Kimochi Joins In Providing
Transitional Care In San Francisco
If you or someone you know has ever
been hospitalized, you can understand
how overwhelming the experience
can be, particularly when it’s time to
go home. On discharge day, there is
usually a flurry of activity and many
people—doctors, nurses, therapists,
case managers, technicians and
visitors—coming in and out of the
room. Although the hospital staff
reviews the discharge instructions
verbally with the patient and provides
a hard copy to take home, recalling
what was explained can be a challenge,
especially if the person is in pain,
under the influence of medications,
or doesn’t have a good grasp of the
English language. There are special
doctor’s orders to follow, appointments to make, and
prescriptions to fill. Life doesn’t stop.
The collaboration of CBOs and
hospitals is key to helping seniors and
families. This is how the program
works. When patients are admitted
to the hospital, the hospital social
services/case management staff
works on identifying seniors who
meet SFTCP criteria. They must be
a resident of San Francisco, in stable
housing, have Medicare fee-for-service
as primary payor source, referred as a
result of acute medical hospitalization,
and able to benefit from coaching/care
coordination services. Seniors meeting
these criteria are referred to the SFTCP
and a Transitional Care Specialist is
assigned to assist the patient. The
patient receives a hospital visit by the
Specialist before being discharged followed by home visits
and support for a maximum of six weeks. During this time,
the senior and family are provided support and coaching
on all aspects of care including medication management,
follow up care with doctors and other health professionals
identified by discharge orders, assessing home care,
nutrition, transportation needs and coordinating services
when appropriate.
Seniors who are hospitalized often have a difficult time
when they first return home. The first four to six weeks are
often critical in determining whether they will successfully
complete the transition home or whether they will return to
the hospital or require more medical care. Kimochi joined
several organizations and health care providers to develop
a program of support for these seniors. The program is
called the San Francisco Transitional Care Program (SFTCP).
Kimochi, along with other community based organizations
(CBOs) and the Department of Aging and Adult Services,
submitted an application to receive funding support to offer
transitional care in San Francisco. After over two years of
negotiation and submittals, funding was approved!
We have hired a full time Transitional Care Specialist for this
important program expanding our capacity to help seniors
and families. We look forward to supporting seniors and
their families as they make the transition from the hospital
back to home.

I truly enjoy the beginning of a New Year. For us in the Japanese American community
there is always the traditional greeting of “Happy New Year,” but also in nihongo there
is the traditional greeting of “akemashite omedetou gozaimasu.” You then remark,
“kotoshi mo yoroshiku onegaiitashimasu” which again loosely translated is, “so will I
ask a favor/request of you in return.” What’s good about this traditional greeting is that
we get to say it each New Year.

This year marks Kimochi’s 42nd anniversary as well as the Kimochi Home’s 30th
anniversary. We are going to have a celebration on May 18, 2013, Saturday, at the
Kabuki Sundance Cinema theatre in San Francisco. Please come and join us. We will
have a program celebrating the Kimochi Home, our history, programs and services, and
most importantly our residents like Mrs. Tameko Shijo, who just celebrated her 109th
birthday on New Year’s day. She is a great example of the kind of quality care Kimochi
strives to provide in a community-based, Japanese American environment.
2013 is shaping up to be a monumental year for Kimochi thanks to all of you!

Even as Kimochi recognizes all
those who helped it sustain its
programs and services for 42
years, it will also acknowledge
its hopes for the future. Beginning in 2013 Kimochi will
unveil plans for its Kimochi San Mateo facility and services.
An update on this project will also be presented at the
anniversary party.
Additionally, Kimochi kai itself will celebrate 42 years of
senior service. The May event program will consist of an
outstanding line up of celebrated community performing
artists. Masayuki Koga and Tim Hamano will play the

Coming up on May 18, the 30th anniversary
of the opening of the Kimochi Home will
be commemorated with a special program
at the Kabuki Sundance Cinema theater.
The Kimochi Home at 1531 Sutter St.
has been providing residential and respite
care programs since it was built in 1983,
following a comprehensive community
fundraising campaign. The Home’s programs
include 24-hour secure supervision, nutritious meals, and
intergenerational adult social
day care activities. Rick Quan,
well-known in the Bay Area for his sports casting, will debut
a special video recognizing the achievements of the Kimochi
Home at the event. Robert Handa of KTVU Fox 2 News
and a long time Kimochi supporter is carrying on master of
ceremonies duties.

A Holiday Shopper’s Paradise!
The St. Mary’s Event Center parking lot filled up quickly.
With the venue so near San Francisco’s Japantown,
Kimochi provided shuttle service to/from Japantown to St.
Mary’s Event Center. The shuttle helped to alleviate the
crowded parking and for those coming from out of town it
was a nice way to enjoy Silver Bells and experience
San Francisco’s Nihonmachi.
On Saturday, December
15th, 2012 thousands of
shoppers came from near
and far for some fun holiday
shopping at Kimochi’s Silver
Bells Arts & Crafts Faire at
the St. Mary’s Event Center
near San Francisco’s Japantown. There were over 90
vendors participating (the most ever!) selling their Asian
and Pacific Islander themed arts & crafts from jewelry to
scented candles to handmade stationary. Also food vendors
were the most ever as well, selling their yummy treats from
cookies to candies, chocolates to popcorn, and jerky to
licorice. With so much to look around, it was a shopper’s
paradise!
On February 2, 2013, Kimochi program volunteers, board
members, staff and guests gathered on the second floor
of Tong Palace in San Francisco to enjoy a banquet-style
lunch. Program volunteers from Kimochi Lounge, Social
Services, Nutrition, Kimochi Home, and Kimochi Senior
Center had a chance to sit, relax, socialize and be served,
and every guest went home with a raffle prize and a
special appreciation gift from Kimochi. Raffle prizes were
donated by Shig and Sumi Higaki, Leona Nakagawa of
Creative Accents, and Steve and Etsie Nakajo.

Kimochi’s Silver Bells Arts & Crafts Faire has been growing
each year attracting more and more shoppers and here’s
the reason why: everyone feels good while they shop,
knowing that a portion of all purchases helps support
Kimochi’s programs and services for seniors!

from our participating vendors. Everyone had multiple
chances to win some fabulous prizes!

Kimochi is a very important agency to the community. Not only does Kimochi meet the
needs of seniors, it also helps strengthen the community as a whole by bringing people
together to work for a common goal. I am committed to Kimochi and its tireless efforts
to touch the lives of so many of today’s seniors and families. My role as a volunteer is
just a small contribution towards continuing Kimochi’s legacy of providing care and
support for seniors.

We would also like to thank all of our many Fundraiser Volunteers who have continuously volunteered their time in making our
numerous events successful throughout the year. Your dedication and tireless efforts are very much appreciated by the Board of
Directors, staff and seniors as it ensures the continuation of Kimochi’s many programs and services.
Access to transportation is a freedom we don’t fully appreciate
or think about until we lose it. It’s even a bigger loss for
many older adults who can no longer drive or take public
transportation to appointments, to go shopping, or to visit
family and friends. They often must depend on taxis, family,
friends and neighbors for transportation or risk their safety
walking and taking public transportation just to get from
point A to point B.
Fortunately, Kimochi
Transportation is an available
option for eligible seniors in San
Francisco. Kimochi provides
door-to-door and group van
services, so passengers can sit
back, relax and let Kimochi do the
driving. Kimochi’s fleet includes
four paratransit vehicles, two
mini vans, and one Toyota Scion.
Drivers are specially trained
to deal with the needs of older
adults. The requested donation amount for a one-way trip is
only $1.00 (requested amount may vary for rides outside of
San Francisco).
Besides regular weekday runs to and from Kimochi Nutrition,
Kimochi Senior Center and Kimochi Home, round-trip
transportation can be scheduled in advance to appointments
(doctors, lab tests, physical therapy, X-rays). There are also
special shopping trips to Safeway,
Trader Joe’s, and Costco, and
other destinations like Westlake,
Lakeshore Plaza, and 280 Metro.
Kimochi even provides monthly
trips to the cemeteries in Colma,
so people can continue paying
respects to long lost family
members and friends.

Ohakamairi (Cemetary Trips) with Kimochi!
The Japanese tradition of ohakamairi, visiting long lost family
and friends at the cemetery, remains a very important practice
for many people, which is why Kimochi Transportation
continues its regular monthly trips to Colma on the first
and last Mondays and Fridays of each month. Besides the
Japanese cemetery, visits to the local flower shop and other
cemeteries can also be arranged. Reservations are required
and a minimum of 3 passengers must attend for the trip to
take place. Pick-up and drop-off location is in front of the
Kimochi Senior Center, 1840 Sutter Street, between Webster
and Buchanan Streets. Departure time is approximately 1:00
PM and return time is approximately 4:00 PM. The requested
roundtrip donation amount is $10. What a deal!

I’ve been working here for almost 18 years now. I have a hard time understanding the
language, but my co-workers have always been there to help me. I feel my job is stressful,
but at the end of the day helping the elderly is a job worthwhile. I always remember the

Sansei (third generation Japanese Americans) realized that because of language and cultural barriers, many elderly Issei (first
generation Japanese Americans) were unable to take advantage of mainstream social services. The organization formed as a
result of those Sansei volunteers began looking for ways to assist the older generation. Today, Kimochi, Inc. provides care and
support to more than 3,000 seniors and their families throughout San Francisco.
Kimochi, Inc.
The Kimochi Lounge, located within Japan Center, is the agency’s original site.
Seniors come to “the lounge” to catch up on community news and socialize. The site
also houses a book and video library where seniors can check out materials for free.
In 2007, a new senior information and resource section was added to have important
health, safety, nutrition, and caregiving resources available for seniors and families. In
July 2009, we opened the lounge for seniors and families on Saturdays.
This site is annually visited by 5,000 seniors, families, and visitors from throughout
California and other countries.

Administration
The Administrative building is owned and operated by the agency and houses the administrative staff. We have approximately
200 volunteers assisting us each year in our programs.
Social Services Program
Kimochi’s Social Services program provides Japanese and Korean language
translation, information and referral, naturalization assistance, counseling, assistance
with insurance and government benefits, seminars and workshops on aging. Case
management services help clients and families identify additional resources to keep
seniors at home and independent for as long as possible. The program also provides
English and Japanese Family Caregiver Support Services. This service targets
spouses, siblings, and children who are actively involved in caring for a senior and
provides them with counseling services, family caregiver support groups, assistance
with hiring caregivers to help the family, and 24 hour respite care placements.
Kimochi collaborates with the San Francisco Transitional Care Network. The
program targets isolated seniors discharged from acute care hospitals who benefit
from more community based care and support as they transition home.
The program annually assists 600 seniors and families providing 5,000 hours of
services.
Club Nikkei
Club Nikkei is a membership based program for targeted seniors in our community.
It is designed to outreach and attract healthy, independent seniors who are interested
in joining a group for exciting overnight trips, dinners, and activities. Staff plans
quarterly calendars and all transportation for excursions is provided by our
transportation program.
Home Safety Service
Kimochi offers a home safety program for senior homeowners and renters in San
Francisco. Staff conducts home safety assessments and helps seniors identify
potential and existing safety issues that could compromise their ability to live safely
and comfortably at home. Staff can assist and provide smoke detectors, stair strips,
and other items free of charge. We have also compiled a referral list of contractors,
electricians, and plumbers who can help seniors in maintaining and repairing their
homes.

Nutrition
A nutritious well-balanced Japanese style hot lunch is provided 5 days a week. Any
senior 60 years of age or older is welcome. A five week menu is produced by staff,
and our nutritionist ensures each meal is healthy, nutritionally well-balanced and
adequately proportioned for seniors. An average of 300 meals is served daily.
The program annually serves 1,200 seniors 75,000 lunches.
Home Delivery Program
Individuals who are physically unable to travel to the nutrition site due to a
disability, illness, or recent hospital discharge, can receive hot meals at their homes.
Approximately 120 lunches are delivered to homebound seniors living in Japantown,
the Western Addition, Sunset, and Richmond districts of San Francisco.
The program annually serves 130 homebound and frail seniors 29,000 lunches.
Senior Center
In addition to enjoying lunch at the nutrition program, seniors are encouraged to
participate in activities at the center. Staff and volunteers plan and offer a variety of
activities on a monthly basis. We have morning exercise classes, a ceramics class,
group singing classes, odori, bingo, arts and crafts, day trips, and overnight trips.
The program annually serves 300 seniors and offers 700 hours of exciting activities.
Transportation
The transportation program offers seniors the opportunity to use safe and reliable
door-to-door van services which help them maintain a connection with their
community and friends. Group rides are provided to and from seniors’ homes to the
nutrition site, Kimochi Home, and Japantown. Cemetery trips, senior center trips,
grocery trips, and medical trips are also provided.
The program annually assists 150 seniors receive 10,000 rides.

Adult Social Day Care Program (License # 380504138)
This is a program licensed by the State of California to provide seniors with a
schedule of social and recreational activities. Staff trained in programming and
providing services to seniors requiring social, emotional and physical support
develop a monthly activity calendar which includes daily exercise, lunch, singing,
bingo, video viewing, trips, and special cultural and intergenerational activities.
The program annually assists 40 seniors receive 21,000 hours of services and
activities.
Residential/Respite Care (License # 380504099)
The residential program is licensed by the State of California to provide 24 hour care
to ambulatory seniors. It has the capacity to house up to 20 seniors in either single
or shared rooms. They receive supervised, non-medical care and assistance with
activities of daily living such as bathing, dressing, and taking medications.
Respite care is 24 hour care for seniors in need of short term placements. Temporary
stays are available for seniors needing a place after a hospital discharge, when family
is out of town or in need of respite, and for seniors interested in potential long term
placement as a resident
