Case from Dr. Iñaki Gamborena
Our patient, a 42-year-old woman, came to us
with a failing crown. Following bone and soft
tissue evaluation, we decided the appropriate
treatment solution would be to place a dental
implant under the Immediate Function™ protocol
and use the Platform Shifting concept to preserve
the maximal amount of soft tissue and bone.
After crown removal, no sound tooth structure was found

Procera® Abutment Zirconia
Indications
• Single- or multiple-tooth implant restorations
• Cement-retained
1. If applicable, remove healing abutment or temporization.
2. Ensure that the implant interface is free from any soft-tissue
or bone remnants. Place Adapter PS RP-NP into RP implant or
Adapter PS WP-RP into WP implant. Check that the lobes on
the adapter are fully engaged in the grooves of the implant.
Note: Ideally the Adapter PS should be placed the same day
as the implant placement, in order not to disturb the Soft Tissue
Integration™ and minimize the bone resorbtion.
Impression
3. Place the Impression Coping PS RP-NP or Impression
Coping PS WP-RP onto the implant. Use the Screwdriver
Unigrip™ and tighten the screw.
4. Take an implant level impression using the closed tray
technique as described in manual the NobelEsthetics™
Procedure & Products manual.
5. Connect suitable Healing Abutment PS RP-NP or Healing
Abutment PS WP-RP on top of the adapter using Screwdriver
Manual Unigrip™.

6. Healing Abutment PS.
7. Dispose of the abutment screw delivered together with the
Procera® Abutment Zirconia Bmk Syst. Use Abutment
Screw PS RP-NP (#34553) or Abutment Screw PS WP-RP
(#34554) instead.
Note: If preparation is needed, make sure that the thickness of the
ceramic material is at least 0.8 mm. This thickness limit is applicable
up to a height of 3 mm above implant level. Use the high-speed
turbine with diamond drills and copious water irrigation.
8. Place the Procera® Abutment Zirconia onto the implant.
Use the Screwdriver Unigrip™ and tighten the abutment screw.
Note: A radiograph can be help full to confirm accurate seating of
the abutment.
9. Tighten the abutment screw to 35 Ncm using the
Manual Torque Wrench Prosthetic and Screwdriver
Machine Unigrip™.
10. Continue prosthetic procedure as described in the
NobelEsthetics™ Procedure & Products manual.

1. Model
• An implant level impression is received with an Impression
Coping PS.
• Confirm the position of the implant level impression coping
and Implant Replica PS assembly in the impression.
• Pour and fabricate a soft tissue master model.
2a. Regarding Procera® Abutment Wax-up
• Create the abutment in a pattern resin or a hard wax. Use a
temporary component as a foundation for the abutment pattern.
– For Implant Replica PS WP-RP,
use Temporary Abutment Plastic Engaging Bmk
Abutment Screw PS WP-RP.
– For Implant Replica PS RP-NP,
use Procera® Wax-up Sleeve PS

• You must respect certain dimensions in the waxed framework
depending on which Procera® product you wish to fabricate:
– For ceramic abutments NP,
maximum dimensions are 4 mm × 12 mm
radius × height (r × h).
– For ceramic abutments RP/WP,
maximum dimensions are 5 mm × 15 mm (r × h).
2b. Regarding Procera® Abutment CAD
To facilitate correct abutment angulations and measurements,
insert a T-bar into the replica. It is contained in the Procera®
Abutment 3D CAD Kit and will help you to feed correct data
into the Procera® Software 3D CAD Design application.

3. Scanning
Note that the scanning procedure is only necessary for Procera®
Abutment Wax-up.
• Open the Procera® Software and follow the instructions in the
tutorial for:
– Creating an open order.
• Scan the die using the Procera® Scanner according to the
tutorial found within the software.
Note: Use the ordinary BmkSyst Lab Screw to attach the wax-up
pattern in the implant platform replica
• Follow instructions in the tutorial for:
– Using the Procera® Software CAD Design Application.
– Dispatching an order.
4. Concluding Procedure
After delivery of the Procera® Abutment from the Nobel Biocare
production facility:
Ceramic Abutments
• If necessary, make minor adjustments with diamond bur
or flex disc with fine grit size under low pressure and with
copious water irrigation.
• When designing/preparing Procera® Abutment Zirconia,
make sure that the thickness of the ceramic material is at
least 0.8 mm. This thickness limit is applicable up to a height
of 3 mm above implant level.
• Sandblast using two to four bars of pressure utilizing
110 µm – 250 µm aluminum oxide, at an approximate
distance of 10 mm.
• Clean in an ultrasonic unit.

Cleaning & Sterilization
Sterile Components
The devices delivered sterile have a “Sterile” marking on the
label. Opened packages of components that have never entered
the oral cavity of a patient may be cleaned and steam autoclaved
again following the established procedures of your local hospital
or clinic for similar products.
Non-Sterile Components
Modified abutments and restorations from a dental laboratory
are cleaned and sterilized (if indicated) according to generally
accepted procedures for dental laboratory work. Procera® Esthetic
Abutment Selection Kit and Procera® Esthetic Abutments are delivered non-sterile. The Procera® Esthetic Abutments and the Procera®
Esthetic Abutment Selection Kit can be autoclaved according to
standard routines either as separate units or when the abutments are mounted in the kit.
Instruments
Instruments are to be cleaned and sterilized before intra-oral use
in compliance with established routines at the hospital/clinic.
Manual Torque Wrench Prosthetic
135˚C/276˚F
Clean the parts thoroughly. Allow them to dry completely.
Sterilize the instrument using a steam autoclave at 135° C for
minimum hold time of 5 minutes or according to recommendations from the manufacturer of the autoclave.
