Our board of directors, staff, and volunteers are honored to present our 2013 Annual Report to our Community. For
over six years Katy Trail Community Health has enriched and helped sustain the lives of thousands of people in West
Central Missouri. We are extremely proud to continue our important work.
2012 was a year filled with continued strong growth and uncertainty in both state and federal healthcare policy.
The Patient Protection and Affordable Care Act (ACA) was largely upheld by the US Supreme Court, and healthcare
organizations throughout the country began the long and complex process of implementing the components of the
ACA. Katy Trail Community Health, and all Federally Qualified Health Centers throughout the country took the lead
in transforming our practices by adopting the Patient Centered Medical Home model of care. Katy Trail received
recognition in December 2011 for our Warsaw and Sedalia Sites, and we will seek recognition for our Versailles site in
2013.
Our dedicated staff of clinicians and support staff served almost 14,000 people at our three locations. This reflects a
19% increase over 2011. Our greatest growth happened at our Versailles location where in our first year of operations
we served almost 2,000 people. Our dental program grew dramatically as well with 6,522 people accessing oral health
services, almost 1,200 of whom received emergency dental services. As you will see in this report, KTCH employees
are changing lives. They are the unsung heroes of our organization.
In partnership with Pathways Community Health, Care Connection for Aging Services, the Warsaw Senior Center
and the entire Benton County Community, we initiated construction on Harbor Village, a 24,000 square foot space
dedicated to providing integrated primary care and wellness services. The financing for the project came through
a variety of sources including three grants from the Missouri Department of Economic Development Neighborhood
Assistance Program, the Health Resources and Service Administration Capital Development Grant, a loan from Third
National Bank in Sedalia, local fundraising, and “last mile” financing through the New Markets Tax Credit program.
Grand Opening is expected to occur in late summer 2013.
Our dental program will also expand in 2013, with the implementation of a school-based oral health program in the
regional school districts in Morgan, Henry, Johnson and Saline Counties.
These are challenging times, but we are prepared to continue with our commitment to our patients, our community
and our organization. Through an extensive strategic planning process, we have renewed our commitment to
providing high quality care and service. We are expanding and renovating our facilities, offering additional services to
our patients, and providing exceptional customer service.

Initiated a partnership with UMKC School of Dentistry to provide community-based
clinical experiences to dental students.
Strengthened our ability to manage patient care by accessing patient discharge
summaries from both Bothwell Regional Health Center and Golden Valley Memorial
Healthcare.
Began offering psychiatry services at our Sedalia site through a partnership with
Pathways Community Health.
Implemented a direct admission process with Bothwell Regional Health Center.
We opened a dental clinic at the Morgan County R-1 and R-2 Schools in Stover and
Versailles.
Provided over $100,000 of dental services to adults in the region through our voucher
program.
Provided almost $1.4 million dollars of free or reduced cost medication to our patients.
Supported four employees who received their Health Information Technology
certification.
Supported two employees who received their certification in medical and dental coding.
Received a clean audit resulting in Katy Trail Community Health holding a low risk status.
