Senator Brewer has served the Commonwealth since 1988, when he was first elected to the House of Representatives, and he has served in the Senate Chamber since 1996. His Senate district is the second largest in the state, covering 28 cities and towns in Worcester, Hampden, Hampshire, and Middlesex Counties, and the senator has been a staunch and tireless advocate for these communities and a leader on fiscal matters. He has worked on behalf of veterans, protected the environment and agriculture, supported higher education, and always kept the needs of families and elders first and foremost in his mind and actions. He persistently advocated for changing the name of the Department of Mental Retardation to the Department of Developmental Services, which reversed an antiquated stigma and promoted dignity and respect for everyone. He sponsored legislation that charged the Executive Office of Health and Human Services to create, in May 2013, the Commission on Rural Access and Improving State Sponsored Services in MA Rural Communities. During his many years in the legislature, Senator Brewer has supported the care of elders in their homes and community. As chair of the Senate Ways and Means committee, he balanced the state’s fiscal reality with many competing needs. FY14 brought an opportunity to restore some of the cuts that began in 2008. Under Senator Brewer’s leadership, the FY14 budget provided important investments in the area of health and human services, including increased services at senior centers. The FY14 budget eliminated the wait list for home care services, allowing more elders to stay in their homes, avoiding costly nursing home placements, saving public expenditures, and improving the quality of life for many. Our sincere gratitude goes to Senator Brewer for all his work, advocacy, and service to the district and Commonwealth for more than two decades. Greetings! FCHCC is fortunate to collaborate with many partners in order to help elders, persons with disabilities, and caregivers. We recognize and appreciate the collective effort and commitment that makes our organization strong. This milestone has us looking back through old records and photos to remind ourselves of our history and purpose. Over the past 40 years, we have remained true to that mission. FCHCC is a nonprofit corporation—designated as an Area Agency on Aging—that develops, provides, and coordinates a range of services to support the independent living of elders. We work in Franklin County and the four Worcester County towns of Athol, Petersham, Royalston, and Phillipston. We started with a modest beginning of less than $100,000 in federal funding. We also began with community support, from the County Commissioners and volunteers who gave their time, to collaborations with local efforts that were under way. Over the years, services to persons with disabilities and support to caregivers and grandparents raising their grandchildren were added because of the commonalities with our mission to provide home and community support. Our geographic area has also grown to encompass Hampshire, Hampden, and Berkshire Counties for some services. We are all part of a family and seek the assistance and support of family, friends, and our community when life presents its challenges. FCHCC has been, and will be, there to answer questions; sort out complex processes for applications; provide health insurance information, education, and counseling; offer home and community options; and provide opportunities for healthy living, volunteer engagement, and making choices best suited to one’s situation. We are proud of what we have brought to the community and look forward to its continuation. We challenge ourselves each day to meet the changing needs of those we serve, and we hold ourselves accountable to honoring self-determination and choice as well as collaborating with families and community partners. FCHCC was founded in 1974 with the understanding that the growing elder population would have expanding needs. Here we are, facing that growing demographic, understanding that we must continue to partner and provide linkages from home to healthcare settings to collectively provide quality and cost-effective care. Our volunteers are more important than ever in order for us to succeed and be efficient and help us to bring peace of mind to the persons and families we touch. We look forward to meeting the challenges of the decade ahead, seeking opportunities to expand options and meet people where they are in order to fulfill their wishes to be at home and be part of their community. FCHCC | 2014 Annual Report 5 Looking Back, Moving Forward FCHCC celebrates its 40th anniversary by taking a look back. July 22, 1974 1976 1978 FCHCC was incorporated as a nonprofit corporation to provide home care programs for Franklin County, plus the four Worcester County towns of Athol, Petersham, Phillipston, and Royalston, for persons over 60 years of age. We offer assistance in the form of hot meals, nutrition, medical services, legal services, nursing services, homemaking, social services, and other services that prolong the life and well-being of older persons. FCHCC began to administer the senior meals program. The agency conducted its first Area Agency Needs Assessment Survey. 724 elders received in-home support at an average cost of $468 per elder, per year. October 1, 1975 FCHCC was the first home care corporation in the Commonwealth to be designated as an Area Agency on Aging. FCHCC assumed the administrative role with a smooth transition and no disruption to the meals service. 6 FCHCC | 2014 Annual Report 1974–1984 1980 1983 The first three-year Area Agency Area Plan was developed from public hearings in Orange and Greenfield and public forums in Shelburne, Deerfield, and Greenfield. The agency was designated as the mandated protective services agency to investigate elder abuse and neglect. “It was a strange and awesome experience . . . no one knew what was ahead. It was the beginning of becoming familiar with acronyms, entitlements, strange designations and account numbers, and also having to deal with two budgetary years—the Commonwealth and the Federal fiscal years.” —FCHCC’s first board of directors 1982 1984 Due to federal budget cuts and the uncertainty of funding for elder nutrition programs, the first Meals on Wheels fund drive was held, raising over $5,000. Two new programs launched: Adult Family Care began accepting its first clients, placing them in the homes of “host families,” and Nursing Home Ombudsman, now called the Long-Term Care Ombudsman program, began to serve residents of area nursing and rest homes. Did you know? Beginning in 1982, “The Good Life” radio show and “SeniorGram” radio spots aired on local stations, with spots even airing on stations in Louisiana, Indiana, Pennsylvania, and Connecticut in 1985. By 1986, the “SeniorGram” radio spots were heard on 86 stations in 26 states. FCHCC | 2014 Annual Report 7 1985–1994 1986 1990 For the first time, more home-delivered meals (73,000) than congregate meals (69,000) were provided. Due to the state budget, the agency was forced to cut clients from its rolls for the first time in its history: 72 “less frail” clients were removed from the caseload. Robert Harrigan, home-delivered meals recipient, receives his noontime meal from a volunteer driver at his home in Turners Falls. 8 1988 1989 Farmer’s market coupons were made available to elders in the area for the first time—elders were able to purchase fresh food and vegetables at the Greenfield Farmer’s Market on Saturdays during the summer months. Severe cutbacks in the State Home Care program decreased services to clients by an average of 18%. FCHCC | 2014 Annual Report Did you know? In June of 1991, FCHCC said goodbye to its original home at the old Central Street School in Turners Falls and moved to its current location into what had been Farren Hospital at the Farren Care Center. 1991 The mission of the agency officially expanded by vote of the board of directors to include providing services to other populations in addition to elders, such as persons with disabilities. The agency became the first home care corporation in the state to receive approval to provide Personal Care Attendant services. Morgan Allen House in Greenfield and Winslow Wentworth House in Turners Falls opened as congregate housing facilities to offer supportive living situations. As a Money Management program volunteer, Elaine Pepin of Northfield provided one-on-one assistance to elders who needed help managing their finances. 1994 The Money Management Program formed and began training volunteers to assist elders in managing their finances by helping with bill paying, check writing, budgeting, and more. The Winslow Wentworth House is named after the first president of the founding board of directors. FCHCC | 2014 Annual Report 9 1995–2009 1995 1996 1999 FCHCC worked cooperatively with several organizations concerning access to health care. The State Department of Public Health formed Community Health Network Areas (CHNAs) across the state. The Franklin County/North Quabbin CHNA chose “immunization” as its initial project. The Community Health Center Planning Group was provided space within FCHCC’s office and received a Medicaid Access Grant to help determine the need and feasibility of establishing a community health center in Franklin County. The Aging Services Access Points (ASAPs) bill was passed, establishing home care corporations across the state as ASAPs to serve as one-stop shopping for older consumers in need of state-funded, long-term care services. In the agency’s 25th year, 45 programs continued to grow at a record-breaking pace, with the Home Care program serving 954 clients and local elders dining on 170,000 congregate and home-delivered meals. July 1997 2001 FCHCC was designated as the lead agency for the SHINE health insurance counseling program in Franklin and Hampshire Counties. In conjunction with local Councils on Aging, the agency recruited and trained new SHINE volunteers in order to provide health insurance information to elders in both counties. The Older Americans Act was reauthorized in 2001, adding the National Family Caregivers Initiative and funding. Planning began to tailor services to caregivers. Did you know? FCHCC was the first Area Agency on Aging or Aging Services Access Point in Massachusetts to have its own website: www.fchcc.org launched in 1997. 10 FCHCC | 2014 Annual Report 2003 Community Choices launched, allowing those needing skilled nursing facility placement to stay in the community and to help those currently at a skilled nursing facility transition back. 2006 A diversity committee was formed through collaboration with the LGBT Aging Project to help us stay aware of diversity in our service area. June: A well-attended breakfast brought together local LGBT elders in dialogue. The first LGBT breakfast took place in the conference room at FCHCC. Since then, the agency has hosted quarterly LGBTIQA socials at venues throughout the region. 2008 2009 FCHCC was prominently featured in a five-part series on retirement in The Recorder. With funding from the Community Foundation of Western Massachusetts, the agency produced the first Home Repair Guide, which helps elders, caregivers, and persons with disabilities learn how to find a handyman, enter into a contract, and find funds to pay for repairs so they can stay in their home as long as possible. June 15: FCHCC and the local community came together for World Elder Abuse Awareness Day, with more than 30 coffee shops and restaurants displaying informational table tents in their establishments to remind people that everyone has a right to be safe. FCHCC | 2014 Annual Report 11 2010–2014 Did you know? In 2008–2009, FCHCC made an effort to “go green.” In 2008, the agency partnered with The Recorder to print and distribute The Good Life within the regional newspaper, nearly eliminating the mailer format that had preceded this change, increasing capacity for email distribution, and increasing circulation to 24,000. The following year, the staff initiated an internal effort to conserve resources by recycling paper, plastic, and glass, and taking online workshops to save on travel expenses and gas. 2011 With a grant from Tufts Health Plan Foundation, the Healthy Aging Program, now known as Healthy Living, launched, attracting more than 100 participants and training 63 peer leaders in leading free community workshops on chronic disease self-management, healthy eating, walking groups, and falls prevention. 2010 While budget cuts placed elders on a wait list for state home care services, FCHCC worked to help elders manage financial challenges. Foundational and grant funding was secured in order to train volunteers in providing benefits counseling and application assistance to elders. The Healthy Living program has continued expanding and has added programs in Diabetes Self-Management, Tai Chi for Healthy Aging, and Chronic Pain Self-Management. 12 FCHCC | 2014 Annual Report December 12, 2013 2014 FCHCC participated in the second annual Valley Gives Day, a regional day of online charitable giving hosted by the Community Foundation of Western Massachusetts, raising $5,186 from 71 donors and a “golden ticket.” Rides for Health launched, offering doorthrough-door assisted transportation for Home Care clients. FCHCC provides home and community programs for the area known as Franklin County, plus the four Worcester County towns of Athol, Petersham, Phillipston, and Royalston for elders, persons with disabilities, and caregivers as the Aging Services Access Point (ASAP) and AAA. FCHCC also operates programs in Hampshire, Hampden, and Berkshire Counties. We are committed to: • Preventing or postponing the need for institutional care • Facilitating the return home after an institutional stay • Advocating for elders and for persons with disabilities • Funding local projects, including Legal Services, Caregiver Services, and Health & Fitness programs • Providing support to local Councils on Aging • Diversity We have been providing family support and home and communitybased services for elders and persons with disabilities living in Franklin County and the North Quabbin areas since 1974. Who We Serve For forty years, FCHCC has provided home and community services to elders. We work with families who provide support to loved ones. In Franklin County and the North Quabbin, more individuals live in their own homes rather than apartments due to the rural character of our area. The care needed at home has gotten more complex, and typically, consumers are on multiple medications and have more than one chronic condition. Generally we serve persons in their late seventies and eighties but also work with individuals in their early sixties and upper nineties. Every situation is different and care plans are tailored to one’s needs and change over time. More individuals have dementia or other cognitive issues. People are able to be supported with direct care, assistive equipment, technology, and services that come to their home. About 40% of the elders we serve are medically eligible to be in a nursing facility. As people live longer, their financial situation may change and they may need assistance to stretch their resources, maintain their home, and cover their basic needs, housing, medical expenses and prescriptions. The Commonwealth of Massachusetts has a strong tradition of home and community services. Social work, nursing, and resource consultants work as a team to help navigate options, resources, and services that are available at home and in the community.
