Consequence UK (CUK) is an established and respected independent company that works exclusively with
NHS and Social Care organisations. Its specialist field is investigating and learning from adverse events
resulting in moderate to severe or catastrophic harm. Its managing director Maria Dineen was a key contributor to
the National Patient Safety Agency’s (NPSA’s) original investigation model and framework for investigating adverse
events in England. Her ‘how to’ investigate manual ‘Six Steps to RCA’ is a referenced text in the recently revised
SI framework for England.
CUK has been training NHS and Social Care professionals in ‘how to investigate effectively’ using systems
analysis principles (including root cause analysis) since 2002, making CUK one of the longest standing providers
of investigation training to the NHS since the inception of the NPSA.
Why is this workshop important?
This workshop will:
Adverse Incidents are part and parcel of everyday life
in all NHS Trusts, private providers and Social Care
organisations. Some incidents occur in spite of our very
best efforts, and some because we made a mistake.
•
Give you clarity about ‘what a good investigation’
comprises
•
Provide you with information and practice
experience across a range of tools and techniques
you can utilise across different investigation types
•
Provide you with a structured process, that enables
you to make sensible decisions about when and
where to stop an investigation, and when and where
‘root cause analysis’ is required
•
Set out the core features you ought to include in
your recommendations
We need to know what we are doing when we conduct
an investigation and we need clarity of purpose.
•
Set out the key features of a meaningful action
implementation plan
Filling in an investigation template, constructing a
‘timeline’ and ‘doing a fishbone’ does not deliver the
above points.
•
Explore with you ‘the effective investigation report’
- what must it achieve?
•
Provide opportunity to focus on the meaningful
delivery of Duty of Candour
To enable us to:
•
•
Provide patients, clients, families and victims, with
answers to sensible questions (thus meeting Duty of
Candour in full)
Optimise our learning and safety improvement
opportunities
What our clients say about CUK workshops
•
A very worthwhile workshop that was informative & that should be available for all staff undertaking RCA
(Pennine Acute NHS T 2015)
•
Lifts the perspective of the purpose & potential of investigations to a new level (Wigan CCG 2015)
•
I feel I have a more comprehensive understanding of RCA processes to enable me to more
effectively undertake CIR'S (Hampshire County Council, Safeguarding Adults 2014)
•
The facilitator has energy. Myths will be busted & eyes will be open to different
approaches... Because they exist (Warrington CCG 2015)
•
It will help you understand the RCA process more fully & help provide structure/clarity and provided useful tools
(NHS Tayside 2015)
•
Content analysis of 'howlers' & using source data to analyse them (Leicestershire and Rutland
Serious Case Review Board 2014)
•
Excellent learning opportunity. As a lead investigator it equips you with the right tools & knowledge
(NHS England-North Learning Event 2015)
•
Opportunity to learn skills which are transferable across many different aspects of role not just incident
investigation (North Devon Healthcare NHS Trust 2015)
•
Excellent facilitation, lots of live/real life scenarios & clear understanding from an early stage of reviewing
& report writing (Combat Stress 2013)
Who should attend:
This workshop is intended for NHS and Social Care staff, staff working in a private health facility and Registered
Care Home Managers who are required to act as ‘lead investigation officers’ following all moderate to serious
incidents including: ‘never events’, mental health, homicide, domestic homicide, safeguarding children and
vulnerable adult investigations.
This workshop also has merit for managers who either ‘instruct’ others to investigate, or undertake the
quality assurance review of the investigation and its report.
