12 Practices Go­Live in MEDENT Hub Project Twelve practices in the Southern Tier and Hudson Valley Regions are now live through the
MEDENT Hub Project with a bi­directional Consolidated Clinical Document Architecture
(CCD­A) connection to the HealthlinkNY Health Information Exchange (HIE). The
following practices are now exchanging health information with HealthlinkNY:

Click here to learn how to get connected today! Two Hudson Valley Health Homes are Connected to
HealthlinkNY Health Information Exchange
HealthlinkNY is proud to announce that the
Hudson Valley Care Coalition (HVCC) and the
CommunityHealth Care Collaborative (CCC)
are now successfully connected to the
HealthlinkNY Health Information Exchange
(HIE) and are in the process of submitting Plan
of Care documents.
HVCC and CCC are Medicaid Health Homes
designed to provide comprehensive care
management for select patients with chronic
and behavioral health conditions. Medicaid
Health Homes implement a care management service model in which all of the patient's
caregivers communicate with one another to ensure that patient's needs are addressed in
a comprehensive manner.
When asked about the documentation of Health Home patient records, Georganne Chapin,
CEO for Hudson Center said, "Ultimately, this information needs to be centralized." The
HIE provides this centralization so that patients receive safer and more efficient care from
their providers while their health records and personal information remains protected.
Chapin went on to add, "It's an enormous benefit that this information is available [in the
HIE] and we don't have to rely on memory­it's so obviously needed..."

Directional Connection in the Hudson Valley
HealthlinkNY's reach continues to expand as more practices are connected to the HIE.
The Institute for Family Health (IFH) is now live with a bi­directional Consolidated Clinical
Document Architecture exchange (CCD­A). This type of connection allows IFH to send
patient data to and from the HIE securely through their electronic health record (EHR)
system.
IFH is a Federally­Qualified Health Center (FQHC), consisting of 26 locations throughout
Manhattan, the Bronx and Hudson Valley. IFH specializes in high quality, patient­centered
primary care for all ages, as well as mental health, dental care, social work and several
other services. In addition to operating health centers, IFH uses its expertise and
resources to address racial and ethnic disparities.
When asked about the benefits that the HIE will bring to the Institute, Virna Little, Senior
VP Psychosocial Services and Community Affairs said, "As we serve more and more
patients, the health information exchange (HIE) will help our staff work more efficiently
and accurately. It will also give our patients more control of their health by giving them
simplified and secure access to their medical records."
