The Gift Badge Purchase Program is only open to sworn or retired (2008-2009) Department personnel.
Please print and complete the Gift Badge Certificate along with the GIFT BADGE ORDER FORM
pages 3 and 4 (For gift badge purchases please only use the gift badge order forms)
CERTIFICATE
 If you are giving the badge please print the name of the gift receiver on the line marked
“Gift Receiver Name”
 To authenticate the certificate you must complete the bottom portion of the certificate with
your title/rank, serial number, signature and date. (Please do not put the gift receiver’s
information here)
ORDER FORMS
 If you are giving a badge please complete page 3 with only your name, title/rank serial
number and billing information.
 PAGE 4; please complete this page with only the gift receiver’s information. Please
indicate their rank and/or serial number as deemed applicable.
If gift receiver is picking up gift badge at Denver P.D. Headquarters Uniform Supply, the receiver must
sign and present certificate at time of pick up, otherwise the gift purchaser must present the gift
certificate and sign for the badge.

compliance with all laws and department policies. Customer agrees to indemnify and hold all other personnel, officials and
employees (other than Customer) harmless from all claims, demands, losses, liabilities, obligations, cause or cause of action,
and expenses, including but not limited to attorney fees and costs, of any type, kind suffered, from any source, arising out of
any failure by the Customer to use, handle and/or store the badge purchased pursuant to this order in complaince with all
applicable laws and policies. The purchaser agrees not to resell, trade or auction the badge set to the general public. All sales
are final.
