4. Include COPIES of the sales receipt/ invoices as proof of purchase.
5. Cut out the ORIGINAL UPC barcode symbol (keep copies for your records).
6. Mail the copy of your receipt/ invoice and the ORIGINAL UPC barcode.to
If you don't have access to the internet, please mail a copy of your orginal receipt/invoice, the ORGINAL Plextor
UPC, and this completed form.

rebate offers per product per household (validated by name on receipt/invoice) as determined by Plextor in its sole discretion. Keep copies of all materials sent. Plextor is
not responsible for late or misdirected mail. Offer not valid with other Plextor promotions or where restricted by law. No reproductions of UPC barcodes accepted. Offer valid in
the U.S and Canada only; excludes resellers of Plextor products and their families, as well as purchases made through Plextor's employee purchase and dealer demo
programs. If terms and conditions are not met, rebate will not be honored. All obligation of Plextor or its agent will be discharged by mailing the rebate check to the address
given. Any tax on rebate is consumer's responsibility. Offer does not apply to any refurbished products or products purchased for resale, demonstration or evaluation. All
requested materials submitted become Plextor property and will not be returned. Plextor reserves the right to rescind or change the offer at any time. Offer void where
prohibited by law. Offer is invalid if the UPC barcode does not match the barcode specified above. Refurbished drives or Plextor drives purchased on eBay do not qualify for the
