The following instructions will demonstrate how to create a new Web Certificate of
Good Standing account as well as create a new Certificate of Good Standing
Request.

Certificates of Good Standing must be requested by the Employer and require a web certificate account to be created.
Note: It is required to change the account password every 90 days.
1. Step 1-Creating New Account:
2. First time users need to create a User Login ID and Password

Step 2‐Creating New Account:
1. All fields with red asterisks are required fields and must be completed. For a password requirements, click on the green question mark. Note: Spaces, punctuation, and
symbols should not be used in the User ID and Password.
2. Select Are you representing a PolicyHolder? radio button.
3. Click Continue

Step 3-Creating New Account:
1. Enter your Policy Number which is the same as you Workers’ Compensation Employer Number
2. Enter the Policy Effective Date. If you have been registered with the Division of Workers’ Compensation for more than one year, enter 01/01/2012 for public entities and 07/01/2012 for private (governmental) entities. If you have been registered for less than a year, you will need to enter the exact effective date of your Workers’ Compensation account. Please contact the Division should you need this information.
3. Enter the Tax ID Type of FEIN
4. Enter your Tax ID/FEIN Number
5. Click Continue

1. Verify the information displayed on the screen and click Yes.

Step 5-Creating New Account:
1. Click OK to Account Created informational box
2. Then click Accept to the Terms and Conditions

Click on View/Add Certificate

Click Continue on informational box. There is also an option to not show this
message again if you so choose.

The Certificate of Good Standing Select A Policy screen may display. If this screen does not display, continue to page 11.
1. Click on Display All Policies (it may take a few moments to load).

A list of all policy periods will display.
1. Select the line containing the current year policy period and click View Certificates.
1. For example, if you are submitting a request and the year is 2012, select the 2012‐2013 policy period. Or if you are submitting the request in 2013, select the 2013‐2014 policy period.

This screen will display all Certificate of Good Standing requests for the current year. This screens also provides the availability to request a new Certificate of Good Standing and to copy an existing request to resubmit as is or with updated information.
Step 1‐New Certificate of Good Standing Request:
1. Click Add New Certificate button at the bottom of the screen.

Select Enter New Name and Address radio button
Enter the Business name of the Certificate Holder (3rd party requestor)
Enter In Care of Name for the Certificate Holder Contact Name-Optional Step
Enter Address of Certificate Holder
Enter Postal Code of Certificate Holder
Click Next

Step 3-New Certificate of Good Standing Request:
1. Insured drop down box will default to your business name, no other options will
be available.
2. Enter Job Number-Optional Step
3. Enter Location-Optional Step
4. Description of Operations selection is for informational purposes only, it will not
display on the Certificate of Good Standing document.
5. Click the Next button

Step 4-New Certificate of Good Standing Request:
1. At this time, the only delivery option available is Carrier Produced which means
the Certificate of Good Standing documents will be generated by the Division
and typically mailed the next business day.
2. Enter your name in the Entered by Name field.
3. Click the Next button.

Step 5-New Certificate of Good Standing Request:
1. Review the information entered and click Yes, Submit Certificate if information
is accurate. If changes are needed, click No, Change Information to navigate
back to Step 2.

• The Web Certificate requests will be processed each night and will typically be
mailed the next business day.
• When finished entering Certificate of Good Standing requests, click Logout in
the upper right hand corner of the screen.
