This pharmacy directory is for your geographic area. All network pharmacies may not be listed in this
directory. Pharmacies may have been added or removed from the list after this directory was developed. To
get current information about network pharmacies in your area call Customer Service at 1-888-556-6648,
TTY 711, 8 a.m. to 8 p.m. local time, 7 days a week.
Beneficiaries must use network pharmacies to access their prescription drug benefit. Benefits, formulary,
pharmacy network, premium and/or copayments/coinsurance may change at the beginning of the plan year.
Plan is insured or covered by UnitedHealthcare Insurance Company or one of its affiliates, a
Medicare-approved Part D sponsor.

This directory provides a partial list of UnitedHealthcare MedicareRx for Groups network pharmacies.
To get a complete description of your prescription coverage, including how to fi ll your prescriptions, please
review the Evidence of Coverage and Formulary (Drug List).

8 a.m. to 8 p.m. local time, 7 days a week.
We call the pharmacies on this list our “network pharmacies” because we have made arrangements with
them to provide prescription drugs to Plan members. A network pharmacy is a pharmacy where members
obtain prescription drug benefits provided by the Plan. In most cases, your prescriptions are covered by the
Plan only if they are fi lled at a network pharmacy or through a network mail order pharmacy service.
Once you go to one, you are not required to continue going to the same pharmacy to fill your prescription.
You can go to any of our network pharmacies. We will fill prescriptions at non-network pharmacies under
certain circumstances as described in your Evidence of Coverage.

90-day supply of your prescription drugs. Generally, it takes mail service pharmacies seven days to
process your order and ship it to you. Please be aware of this turn-around time, and request refills
early enough to leave time for your refills to be processed and sent you. You should fill your
prescription at a local network pharmacy if you are out of your medication and cannot wait for your
mail service prescription to arrive. Please refer to the pharmacy listing on the following pages for
