Hospitalist Program Our Hospitalist Program provides care for hospital inpatients who do not have a primary care physician or whose primary care physician does not provide patient care in the hospital setting. Our hospitalists specialize in managing the care of patients while they are in the hospital. If your physician chooses to use our Hospitalist Program, the hospitalist is responsible for ordering tests and procedures, coordinating consultations and personally providing you with quality care throughout your hospital stay. If you are admitted to Faxton St. Luke’s Healthcare (FSLH) either by referral from your primary care physician or through the Emergency Department, a hospitalist will coordinate your plan of care and keep your family informed on details of your hospital stay. At discharge, the hospitalist will communicate with your doctor and discuss your additional treatment needs, as well as help arrange follow up care. The hospitalist will also send your hospital records to your primary care physician. Hospitalists are available 24 hours a day, seven days a week and work closely with the nursing team and hospital support staff to care for you. With a physician who practices exclusively in the hospital, it may be easier for you and your family to conveniently communicate about your illness and work toward a recovery plan of care. Hospitalist Providers Lisette Alfaro-Berg, MD Dr. Alfaro-Berg received her bachelor’s degree in Biology from New York University and her medical degree from SUNY-Downstate Medical Center in Brooklyn, New York. She completed her residency in Internal Medicine at the University of Texas Southwestern Medical Center in Dallas, Texas, and is board certified in Internal Medicine. Joy Black, MD Dr. Black earned her undergraduate degree at The Johns Hopkins University in Baltimore, Maryland, and her medical degree at New Jersey Medical School in Newark, New Jersey. She completed her internship and residency in Family Practice at the John F. Kennedy Medical Center in Edison, New Jersey. After 20 years of private practice, Dr. Black became a hospitalist at FSLH. She is board certified in Family Medicine and is a member of the American Academy of Family Physicians and the American Medical Association. Brian Chase, MD Dr. Chase received his medical degree at SUNY Upstate Medical University in Syracuse, New York. He also completed his residency at the Family Medicine Residency Program at St. Joseph’s Hospital Health Center in Syracuse, New York. Elizabeth Cooley, MD Dr. Cooley is a hospitalist and also a family practice physician with the Adirondack Community Physicians (ACP) North Utica Medical Office. She received her bachelor’s degree from Middlebury College in Vermont and her medical degree from the University of Vermont College of Medicine in Burlington, Vermont. She completed her Family Medicine residency at St. Joseph’s Hospital Health Center and an internship in Obstetrics and Gynecology at SUNY Upstate Medical University, both located in Syracuse, New York. Andrew J. Faber, MD Dr. Faber holds a bachelor’s degree from Union College in Schenectady, New York, and his medical degree from New York Medical College in Valhalla, New York. He completed his residency in Internal Medicine at Gelsinger Medical Center in Danville, Pennsylvania. Dr. Faber has been a member of FSLH’s staff since 1986 and is board certified in Internal Medicine. Satish Gedela, MD Dr. Gedela received his master’s degree in Public Health at Missouri State University in Springfield, Missouri, and completed his residency at St. Elizabeth’s Family Medicine Residency Program in Utica, New York. Maxine Jones, MS, ANP-BC Maxine Jones received her bachelor’s degree in Nursing and a master’s degree of Adult Nurse Practitioner at SUNY Utica/Rome in Marcy, New York. Marissa Kalil, DO Dr. Kalil is a hospitalist and also an internist with the ACP New Hartford Medical Office. She earned a bachelor’s degree in Biology from Utica College and her Doctor of Osteopathic Medicine from the University of New England College of Osteopathic Medicine in Biddford, Maine. She completed her residency in Internal Medicine at Bassett Healthcare in Cooperstown, New York. Dr. Kalil is board certified in Internal Medicine. Soojung Kwon, MD Dr. Kwon received her medical degree from the College of Medicine, Pusan National University in Pusan, South Korea, and completed residencies in Internal Medicine at Sound Shore Medical Center in Westchester, New York, and at Asan Medical Center in Seoul, South Korea. During this time she also earned her master’s degree in Internal Medicine at the School of Medicine, Ulsan University, in Seoul, South Korea. She is board certified in Geriatric Medicine and Internal Medicine. Hospitalist Providers Carol Lareau, RN, FNP Carol Lareau, RN, FNP, received her registered nursing degree from St. Luke’s-Roosevelt Hospital School of Nursing in New York City and her family nurse practitioner certification from Community General Hospital in Syracuse, New York. She is a member of the American Academy of Nurse Practitioners and New York State Association of Nurse Practitioners. Goutham Malempati, MD Dr. Malempati received his undergraduate degree and medical degree from Mahadevappa Rampure Medical College in India. He is board certified in Family Medicine. Tatjana Mondom, MD Dr. Mondom received her medical degree from University of Belgrade, School of Medicine in Yugoslavia and completed her residency in Family Medicine at St. Joseph’s Hospital in Syracuse, New York. She is board certified in Family Medicine. Anil Patel, MD Dr. Patel received his medical degree from Pramukhswami Medical College in India. He completed his residency training at Overlook Hospital in Summit, New Jersey, and is board certified in Internal Medicine. Anand Ravi, MD Dr. Ravi earned his Bachelor of Medicine and Bachelor of Surgery from M. R. Medical College in Gulbarga, India. He completed his residency at St. Elizabeth Medical Center Family Medicine Residency Program in Utica, New York. He is board certified in Family Medicine and is a member of the American Academy of Family Physicians and the American Medical Association. Sarmad Siddiqui, MD Dr. Siddiqui received his medical degree from Nagpur University, Indira Gandhi Medical College and Mayo Hospital in India and completed his residency in Family Medicine at St. Elizabeth Medical Center in Utica, New York.
