Purchase one (1) qualifying Monroe or (1) qualifying Rancho product purchase (referenced above) and get a rebate for the dollar amount
specified next to qualified product purchase. Rebate does not include installation/labor costs or taxes and offer is good only on qualifying
products (see below for list of qualifying products). Offer valid on purchases made between September 1 – October 31, 2011. Upon
fulfilling the requirements below, Tenneco will send you a rebate check for the specified dollar amount.
2. You must submit the following items to receive your Monroe “What Shocks You?” rebate check:
a. Authentic dated sales receipt (copy or original) with business identification verifying the purchase of the qualifying products. The
purchased product part numbers MUST be identified on the sales receipt. Offer does not apply to transactions when product is being
replaced under warranty.
b. This form, filled out completely and legibly. Visually altered documentation will not be honored.
3. Send this completed form and required proof-of-purchase (dated sales receipt) to the address indicated below. Requests must be
postmarked by December 1, 2011. Tenneco is not responsible for late, lost or misdirected mail, and all proofs-of-purchase become the
property of Tenneco and will not be returned. Please retain copies for your files.
4. Offer good only in the U.S.A. at participating locations. Offer open to individuals; requests from groups, clubs or organizations will not be
honored. Void where prohibited, taxed or otherwise restricted. Please allow 8-10 weeks for processing from Tenneco’s receipt of
redemption form and all required proofs-of-purchase. Fraudulent submission could result in federal prosecution under mail fraud statutes.
Purchase, sale or trade of any offer requirement is prohibited. This promotion is prohibited for persons under the age of 18 residing in the
state of Maine.

6. The Monroe “What Shocks You?” rebate check is valid for 90 days from the date of issue.
