruiting and retaining qualified and dedicated home health aides. Efficiencies don’t come without great cost, however. That is why the partnership with the Eldercare Foundation is so critical. Together, both organizations are committed to our common vision: A community where all individuals receive the love, care and support they need as they age. 2010 Highlights l Provided over 252,000 hours of support to 1,400 aging, ill and frail individuals in our community. l Helped over 950 callers with information and referral assistance. l Coordinated 144 respite stays for loved ones, giving family caregivers some relief from their round-the-clock responsibilities. This program is made available through the financial support of our local legislators and the State Office on Aging. l Saw a 6% growth in Philips Lifeline subscribers, serving a total of 460 clients and helping them reduce their risk when living alone. l Hired 54 home health aides, to meet the growing demand for these critical home care workers. l Trained over 40 home health aides in specialty care programs (Alzheimer’s disease, end of life, and mental health), resulting in 58% of the workforce being specialty trained. l Graduated one aide from our agency – supported LPN program. “We would like to thank you for coming into our Dad’s home and making him feel so very special.” client family members 2010 Financials Revenue Sources Expenses by Service Community Donations & Events 13% Government Grants and Fees 41% Other Revenue 1% Lifeline Services 2% Training 3% Respite Services 3% Fundraising 2% Aide & Nursing Services 90% Client Fees 30% Home Health, Hospice and Eldercare Agencies 15% Total Revenue = $6.61 million Total Expenses = $6.56 million Our Services Nursing Service A registered nurse works with family members to determine the services needed, develop a care “Your aides brought joy and a high level of caring into our home. When they came, I could do my errands in peace because I knew Mom was in such good hands.” client’s daughter plan for the aide to follow, monitor the condition of the person receiving the service, supervise the aide activities within the home, and provide updates to family members and physicians. Home Health Aide Service Certified Home Health Aides provide assistance with daily living activities such as bathing, dressing, other personal care, and medication reminders to help keep someone at home. Homemaker Service Household service includes tasks such as grocery shopping, meal preparation, laundry, and light housekeeping. Companion Service Periodic visits by a companion aide can provide periodic supervision and cheer older individuals, giving family members the reassurance that a caring person is checking in on the situation. Caregiver Respite Service We offer full-time caregivers a break from their round-the-clock responsibilities: In-home aide service enables caregivers to leave home for a short break each week, or to leave home for more than a day at a time. Temporary out-of-home placement in an adult home (assisted living facility) or nursing home enables caregivers to leave for a planned break, such as a vacation, or scheduled surgery. Emergency respite due to sudden illness, death or other unexpected circumstance is also available. Lifeline Medical Alert Service ® A medical alert service enables a person to reach friends and family for help 24 hours a day, with just the push of a button. Our agency is an authorized Phillips Lifeline® Medical Alert Service provider. The Eldercare Foundation T he Eldercare Foundation was established to help support the programs and services of Home Aides of Central New York. We raise funds through special events, annual giving campaigns, memorial/honor gifts and planned gifts. The Eldercare Foundation has raised over $1.66 million in cash donations and another $350,000 in grants for Home Aides of Central New York since its formation in 2000. In 2010, we received our largest gift to date – a million dollar bequest from the estate of John Gallinger. As the first Chair of the Eldercare Foundation board of directors, John’s vision for the Foundation was that it could be a “force for good in the field of eldercare.” In a letter penned to us in 2005, he wrote his thoughts on how our community needed to make eldercare a priority, and how the foundation could “put the wheels in motion to enlarge the pool of caregivers and to improve their skills.” We believe John would be pleased to see how far the Foundation has come since its founding in 2000.
