RMA PROCEDURES
1. Call Hairpiece.com at (323) 930-4747 to request a RMA Number.
2. Complete this RMA REQUEST FORM with detailed response.
3. Pack copy of invoice and items properly with RMA # clearly marked on the package.
4. Ship to: The Hair Shop RMA, 1258 N Highland Ave., Los Angeles, CA 90038
5. Keep Tracking#, RMA #, and original invoice for shipment purposes.
6. If you don’t receive a response after package has been shipped please call 323-930-4747.
** The RMA number must be included on the outside label of your shipping box. Our store is not authorized to accept returns without
an RMA on the shipping label. RMA numbers are valid for 10 days from the date of issue.
