GENERAL INFORMATION
Meeting Headquarters Hotel Reservation Information
Special sleeping room rates have been secured at the hotel. The rates are US $199 for single and double occupancy
(maximum occupancy per room is four people). Room reservations can be made by directly contacting The Hilton
Austin 1-800-482-8000 or on-line.
IMPORTANT: These special room rates are good on or before February 21, 2015. After February 21st , or before,
if the IAO room block sells out, reservations will be accepted on a space availability basis and the special room
rate cannot be guaranteed. The number of hotel rooms at this special group rate is limited and it is suggested that
you make room reservations as early as possible.

The Austin-Bergstrom International Airport (AUS) is the closest airport to the hotel.
For Dentists Living Outside the United States and Canada
When planning to attend the 2015 IAO Annual Meeting, be sure that you make travel arrangements well in advance.
This includes obtaining all necessary visas to enter the United States. Check with your local United States Embassy
or Consulate for your nationality’s requirements for visiting the United States. The IAO headquarters office will
gladly send interested dentists an official invitation to attend the annual meeting if requested.

About the IAO
• The International Association for Orthodontics (IAO) is the oldest national and international non-specialty
orthodontic group in existence.
• The IAO has a worldwide membership of nearly 3,800 general, pediatric and other dentists.
• The IAO has approximately 50 IAO recognized study clubs worldwide.
• The IAO was formed in 1961 to promote national and international cooperation in the orthodontic field of dentistry.
• The IAO maintains “tier” membership categories for dentists to promote and recognize continuing orthodontic
education excellence for its members.

Doctors are cautioned against the use of limited knowledge when incorporating techniques and procedures into
their practices, especially when a session has not provided them with the necessary supervised clinical experience
in the technique or procedure to ensure that they have attained competence.
Cancellation and Refund Policy
All paid meeting registrations will be refunded totally on or before March 10, 2015. Paid registration cancellations
between March 11, 2015 and March 25, 2015 will be refunded half of the registration paid. There will be no refunds
made after March 26, 2015. In the unlikely event that the IAO Annual Meeting is cancelled, a full refund will be made
to all advanced registrants. Cancellation of an individual speaker will not be cause for a refund.

There will be continental breakfast between 7:30 AM and 9:00 AM on Thursday and Sunday morning outside the
lecture room and Friday and Saturday morning in the Exhibit Hall. This will be a courtesy to all persons registered
for the meeting.
IAO Continuing Education Provider Recognition and IAO Course Recording Program
The IAO is an ADA CERP Recognized Provider.ADA CERP is a service of the American Dental
Association to assist dental professionals in identifying quality providers of continuing dental
education.ADA CERP does not approve or endorse individual courses or instructors, nor does
it imply acceptance of credit hours by boards of dentistry. Concerns or complaints about a CE
provider may be directed to the provider or to the ADA CERP at www.ada.org/goto/cerp. The
formal continuing education programs of this program provider are accepted by AGD for Fellowship/ Mastership credit.
The current term of acceptance extends from 10/2014 - 12/2018. IAO Members who attend the 2015 IAO Annual Meeting
will, after they submit their participation forms, have their courses recorded with the IAO Continuing Education Course
Recording Program that is used in determining IAO membership tier advancement.
The Annual Banquet and General Assembly Luncheon
Make plans to attend two popular annual meeting functions. On Friday, March 27th, the General Assembly
Luncheon will take place. Enjoy lunch and participate in the association’s yearly general membership meeting.
The luncheon is included in the Dentist and Auxiliary/Staff registration fees. For others the charge is US $60. The
Annual Banquet is scheduled for Saturday, March 28th. There will be a no-host reception before the banquet
at 7:00 PM. Dinner begins at 8:00 PM with live entertainment provided after the meal and program. This banquet
is included in the Dentist, Spouse/Guest and Auxiliary/Staff registration fees. For others the charge is US $85.
The Exhibit Hall – Your Orthodontic Marketplace!
The 2015 IAO Annual Meeting's Exhibit Hall will feature exhibits from national and international vendors that provide
both equipment and services to dentists who practice orthodontics. Attendees will have an opportunity to meet
knowledgeable representatives who will answer their questions. The Exhibit Hall is open to all registered attendees. All
breaks will be at the Exhibit Hall. Don't miss the IAO President's Reception in honor of IAO President Dr. Kevin
Williams, held in the Exhibit Hall Thursday evening, March 26th, between 5:00 PM and 7:00 PM.

Table clinics and case displays prepared by IAO members will be presented on Saturday, March 28th, between 12:00
PM and 2:00 PM in the exhibit hall. Showcase new techniques or treatments at the Table Clinics session or present
excellent finished cases and compete for Case of the Year at the Case Displays session. Also at Case Displays, successful
IBO candidates will display their best cases. This is always an excellent learning opportunity for attendees. If you wish
to participate as a presenter, you must sign up by March 1st, please contact the IAO headquarters office for an

You may have heard that folks are trying to Keep Austin Weird or perhaps that the state capital of Texas is the Live
Music Capital of the World? Maybe it’s watching the Texas Longhorns duke it out on the football field that sticks
in your mind? Well today you will get to see how all of the many identities of Austin came to be through a historic,
cultural and modern day tour of this unique town. You’ll see the heart of the music and nightlife scene driving
down 6th street with a view of the historic Driskill Hotel. Today, shopping and dining have branched into the
warehouse district and the upscale 2nd street with the opening of The W Hotel... and inside is the Moody Theater
where Austin City Limits (ACL), the longest running music series in American television history, is now held.
Driving along Congress Avenue the main thoroughfare of the city will take you from the steps of the sunset red
Texas Capitol to SOCO, a lively area where we find food trucks, music, and unusual and interesting stores and
boutiques. Enjoy some time to explore and the chance to pick-up items from western boots to jewelry to clothing.
This tour will end at the famous Austin Farmer’s Market with the option of walking back to the hotel or continuing
on the bus.

WHEN IS THERE A NEED TO EXTRACT IN ORTHODONTIC TREATMENT PLANNING?
THE DIAGNOSIS AND BIOMECHANICS ASSOCIATED WITH BICUSPID EXTRACTIONS IN
PATIENTS WHOSE PROFILE IS TOO FULL
When there is a need to extract in Orthodontic treatment planning, you must have a greater understanding of
extracting for the face and NOT for the space, comprehend that the lower arch is treated ahead of the upper arch
to create an overjet, list critical elements to ensure that the bite is opened before retraction commences, understand
the correct torque for canines so that the roots DO NOT hit the cortical plate during retraction, and recognize the
increased need for anchorage in high angle cases ie low pull mechanics using TADs. Following the conclusion of
this session each attendee will be able to: identify the appropriate elements to include in a discussion with parents
regarding the importance of extractions in a profile that is too full, treatment plan a Bimax case with appropriate
biomechanics, provide the appropriate information to obtain consent for treatment, and identify the elements of
and/or recognize an effective post-treatment retention regime to prevent the extraction spaces from reopening.
Derek Mahony is a Specialist Orthodontist who has spoken to thousands of practitioners about the benefits of interceptive
orthodontic treatment. Early in his career Dr. Mahony learned from leading clinicians the dramatic effect proper nasal
respiration can afford patients in orthodontic treatment. He has promoted early ENT consultations, and normalized
tongue posture, ever since. His lectures are based on the positive impact such a combined treatment approach has had
on his orthodontic results and the benefits this philosophy provides from a practice management viewpoint. Dr. Mahony
has been actively involved in research that links constricted maxillary arch forms to nasal breathing problems, adverse
facial growth and systemic health problems such as nocturnal enuresis. He has presented over 400 lectures on orthodontic
topics in more than 30 countries. As a practicing clinician, Dr. Mahony’s research interests are in the etiology of
malocclusion and the guidance of facial growth. Dr. Mahony is an IAO Master Senior Certified Instructor.

PROPEL: ACCELERATED ORTHODONTICS WITH MICRO-OSTEOPERFORATIONS
The use of Propel will be discussed including current research and practical applications. Clinical cases will be presented
demonstrating the ability of Propel to accelerate orthodontic tooth movement in a comfortable, efficient and costeffective manner. At the conclusion of this lecture, attendees will: understand the biology and histology behind
Micro-Osteoperforation with Propel, specifically how it creates faster more predictable tooth movement, have learned
how to use Propel to facilitate challenging movements with any orthodontic modality, including clear aligner therapy,
know how to plan orthodontic treatment with Propel, understand how to incorporate Propel into their current
orthodontic treatment options, and understand the patient benefits of accelerated orthodontics with Propel.
Dr. Ben Miraglia graduated from the State University of New York at Buffalo School of Dental Medicine. He is a
proud member of the Omicron Kappa Upsilon Dental Honor Society. Dr. Miraglia completed a general practice
residency program at Danbury Hospital in Connecticut. He has 21 years private practice experience in Mt. Kisco,
New York. Dr. Miraglia is a graduate of the United States Dental Institute where he learned his non-extraction and
non-surgical orthodontic philosophy. He was appointed to the USDI faculty in 2008 and teaches nationally for
them. He is also an Invisiaglign faculty member since 2008. Dr. Miraglia was recently honored with a position on
the President’s Council of Northern Westchester Hospital in Mt. Kisco, New York.

ORTHODONTICS TODAY: MORE THAN JUST STRAIGHTENING TEETH
Much of modern orthodontics is based on the clinician establishing a foundation to optimize the patient’s health
and finding balanced facial esthetics as an end result. This foundation is built by optimizing airway and respiratory
function leading to good sleep respiration, placing the temporomandibular joints in the correct position to prevent
later joint issues, which are pandemic in modern society, and establishing good skeletal relationships for facial
harmony and balance in growing patients. Now more than ever, it’s about growth guidance and maximizing our
patient’s genetic growth potential instead of just thinking about straightening teeth. At the conclusion of this lecture,
attendees will: understand the use of appliance therapy, be able to discuss dietary advice from birth and have
knowledge of myofunctional therapy; all as it relates to Phase 1 treatment prior to placing braces.
Dr. Fraser has practiced dentistry for over 28 years, and now restricts his practice to orthodontics. He has lectured
in Australia and overseas. Dr. Fraser graduated dental school from the University of Sydney, holds a Fellowship
from the Australasian College of Nutritional and Environmental Medicine, is a Diplomate of the IAO, an IAO
Education Committee Examiner, and an IAO Master Senior Certified Instructor. He is a Past President of the
Australian Association of Orofacial Orthopedics and a Past President of the IAO.
Dr. Lee, MS, MAOI is a full time practicing dentist in Sydney, Australia. He has a particular interest in orthodontics,
TMJ and the treatment of head and neck pain. Dr. Lee graduated from dental school at the University of Sydney, is
an IAO Master Senior Certified Instructor, a Fellow of the IAO, a Fellow and Diplomate of the ICOI and a member
of the Australian Association of Orofacial Orthopedics.

THE THREE KINGS OF FJO TREATMENT
Three basic appliance techniques lay at the heart of Anterior Repositioning Treatment (ART) for the Ortho/TMJ/
Headache patient, The Functional Advancing Condylar Translation appliance (FACT), the Bionator “Retainer” and
the “SSV”. At the conclusion of this lecture the attendee will: understand why the FACT appliance is the common
sense “super-splint” for TMD patients of an advanced level of chronic headache joint pains, know why the Bionator
H.S. Retainer is the perfect night time appliance for use in conjunction with the day time FACT, and have knowledge
of the most advanced technology available in appliance design and usage for easily and rapidly closing post-ART
posterior open bites- is the SSV. Its design, use and adjustment will be thoroughly explained.
Dr. T.J. Spahl is a graduate of the University of Minnesota in 1969. He is an IAO Certified Senior Instructor. He has
lectured extensively throughout the United States and 8 foreign countries. He is the principle author of a trilogy of
textbooks on orthodontics and TMJ therapeutics written in the 1980’s in conjunction with the late Dr. John Witzig.

THE EASY WAY TO SUBMIT IBO CASES: ONLINE SUBMISSION WITH CASEWORX SOFTWARE
IBO case submission has always been through written and printed material (plus study models). The IBO has
introduced a new innovative way: completely online through the software CASEWORX. This software produces a
complete blind case submission which guarantees anonymity and impartiality and is designed to be user friendly.
Following the conclusion of this session, each attendee will be able to: understand the IBO case submission options,
understand the requirements for the IBO cases, and have insight in the case submission through CASEWORX,
including how the software makes the case grading anonymous and impartial.

ORTHODONTICS & THE T.M.J.S - PROTECT YOUR PRACTICE!
Many general dentists and specialist orthodontists do not appreciate the impact their treatment can have on the
function of the T.M.J.S. This impact can be both positive and negative. Failure to understand and to document the
function of the T.M.J.S can result in serious legal ramifications. At the conclusion of this lecture, each attendee will know
how all dental practices should document the existing function of the T.M.J.s as part of a routine dental examination,
how to document any joint dysfunction as a pre-existing condition, and have the patient advised accordingly, how to
classify the T.M.D so that it is simple and easy for the patient to understand, and be able to identify common mechanical
errors that can cause T.M.D and provide solutions to many joints dysfunction problems.

BONDING ALL TYPES OF ORTHODONTIC APPLIANCES
WITH CURRENT MATERIALS AND TECHNIQUES
This lecture will explain how to bond all types of appliances to any surface in the mouth. In depth descriptions of
long term passive lingual retention, various methods of banded, bonded and crown expansion and Herbst
appliances will be shown. In addition, unique new concepts in aligner treatment will be shown including the ability
to bond elastic hooks and power arms directly to aligner plastics. Upon completion of this lecture attendees will
have learned: how to successfully keep teeth in alignment post treatment on the maxilla and mandible without
relapse, how to bond brackets to all enamel and non-enamel surfaces, the best way to bond hooks and power arms

MYOFUNCTIONAL ORTHODONTICS - GOING BEYOND JUST TREATING MALOCCLUSION
The global demand for orthodontics without braces continues to grow. More than 75% of children have a developing
malocclusion which is evident from as early as 5 years of age. The influences of allergies, asthma, poor posture and
nutrition have had an effect on facial and dental development. The way a child swallows determines if the teeth will
be correctly aligned or crowded. These problems should be evaluated and treated while the child is still growing,
well before the usual age when children have braces. Orthodontic treatment with extraction of permanent teeth can
be avoided by early intervention with a comprehensive and holistic approach. It's an option that many parents and
patients would prefer. Myofunctional orthodontics offers a viable alternative to traditional orthodontic methods.
At the conclusion of this lecture, attendees will: have knowledge of preformed appliances that allow practitioners
to provide a no-braces treatment to a greater number of patients, know how to produce stable treatment results and
be able to gain financial benefits for both doctor and patient.
Dr. Urbi graduated from Centro Escolar University and has been practicing orthodontics for 22 years. He has
benefited from all the advances that modern orthodontic treatment has to offer, including functional orthodontics
and low-force, low-friction techniques. Dr. Urbi began to see the benefits of myofunctional therapy in his practice
and has first-hand experience with moving from a “tooth-centric” philosophy of orthodontic mechanotherapy to

HALF OF WHAT YOU KNOW ABOUT ORTHODONTICS IS WRONG
The biggest problem with the acquisition and application of new ideas and techniques is not the actual learning of
them, rather the forgetting and using of older and more habitual thoughts. This reluctance to unlearn old lessons is
known as path dependence, which broadly means history matters. Path dependence explains how the set of decisions
one faces for any given circumstance is limited by the decisions one has made in the past, even though past circumstances
may no longer have relevance. Unfortunately, much of orthodontic knowledge and practice still depends on path
dependence. This lecture intends to dispel some of those commonly held beliefs by those who practice clinical
orthodontics and suggest some alternative and more beneficial diagnostic, treatment planning and therapeutic protocols.
At the conclusion of this lecture, attendees will have some of their most common and cherished beliefs challenged and
disproved and they will have knowledge of preferable alternatives that will benefit them and their patients.
Larry W. White, DDS, MSD, graduated from Baylor Dental College. He then served two years in the United States Air
Force Dental Corps before moving to Hobbs where he practiced general dentistry for five years. He returned to
Baylor Dental College and received a graduate degree in orthodontics. He practiced orthodontics in Hobbs for 31
years and then began an academic career by starting the University of Texas Health Science Center in San Antonio’s
first graduate orthodontic program. Dr. White has published over 100 professional articles, authored several books
about orthodontics and edited numerous professional publications. Dr. White continues to practice orthodontics
and teach in the Orthodontic Department at Baylor Dental College.

It is very important for dental professionals who provides treatment of orthopedics and orthodontics to know in
detail the anatomy of the upper airway and determine if there is any obstruction that is causing problems such
as crowded mandibular anterior teeth; “falling” mandible (open bite); sleep apnea and dysfunction of the TMJ. At
the conclusion of this lecture, each attendee will, after analyzing an x-ray and a ceph, be capable of: identifying all
the anatomical structures of the upper airway, determining if any of these structures could be the cause of
obstruction, determining the extent of the obstruction, and establishing a treatment plan.
Dr. Bobea obtained his Doctor in Dental Surgery Degree and a Master in Science in Neuroanatomy from the
Universidad Autónoma de Santo Domingo, Dominican Republic in 1979. He is the President of the IAO Puerto Rico

CEPHALOMETRIC TRACING, BOLTON ANALYSIS, ARCH LENGTH DISCREPANCY
Cephalometric and Bolton analysis along with arch length discrepancy are large portions of the data needed to present
cases to the IBO Board. One of biggest problems facing a doctor wanting to participate in IBO Tier Advancement is
inadequate records. A properly trained auxiliary can create most or all of these records for the doctor. Following the
conclusion of this session, each attendee will be able to: identify each cephalometric landmark used by the IBO
presentation, trace a lateral cephalogram, perform a Bolton Analysis, compile an arch length discrepancy, complete the
cephalometric analysis, fill out an IBO Case report form, and super impose succeeding cephs.

IDENTIFYING AND SCREENING OF YOUR PEDIATRIC
AND ADULT SLEEP APNEA PATIENTS
Adults and Children with airway obstructions, such as enlarged tonsils, constricted arches and retrognathic profiles
often snore. This is a telltale sign that could potentially lead to a more serious condition known as snoring
obstructive sleep apnea. This frequently causes poor sleep habits, excessive daytime fatigue and the inability to
concentrate. Following the conclusion of this session each attendee will be able to: screen and identify both the
adult and pediatric patients for sleep disorder breathing concerns, identify the appropriate elements to include in
a discussion with parents regarding the importance of airway discrepancies, and utilize diagnostic tools used to
