Mission Statement
Kentucky River Community Care, Inc., (KRCC) is a private
nonprofit Community Mental Health Center dedicated to
improving the health and wellbeing of the people of our
region. We help individuals and families in the eight counties
of the Kentucky River region by providing mental health,
developmental disabilities, substance abuse and trauma
services. KRCC seeks to promote public safety, boost economic
wellbeing and improve community and individual quality of
life.
Since 1969, KRCC has been addressing health issues by
identifying needs, bringing people together, forming
community partnerships, identifying resources to fund
solutions and seeking changes in the overall health
environment. KRCC has a team of over 536 experienced and
caring service providers in 58 operating sites across all eight
counties.
KRCC provides client centered care designed to ensure
that each individual gets the care and attention they need,
in a manner that is sensitive to their desires. Our services
include: assessment, referral, individual and group therapy,
consultation, education, residential treatment, aftercare and
case management. Our services are confidential and our
priority is to serve our clients, care for their health and honor
their individual rights. Transportation can even be arranged
for many of our programs.
We collaborate with local organizations on innovative
solutions, to pool our resources to serve the region. Our work
often influences local, state and national policy. KRCC also
consistently reinvests in the area through its reliance on local
vendors and service providers.

Service Information
Kentucky River Community Care, Inc. (KRCC) operates a variety of programs
and facilities in the eight counties of the Kentucky River Region.
There are two primary ways to navigate our system of care:
CRISIS CARE:
If you are experiencing an emergency, you can access our 24-hour
Crisis/Information Line to get immediate care 1-800-262-7491, or
our 24-hour sexual assault response program 1-800-375-7273.
SUBSTANCE ABUSE & MENTAL HEALTH CARE:
In non-crisis situations, the best way to access our professional
services is by contacting the 0VUQBUJFOU$MJOJDT in your County.
We provide individual, group, family therapy and medication
management services through licensed or credentialed
professionals. Referrals and support through other agencies will be
made as needed. Contacting the Outpatient Clinic in your county
is the first step toward finding the care you need.

To help prepare you for your first visit, which will last about an hour, here
are the steps in the process:
Check In with the Customer Service Representative.
He/She will ask for your name and verify your appointment.
Complete Forms that will give us some
information about you and any medical issues related to your visit.
If you need help with these forms, please let the Customer Service
Representative know. You will also be given information about our
confidentiality policy and material about the services we offer.
Come On In a Customer Service

Representative will ask you to come into an office. This private
conversation is to develop a financial demographic and address
key billing information. Remember that KRCC is committed to
providing care to everyone who seeks it.
How Do I Pay? KRCC accepts Medicaid, Medicare and most
private/commercial insurance. A sliding fee scale is available for those
without insurance who meet the eligibility requirements and provide
proof of income. We have staff who are available to assist you in obtaining
health insurance through the KY Health Benefits Exchange, if you are
eligible. We want to make certain that concerns about how to pay for
medical care are addressed before you meet with the therapist.
That’s it, you’re ready to go! You will then return to the
waiting room until you are called to see the therapist. The therapist will
show you to his or her office and talk with you about the purpose for your
visit. S/he will talk to you about your options and the services we offer that
will best meet your needs.

How To Make A Referral
Deciding to make a referral for a family member or a friend is a big
step. It is also a skill that requires tact, sensitivity and at times firmness.
Your attitude of sincere interest and helpfulness toward the person in need
is most important.
You should be aware that many people, even if they ask you for your help,
are embarrassed and uncertain about asking for help. One way that you
can help is to provide support, address their fears and try to point them in
the right direction.

It is usually best to be frank with the person and to tell them that you
are going to call KRCC and recommend an appointment. If you are
comfortable with the idea, offer to accompany the person to the first visit
and call to make an appointment that is convenient for each of you, or
encourage them to make the call for the appointment.

An appointment can be scheduled at the time of the call. Referral
information and requests for appointments may also be faxed to the
corresponding outpatient office.
Releasing Your
Client Information
Referral sources, concerned friends and family members frequently
want to participate in the ongoing care a person receives. To facilitate
good communication, yet assure privacy is maintained, clients need to
sign releases of information identifying what type of information can
be shared about their care, and who can receive this information.

A toll-free, 24-hour telephone intervention and crisis response system is
available throughout the region.
THE RISING CENTER
1-800- RAPE (7273)
The Rising Center provides support to survivors
of all forms of sexual violence: sexual assault,
sexual abuse and sexual harassment. These
services include a 24-hour crisis line, medical
advocacy, legal advocacy and individual, group,
family and/or group counseling. The Rising
Center also offers community education and a
volunteer program. The Rising Center is located
at 437 Morton Blvd. in Hazard, Kentucky.
CROSSROADS CENTER
Crossroads Center is a 24-hour facility for adults
and children that specializes in evaluation,
assessment, and de-escalation of crises and can
help make connections to community resources.
Crossroads is located at 118 Willies Way in
Hazard, Ky.

THE SEWELL CENTER
** 24-hours a day 7 days a week **
Provides assessment, intervention, and stabilization services for families
whose youth are identified with an emotional/behavioral crisis that exceeds
current resources and/or are currently at risk of psychiatric hospitalization
or out of home placement. The Sewell Center also provides consultation
and education to community agencies regarding specific cases, crisis
intervention techniques and program planning. Service coordination to
children and their families who are in crisis is also available. The Sewell
Center is located at Hwy 15 N in Jackson, Kentucky.

Addiction Services
KRCC is a recognized community partner that provides a liaison between
individuals, the court system, law enforcement and other community
partners dealing with substance abuse.
DUI PROGRAM
Assessments include a clinical interview, Kentucky Assessment
Battery, 20-hour pre-treatment, outpatient therapy (group and
individual) and referral to residential treatment, if a higher level of
care is needed or another program is requested. DUI services are
offered at each KRCC Outpatient Office. Referral by the courts or
other DUI program is required.
DRUG COURT PROGRAM
These services include assessment, referral, individual and group therapy,
consultation, education, residential treatment, after-care and case
management for individuals determined eligible for Drug Court Services.
Referrals must be received from the court officer, usually following alcohol/
drug related charges or testing positive on a drug screen.
EARLY INTERVENTION PROGRAM
This program targets youth under the age of 21, who have become
involved with the courts due to violence and/or alcohol, tobacco or
other drugs, or driving while impaired. The mission of the program
is to educate adolescents to make low risk choices, improve family
relations and reduce the recidivism rate of first and second time
juvenile offenders.
KIDS NOW Plus
Helps pregnant women find the assistance they need to have a happier,
healthier pregnancy and baby. Our Enhanced Care Specialist understands
how stressful pregnancy can be, and is there to listen and to help find
the assistance you and your baby need. You can see a KIDS NOW Plus
Enhanced Care Specialist at any time during your pregnancy and for up to
two months after delivery.

REGIONAL PREVENTION CENTER
The KRCC Regional Prevention Center provides consultation, training,
education and technical assistance to community members, service
providers, church and civic groups, in the planning and implementation
phases of substance abuse prevention strategies.
The Resource Library at the Prevention Center offers an extensive selection
of materials and curriculum on substance abuse prevention and treatment,
violence prevention, conflict resolution and other mental health related
issues.
The Regional Prevention Center provides

The Prevention Resource Library is open
from 8 am to 4:30 pm, Monday through
Friday, and is located off of HWY 15 N at
Grapevine Road in Hazard, Kentucky.

Children’s Services
Individual, group, family therapy and medication management services
are provided for children, adolescents and adults. Licensed professionals
(including psychiatrists, psychologists and clinical social workers) are
available at KRCC clinics, schools and other designated sites within the
community. Parent and child support groups, art and expressive therapies
are also available.
KRCC conducts comprehensive psychological assessment for treatment
planning and diagnostic clarification. We also provide evaluations to
respond to court orders to assess competency and dangerousness or to
assist with placement in foster care, for residential treatment, etc...
CHILDREN AND ADOLESCENTS
SCHOOL-BASED THERAPY
School-Based Therapy is a service provided by a KRCC SchoolBased Therapist and delivered at a particular school. This
program offers the majority of therapeutic services for youth
regarding mental health and/or substance abuse issues, within
the school they attend such as; attending to mental health
emergencies, working with families and other agencies to meet
the needs of the youth, referring clients to other services as
necessary, reporting abuse and neglect when necessary to the
Dept. of Community Based Service Division of Protection and
Permanency.
CHILDREN WITH SEVERE EMOTIONAL
DISABILITIES (SED)
Family network facilitators are available to all parents with children that
have a disability. The facilitator provides families with individual and
group support, training, information and advocacy. These services may be
provided within the home, the school or other community sites convenient
for the families.

EARLY CHILDHOOD MENTAL HEALTH
PROGRAM
The Early Childhood Mental Health Program was created
to promote the social and emotional development of young
children ages birth to five. Early Childhood Mental Health
Specialists are employed by the Regional Community Mental
Health Centers across the state to provide a variety of services
to children and their families, as well as other agencies that
serve young children. We provide assessments of mental health
needs of children, age birth to five currently within the region.
We provide therapeutic treatment services to children and/
or their families. We collaborate with other service providers
to assess the needs of children and their families. We foster
community awareness of the need for mental health services.
We provide outreach and instructional programs to caregivers
and families through public and private child care centers and
other organizations/providers.
IMPACT
(Interagency Mobilization Program for Adolescent and Child Treatment)
provides resource development and service coordination to children and
their families who have been accepted into the IMPACT program by the
Regional Inter-Agency Council. The program provides support to a child
in the least restrictive environment possible and establishes a community
support system.

SUMMER SPLASH
KRCC also offers an intensive summer program of therapeutic
interventions, social skill practice activities and learning opportunities for
children and youth with severe emotional disabilities.
RECLAIMING FUTURES
Works to ensure an integrated system of care
for youth involved with, or who are at-risk
of, involvement with the courts because of
substance abuse through a grant funded by
the Robert Wood Johnson Foundation. The
program works to provide links between
the legal community, service providers and
community partners.
THE SEWELL CENTER (Focus Recovery Center)
Provides residential treatment for male and female children ages 2-18.
Services include individual, group and family counseling, alcohol/drug
education, relapse prevention, recreation, referrals and case management.

Adult Services
DAY TREATMENT
KRCC provides a Therapeutic Rehabilitation (TR) program for people with
serious mental illness (SMI), who require less than 24 hours of care a day
but more than outpatient counseling. Services are designed to promote
social, personal adjustment and daily living skills. Adults in the program
are encouraged to retain control of their lives, to set their own goals and to
participate fully in decisions affecting their future. TR Centers are located
in Breathitt, Letcher, Wolfe, and Perry Counties.
CASE MANAGEMENT
KRCC provides Case Management services to assist consumers with their
medical, education, social and other support needs. Recipients must be at
least 18 years of age, have an Axis I Diagnosis of severe mental illness and
not be in immediate danger to themselves or others. They must not be
actively using substances.
MOVING BEYOND DEPRESSION
This program is a home visiting program providing In-Home Cognitive
Behavior Therapy for pregnant women, or women up to 18 months post
partum with a diagnosis of Major Depressive disorder. All clients are
referred by the HANDS home visitors with the local health departments.
This program allows a licensed, Masters level therapist to provide 15
sessions of In-Home CBT to women who are suffering from maternal or
post-partum depression. Sessions are 1 hour per week for 15 weeks, with a
Booster session 1 month later.
PROJECT A.D.D.V.A.N.C.E.
Offers an intensive outpatient program of comprehensive
treatment services to women who have experienced domestic
violence, substance abuse, childhood abuse or other trauma. The
program serves women who may not need, or who are stepping
down from, hospitalization and require more intensive care than
regular outpatient services can offer. Project A.D.D.V.A.N.C.E.
is located near Hazard, Kentucky.

Adult Services
INCREDIBLE FAMILIES
The Incredible Families Program is a home
visiting services model designed to work with
families caring for children birth to 18 with
limited resources. The program also works
with foster families caring for children from
birth to 18, families with children placed out
of home, and first time expectant parents who
need support. Our program provides individual
treatment, family intervention, and parenting
skills. Every family enrolled in the Incredible
Families Program will receive a minimum of 2
hours per week of services.
SOLUTIONS
Solutions works with other community agencies in a professional team
setting to provide quality complete treatment. Solutions is a program for
women that provides compassionate, innovative, and quality client care
in a safe and supportive environment. Intensive treatment is offered for
women addicted to alcohol and/or other drugs who may also be struggling
with mental health, trauma, and/or behavioral health issues. Women
are empowered to obtain their optimal health. Education related to
personal and family safety is offered as well as access to Domestic Violence
Advocacy services.

INTEGRATED SERVICES PROGRAM
Integrated Services Program (ISP) is the coordination of providing
behavioral health and physical health services. Through ISP, we can
provide better healthcare by caring for people’s needs as a whole. We focus
on behavioral health while taking into consideration all factors that may
affect one’s physical health. Referrals can be made between KRCC staff and
all KRCC partners.
FAMILIES IN SAFE HOME NETWORK (FISHN-I)
FISHN-I works with families and community partners to ensure families
receive the treatment they need while providing a care coordinator to
help them navigate the process. Participating families will include at
risk families who are working toward healthy, happy and permanent
relationships with their children. Servicers are offered in all 8 counties

Adult Services
KRCC provides a broad array of programs to meet the needs of adults
with intellectual or developmental disabilities. These programs and
services are designed to:
t Provide services for adults with intellectual disabilities who are 18 years
or older.
t Educate each individual on the issues of abuse and neglect so they can
protect themselves.
t Educate the public about persons with intellectual disabilities and
inspire them to accept each person within their community.
t Assist the medical professionals within the community in understanding the persons we serve.
t Help individuals in obtaining the medical assistance they require to live
happy and healthy lives.
RESIDENTIAL SERVICES
Our goal at the “Journeys” Program is to provide a supportive home
environment within the community for the people we serve and to assist
them with achieving independent living skills.
t We serve individuals in all eight counties of the Kentucky River Region.
t We provide residential homes in which we serve up to three
individuals.

We match and encourage the individual’s skills with proposed job duties
and the employers with the individuals. Each individual has an on-site
job coach to assist them in their duties. The job- coach encourages good
work ethics. The job-coach is provided at no cost to the individual or
the employer. We follow up with the individual and the employer to
ensure the placement is appropriate and the individual is accomplishing
his/her duties. Work independence is the key for the individuals in our
program. We inspire each individual to work independently within his/her
community.

Adult Services
CONSUMER DIRECTED OPTION
Consumer Directed Option (CDO) gives Medicaid waiver members more
choices in the way some of their Medicaid services are provided. With
CDO, you can choose who will provide the services that you need, as well
as how, when and where the services will be provided.
SUPPORTS FOR COMMUNITY LIVING
Supports for Community Living (SCL) waiver provides adult day training,
community living supports, respite and supported employment for persons
with intellectual and/or developmental disabilities.
MICHELLE P. WAIVER
Michelle P. (MP) waiver is a combination of the HCB and SCL waivers. It
provides adult day training, attendant care, community living supports,
homemaker, personal care, respite and supported employment for persons
with intellectual and/or developmental disabilities.
KRCC’s Assertive Community Treatment
Teams (ACT)
The Assertive Community Treatment Assertive
Community Treatment is a team treatment approach
designed to provide comprehensive, community-based
psychiatric treatment, rehabilitation, and support to
persons with serious and persistent mental illness such as
schizophrenia.
Services are provided by a team of professionals with
backgrounds in social work, rehabilitation, counseling,
nursing and psychiatry. ACT is an evidenced based
practice which has been extensively researched and
evaluated and has proven clinical and cost effective.

Breathitt, Lee, Owsley, and Wolfe and an ACT Team for
the Southern Counties: Knott, Letcher, Leslie, and Perry.
Housing Services
The Mission of the Housing Options Division of KRCC, Inc. is to increase
availability of and access to safe, decent, and affordable housing for
families in the Kentucky River Region. We further commit to contribute
to the development of healthy local communities by increasing housing
opportunities, increasing employment opportunities, improving living
standards, decreasing homelessness, and strengthening local economies by
investing our knowledge, income and resources.

TOWNVIEW
A 17-unit apartment building in Breathitt County that provides
independent living for people with intellectual/developmental disabilities,
who have the skills necessary to live independently and require limited
assistance with daily living skills.

Residential Services
CANEY CREEK REHABILITATION COMPLEX (CCRC)
CCRC is a specialized personal care facility in Knott County that offers 24
hour direct care services for adults with severe mental illnesses. Residents
practice interpersonal skills daily living skills, and learn work skills and
how to manage their illness. The goal is to help each person achieve the
most independent living environment possible.
SUN VALLEY
A residential independent living program for
adults with a serious mental illness, who possess
daily living skills but may benefit from additional
support. Sun Valley is located in Knott County.
SUPPORTED COMMUNITY LIVING (SCL)
Provides 24-hour supervision and training in activities such as laundry,
routine household care, self- care, shopping, money management, and
socialization and leisure activities. No more than three individuals
receiving waiver services can live in any single residential setting.

Advocacy Information
VOLUNTEERING
We have several programs that accept people as volunteers, provided that
they pass our screening process. Almost all our programs have volunteer

PSUIF*OUFMMFDUVBMBOE%FWFMPQNFOUBM%JTBCJMJUJFT%FQBSUNFOU.
You can volunteer to help with special events or for certain activities within
a center or program. Some examples include: helping decorate a facility,
monitoring phones or aiding our consumers during an event (such as the Special
Olympics or a conference). As long as the Program Director approves the
position for which you are volunteering, you may apply as a volunteer.
ćF3JTJOH$FOUFS has an outstanding volunteer opportunities for community
members. You must be over the age of 20 and attend the required 40-hour
training program.
All applicants are screened to ensure the confidentiality and safety of our clients.

ADVOCACY
Our clients come first, so it is important to offer services to fight for the rights
of our clients that are being ignored. Persons who are victims of interpersonal
violence or who may not know the proper process may benefit from support
provided through the following:
MEDICAL ADVOCACY
Rape victims are encouraged to seek medical attention and to receive support
after an assault. The Rising Center’s trained crisis counselors are available 24
hours a day to provide this support at any of the hospitals in the Kentucky River
Region.
LEGAL ADVOCACY
A rape victim is faced with making decisions regarding talking to police and
reporting the crime. The Rising Center’s Victim Advocate offers information
regarding legal processes associated with rape. The Advocate can assist a victim
in applying for and receiving financial compensation through the OVC Crime
Victims Fund.
