A
t Magee-Womens Research Institute (MWRI), something incredible is
being uncovered in every laboratory, by every scientist, on every day. There’s a
spirit here…part curiosity, part fascination, part determination.
Individually and collectively, researchers at MWRI are changing the shape of
medicine for women and infants. Their incredible work is impacting lives today,
tomorrow and well into the future.
As recently as 20 years ago, all medical research focused on men’s issues.
With the establishment of MWRI, the nation’s first research institute focused
exclusively on the health of women and infants, the breadth of medical research
expanded.
Convinced that the differences in genders exist on every level, including cellular
and molecular, the MWRI team established a new standard for research dedicated
to the medical issues facing women and infants. Scientists explore problems such
as gynecologic cancers, infectious diseases, genetic conditions and autoimmune
deficiencies and then translate their findings into more effective prevention,
detection and treatment for patients.
The transfer of knowledge from bench to bedside has improved the outcomes for
patients at our sister institution, Magee-Womens Hospital of UPMC. In fact, U.S.
News & World Report recently ranked Magee’s gynecology program No. 6 in the
country. As MWRI advances research, Magee-Womens Hospital advances care.

It’s said that success breeds success. That might be part of the reason why MWRI
continues to attract the world’s most respected and renowned researchers
specializing in women’s and infants’ issues. As the MWRI team works together to
find the keys that will unlock medical advancements, patients in our region, in our
country and throughout the world continue to reap the benefits.
Some of the current research initiatives taking place at MWRI include:
• Researchers in Development & Regenerative Medicine are accelerating
discoveries in infertility treatments, including preservation techniques for
men, women, boys and girls who must undergo cancer treatments.
• Collaborating with physicians and scientists at neighboring Western
Psychiatric Institute and Clinic, MWRI researchers are studying postpartum
depression and the impact of antidepressants on maternal functioning and
infant development.
• The Microbicide Trial Network, which is housed at MWRI, is working
to change the face of HIV by reducing through transmission the
development of topical and/or oral products.
Thanks to your support, MWRI has touched the lives of countless women and
infants throughout the Pittsburgh region and beyond. You have helped to ensure
that the discoveries made at the laboratory bench are translated into advancements
at patients’ bedsides.
The researchers at MWRI are challenging conventional thinking and finding
better methods of treatment and prevention. Despite many advances, they never
lose sight of the reality that their work is far from being complete. There are more
questions to be asked, more theories to be challenged, more hope to be developed.

Maternal-Fetal Medicine:
Limitless Possibilities
Each year, more than 540,000 babies are born
prematurely in the United States — about one per
minute. In many cases, these infants are born to mothers
with problem pregnancies such as diabetes, preeclampsia,
multiple fetuses, or conditions known to increase the
risk of complications for the infant or mother. It wasn’t
until the recognition of Maternal-Fetal Medicine (MFM)
— or perinatology — as a subspecialty of obstetrics and
gynecology and the development of advanced testing
that physicians could understand the mechanisms by
which such conditions affected mothers and their unborn
children.
“Prior to the early 1970s, we could not image
the fetus or determine how healthy or mature it was,”
explains Steve N. Caritis, MD, professor, Obstetrics and
Gynecology.“We had very little understanding of how
maternal diseases affected fetal health. All that changed
dramatically during the 70’s. Technological advances
revolutionized the field.”
The Division of Maternal-Fetal Medicine at MageeWomens Hospital of UPMC was established at Magee
in 1975, just one year after the American Board of
Obstetrics and Gynecology recognized it as a subspecialty.

MFM program grew to one of
national prominence. Physicians
in the division were conducting
research on maternal diseases
and fetal physiology, providing
complete prenatal care to women
with challenging pregnancies and

cases to obstetricians.
Clinical research at Magee advanced tremendously
in 1986, when the National Institute of Child Health
and Human Development created the Maternal-Fetal
Medicine Units (MFMU) Network, a multicenter,
perinatal research network. Dr. Caritis was the Principal
Investigator from Magee in this network and has been
with the MFMU Network since its inception. He is
the only lead scientist who has been included in the
Network since its establishment. Dr. Hyagriv Simhan has
been the Co-Principal Investigator for the MFMU for
the last six years. Inclusion in the MFMU Network is
highly competitive, as it has become the premier clinical
research entity in the world focusing on pregnancy.

So grateful for the care and concern they received [from Magee’s Maternal-Fetal Medicine
Department], the Orsattis struggled to translate the word “Magee” into their native Italian and
Spanish tongues. “Had we been able to find a way, Lucas would have been named for Magee,”
Mark Orsatti says.
Membership in the MFMU requires a new grant to be
submitted every five years. Centers are chosen based on
research experience, leadership, experience in multicenter
clinical research, state-of-the-art facilities and subspecialty
support. There are only 14 centers from around the
country currently included in the network.
Clinical research at Magee has advanced beyond
the MFMU. Dr Caritis is also the lead scientist on
another NIH-sponsored multicenter research network,
the Obstetrical-Fetal Pharmacology Research Units
(OPRU) Network. This Network is unique in its focus
on pharmacology during pregnancy. The four sites in the
Network study how pregnancy modifies the metabolism
and elimination of medications taken during pregnancy.
Pregnant women have been totally ignored in this regard
and most of the time receive the same dose of medication
as men and non-pregnant women. Pregnancy has a
dramatic effect on drug handling, necessitating special
dosing consideration.
Recently, the NIH funded another large multicenter
clinical research network to study first-time pregnant
women (NuMOM2B). Dr. Simhan is the lead investigator
from Magee. Entry into this network was highly
competitive and Dr. Simhan has had a major role in
developing the protocol that will be implemented across
the eight centers involved in the network. Numerous
other investigators in the Division of Maternal Fetal
Medicine have been successful in clinical research and
have helped to enhance the research reputation of the
scientific endeavors at Magee.
Basic and translational research also began modestly
at Magee, but major advances were made with the
construction of the Magee-Womens Research Institute
in 1990. “Irma Goertzen, who was the president of
Magee at the time, supported the building of a MageeWomens Research Institute,” Dr. Caritis remembers.
“The Research Institute broadened both our perspective
and opportunities. Our clinical care and clinical research
have always been superb. The Research Institute
has enhanced our ability to also perform basic and
translational research. Very few
programs can succeed in clinical,
translational and basic research, but
the program at Magee has.
“In order to be successful at
research,” Dr. Caritis continues,
“infrastructure is needed.
Resources must be in place that
allow researchers to pose questions

answering the questions. We have
that kind of infrastructure here at Magee.”
Over the years, Magee’s MFM division has evolved
into a world-class center with experts in the field of
preterm birth, pharmacology in pregnancy, preeclampsia,
placental function and fetal growth.
In July 2009, leadership of Magee’s MFM division
transitioned from Dr. Caritis to Hyagriv N. Simhan, MD,
MS. It was a seamless, collaborative transition rooted in
respect and trust. “Hy is the best person in the country
for this job. He has the energy, the expertise and the tools
to take the division further,” says Dr. Caritis.
Dr. Caritis, who directs the MFM Fellowship
Training Program, is available to provide support to his
protégé when necessary. In fact, Drs. Caritis and Simhan
are collaborators on a number of research projects not
just at Magee but with researchers across the country and
around the world.
“This program is unparalleled. There is no better
training program than this one,” says Dr. Simhan. “To raise
the quality of health outcomes for pregnant women and
their newborns meaningfully requires both clinical care
and research to be integrated and excellent.
“I think that the sky’s the limit,“ Dr. Simhan
continues. “I don’t think there is an area in perinatal
research that we can’t do, and do better than anyone else.”
Dr. Caritis seconds that sentiment. “Even though we
have come a long way since 1975, we’re nowhere near
fulfilling our potential. With the talent and resources
available to us, the possibilities are endless.”

DEVELOPING TECHNIQUES TO
PRESERVE FERTILITY
Of the many functional casualties of cancer, the
ability to conceive a child may be among the more heart
wrenching. Kyle Orwig, PhD, director of the Fertility
Preservation in Pittsburgh (FPP) program at MageeWomens Research Institute (MWRI), is working with
his colleagues to educate patients and their physicians
about the reproductive risks of cancer treatments and
options for preserving fertility. For some patients (e.g.,
pre-pubertal boys and girls) there are no established
clinical options to preserve fertility. So, Dr. Orwig and his
team are developing new technologies to preserve and
restore fertility.
The FPP program comprises a multidisciplinary
consortium of researchers, clinicians, ethicists,
administrators and trainees within and outside the
University of Pittsburgh community. Together with
colleagues in pediatric oncology, the FPP program has
established a fertility preservation clinic in the Center for
Fertility and Reproductive Endocrinology (CFRE) of
Magee-Womens Hospital to counsel patients and their
physicians about the reproductive risks of their cancer
treatment. When possible, patients are scheduled for
fertility-sparing procedures before they initiate cancer
treatment.
For patients who have no
option to preserve fertility, the
team has obtained IRB approval to
cryopreserve ovarian and testicular
tissues so that this tissue will be
available to the patient when new
fertility technologies emerge from
the research pipeline. Dr. Orwig
is in the process of setting up a

gonadal tissue cryopreservation
laboratory in the CFRE and is
training the clinical lab staff in gonadal tissue processing
and freezing techniques.
Dr. Orwig’s team is actively engaged in developing
new fertility technologies. One study involves removing
and preserving spermatogonial stem cells from the
testes of a boy or man before he undergoes cancer
treatment. The cells can then be transplanted to restore
spermatogenesis after the malignant disease is cured.
Dr. Orwig’s approach has been successful in the
laboratory and he is working to translate spermatogonial
stem cell transplantation to the clinic for humans.
“By preserving testicular tissue prior to treatment,
male cancer survivors may one day have access to
new fertility-restoring options,” Dr. Orwig says. “The
discussion and decision to preserve gonadal tissue or cells
should occur prior to cancer treatment because some
treatments can irreversibly destroy fertility, including
the potentially therapeutic stem cells. Therefore, it is
important to facilitate discussions among patients, cancer
doctors, reproductive specialists and scientists about
reproductive risks and realistic options on the fertilitypreservation frontier.
“Infertility is an incredibly desperate concern for
some people,” Dr. Orwig explains. “Reproductive issues
are significant contributors to the perceived quality of
life for cancer survivors.” With initiatives to provide
fertility preservation for women, girls, men and boys,
the FPP program may be the most comprehensive and
integrated fertility-preservation center in the country.

Microbicide Trial at MWRI
Holds Promise for
Preventing HIV
The United Nations estimates that half of those
living with AIDS around the world are women. In subSaharan Africa, which accounts for most new cases of the
disease, the difference is even greater. Nearly 13 million
women live with HIV compared to 9 million men. The
typical victims of HIV in Africa are not sex workers or
women with high numbers of sexual partners as many
believe; they are women in a stable relationship whose
husbands were infected with HIV prior to marriage
or have acquired HIV outside of the relationship. Such
sexual “rights,” social pressures and societal “norms” are
what drive Sharon Hillier, PhD, principal investigator of
the Microbicides Trial Network (MTN).
The MTN is an HIV/AIDS clinical trials network
that was established in 2006 by the National Institute
of Allergy and Infectious Diseases, the National
Institute of Mental Health and the Eunice Kennedy
Shriver National Institute of Child Health and Human

part of the U.S. National Institutes
of Health. The mission of the
MTN is to conduct clinical trials
that will lead to the licensure
of products that can be used to
prevent HIV infections in women
and men. Based at Magee-Womens
Research Institute (MWRI) and

the University of Pittsburgh,
MTN oversees 13 clinical trial
units at 20 locations in seven countries, including India,
Durban and the United States. The network has gained
worldwide attention for groundbreaking work with
microbicides — products that are applied vaginally or
rectally or taken orally to prevent HIV.
“Even though we are now 30 years into the HIV/
AIDS epidemic, the only strategies we have available

for prevention of new infections are condoms and
male circumcision. A vaccine appears to be years away,”
explains Dr. Hillier, who is also a senior investigator at
MWRI. “By utilizing efficient, scientifically rigorous
and ethically sound clinical studies, the development of
new biomedical approaches for HIV prevention is within
reach.”
The majority of the network’s studies evaluate
microbicides that women apply vaginally to prevent HIV,
but the MTN recently expanded research to include the
assessment of microbicides when applied rectally in both
men and women. The team also is exploring whether
HIV treatment medications known as antiretroviral drugs
can protect a person from contracting HIV. And in the
summer of 2010, the breadth of Dr. Hillier’s work further
expanded when she and her co-investigators received an
$11.8 million grant to develop small microbicide films,
similar to breath strips, which could greatly decrease the
cost of providing doses of microbicides for daily use.
The team hopes the films will be ready to transition into
the first evaluations in humans within a year.
A recent study demonstrates the promise of using
antiretroviral drugs for prevention of HIV in women.
A study team showed that among 899 women from
Durban and a neighboring community in South Africa,
there were 39 percent fewer HIV infections among
women who used the gel containing tenofovir before
and after sex when compared to those who used a gel
with no active ingredients.
“When it comes to HIV prevention, the road is
long, and there are always mountains to climb, but I
firmly believe that microbicide research will change the
face of the global HIV epidemic in much the same way
that effective contraceptives changed women’s lives 50
years ago,” says Dr. Hillier. “We have an unprecedented
opportunity to demonstrate that a product can offer
both women and men an accessible, inexpensive and
acceptable way of decreasing their risk of HIV regardless
of where they live.”

Sarah Brown Visits MWRI
As world leaders poured into Pittsburgh during
the 2009 G-20 Summit, the Magee-Womens
Research Institute (MWRI) welcomed an important
international visitor of their own — Sarah Brown,
wife of then prime minister of the United Kingdom
Gordon Brown. Prompted to visit by her friend
Teresa Heinz, Mrs. Brown knew that MWRI was a
critical stop on her tour of the Steel City.
The Browns were struck by tragedy in 2001
after losing their daughter, Jennifer, who died only
10 days after being born seven weeks premature. This
heartbreaking incident left them wanting to better
understand and prevent the problems that afflicted
Jennifer, making the groundbreaking pregnancy
and neonatal research being conducted in MWRI’s
laboratories of great interest to Mrs. Brown.
Driven by their thirst for knowledge and
desire to help save other newborn lives, the Browns
established the Jennifer Brown Research Laboratory
in 2004, based at the Queen’s Medical Research
Institute at the University of Edinburgh. The lab
focuses on preventing death and long-term handicaps
that may result from impaired development,
premature birth and other neonatal complications.
Mrs. Brown was accompanied to MWRI by
Teresa Heinz; Pennsylvania Governor Ed Rendell;
Jeffrey Romoff, CEO of UPMC; city leaders; and
guests of Reed Smith, who hosted the event.

As Mrs. Brown stated in her blog after her
visit, “It was inspiring to see the good work that is
taking place at Magee to help women in pregnancy,
premature newborns and long-term protection of
women’s health. I believe that it is so valuable to
scientists around the globe to share their knowledge
and expertise.” MWRI hopes that Mrs. Brown’s visit
will lay the groundwork for a partnership between
themselves and the Jennifer Brown Research
Laboratory.

Research Day Marks Inauguration of New Lectureships
Just as research is critical to advancing medicine,
training is vital to developing excellent physicians.
Thanks to generous philanthropic gifts, two new
lectureships are a valued addition to Research Day at
Magee, an annual event where fellows and residents
of Magee-Womens Research Institute (MWRI) and
Magee-Womens Hospital’s Department of Obstetrics,
Gynecology and Reproductive Sciences (OB/GYN/
RS) gather to showcase their work through oral and
poster presentations.
With a vital interest in supporting research
and nurturing young scientists, MWRIF Board
chairwoman Peggy Joy provided funding for the first
new lectureship, titled The Margaret Prine Joy Lecture in
Reproductive Sciences. Joy’s involvement with MWRI
since the late 1990s fostered her desire to make an
investment in the mission of the Research Institute.
The second new lectureship, Celebration of Life: A
Lectureship Honoring Mothers, was funded by the chair
of the OB/GYN/RS Department, W. Allen Hogge,
MD, and his wife, Joan. The couple were searching
for a way to honor their own mothers along with the
many mothers and families of Magee. The Hogges’
gift will enable the nurturing of other promising
young staff at both MWRI and the hospital.
This year, the lectureships funded keynote
speakers Fred Turek, MD, of Northwestern
University and Mark Phillippe, MD, of the University
of Vermont College of Medicine to share their
expertise at Research Day. Through the generosity
and commitment of both Ms. Joy and the Hogges,
these lectureships will help to inspire residents and
researchers at Magee for years to come.

Stylish Fundraiser Designed by Magee Physician
Specializing in gynecologic oncology, Joseph
Kelley, MD, director of the Division of Gynecologic
Oncology of the Department of Obstetrics,
Gynecology and Reproductive Sciences at MageeWomens Hospital, sees firsthand how cancer
affects women’s lives. Inspired by the strength and
determination of the patients he works with,
Dr. Kelley has taken up another successful
occupation: fashion designer. “Ties (Totes and
Scarves) to the Cure” is a line of stylish accessories
including four different colored ties, a fashionable
scarf and tote bag designed by Dr. Kelley and
Vineyard Vines. Proceeds benefit breast and ovarian
cancer research to save the lives of women affected by
cancer in western Pennsylvania and beyond.
Magee Society to be Launched During
Hospital’s Centennial
For 100 years, Magee-Womens Hospital has
been an integral part of western Pennsylvania. Long
respected for its gynecologic and obstetric care,
Magee has evolved to become a national leader in
providing comprehensive medical care.
In 2011, during Magee’s centennial, MageeWomens Research Institute & Foundation will
introduce a means for people to invest in the work of
both the hospital and of Magee-Womens Research

Institute (MWRI). The Magee Society is a giving
club that will enable donors to ensure that the
work of Magee and MWRI continues well into the
next century. Based on the level of support, donors
will receive special expressions of appreciation,
from invitations to private events to recognition in
publications. But more importantly, these leadership
gifts will help benefit the world-renowned research
and patient care carried out at Magee and MWRI.
Cindy and Christopher Hughes: Moving Past Tragedy
With two healthy children at home, Cindy and
Christopher Hughes were ready to bring another
child into their life. The couple was devastated,
though, when Cindy’s two subsequent pregnancies
failed. Despite their experience, they were touched
by the care, dedication and compassion they received
from the staff at Magee.
As time passed, Cindy and Chris wanted
to turn their tragedy into something positive in
remembrance of their unborn children. The Hughes
chose to open a life insurance policy supporting
resident training education in Magee’s Department of
Obstetrics, Gynecology and Reproductive Services.
The need to continue a relationship with Magee
was important to the family, since they felt that a
bond had been established beyond that of a medical
center providing a service. According to Christopher,
“Giving to Magee was easy — they earned it every
step of the way. Now we feel like we’re part of not
only the Hughes family, but also the Magee family.”
Debra Limbach: A Sisterly Bond in the Face of Cancer
When Debra Limbach learned in 2005 that she
had lymphoma of the cervix, the experts at MageeWomens Hospital were there to help, just as they had
helped her newborn son in the Neonatal Intensive
Care Unit 16 years prior. Her cancer was extremely
rare, but Joseph Kelley, MD, had encountered it
before and was successful in helping Debra become
cancer-free.
Throughout her battle, Debra’s dedicated sisterin-law Elsa was there with a helping hand. “I wanted
to do something meaningful for Debra. She didn’t
need another pair of shoes or a purse,” said Elsa.
“Her only request was help for increased research
into her condition.” Investing in the place that had
saved the lives of both her sister-in-law and her
nephew made perfect sense. Elsa made a donation to
Magee-Womens Research Institute & Foundation
(MWRIF) in honor of Debra to support women’s
gynecologic cancer research. Over the years, Elsa
has continued to support MWRIF. Debra and Elsa’s
sisterly commitment and dedication support a great
cause that helps families everywhere.

“The Magee-Womens Research Institute High School Summer Internship Program is a treasure trove for students. Having that kind of head start is unbelievable,
and it made a difference in my life.The program was certainly the keystone experience
in my life that led me to where I am today. For that, I am forever thankful,”
says Vivek Verma.
Man Bikes 325 Miles to Raise Money for
Breast Cancer Awareness, Education
On June 24, 2010, Joe Scarpaci biked into
Magee-Womens Hospital, marking the end of a 325mile four-day trek from Blacksburg,Va., to Pittsburgh.
Every push of the pedals along the way was fueled by
the memory of his mother, Josie, who lost her battle
to breast cancer in 1998. Joe’s ride raised proceeds for
the Josie Scarpaci Breast Health Access Fund, which
was established through Magee-Womens Foundation.
The fund allows Magee to provide a comprehensive
resource binder to newly diagnosed breast cancer
patients. To date, nearly $25,000 has been raised in
Josie’s name. Through the efforts of her son, Josie’s
life is commemorated with each Magee patient who
