You’re joining senior executives, clinical leaders, and trustees who are committed to helping their organizations provide better health care in their communities through innovative approaches to achieve exceptional performance. Our theme this year, Delivering Greater Value: New Business and Care Delivery Models, reflects what we believe to be the right goal at the right time as the nation’s hospitals and health systems play a pivotal role in the transformation of the delivery system—defined by high quality, coordinated care, efficient resource management, and exceptional patient experience. Incorporating promising components of prevention and wellness and a culture of healing will further distinguish our health care system in the 21st century. While the rules and regulations of health care reform continue to roll out of Washington and developments proceed at the state level, new models for improving care delivery and business processes are unfolding at an unprecedented rate in local markets everywhere. And as performance expectations continue to escalate along a range of dimensions, it becomes even more critical to be in touch with the latest ideas and have direct exchange with your peers, so that new and effective methods are spread widely and adopted successfully. We encourage you to make the most of your Summit experience by actively participating in the educational sessions and in networking events when you can engage with colleagues and business partners. Our seven content areas are designed to help you accelerate advances in quality, safety and efficiency, and they provide valuable strategic direction while showcasing excellence at the patient care and operational level. The Summit will help you respond to the complex and evolving demands of your role. We are confident that you will leave with new ideas for leading your organization and positioning it for a successful future. But, we also want you to enjoy yourselves and to take a moment to celebrate the extraordinary work that you and your colleagues are achieving in hospitals and health systems across America. With the winners of some of the nation’s most distinguished health care awards and many of our most admired hospital leaders in attendance, there is much by which to be inspired. Please make an effort to meet them, along with our sponsors and exhibit company representatives. WORKSHOP #1 WORKSHOP #2 WORKSHOP #3 Culture of Health—Transformative Models of Wellness in Health Care Institutions Jeanene R. Martin, MPH, SPHR, SVP, Human Resources, WakeMed Health & Hospitals, Raleigh, NC; Corey Heller, Corporate Vice President and Chief Human Resources Officer, Baptist Hospital, Miami, FL; and Dan Zuhlke, VP, Human Resources, Intermountain Healthcare, Inc., Salt Lake City, Utah. Moderated by Jeffery Payne, SPHR, Immediate Past President, ASHHRA, Vice President, Human Resources, Lakeland Regional Medical Center, Lakeland, FL Getting Your Team in Shape for the 2016 Health Care Games Nathan Kaufman, Managing Director, Kaufman Strategic Advisors, LLC, San Diego, CA Employing Lean and Six Sigma Principles to Optimize Financial and Clinical Outcomes Ian R. Lazarus, FACHE, Director, Kaiser Permanente’s KP OnCall, San Diego, CA and Tiffany A. Davis, Associate Partner, Creative Healthcare, Scottsdale, AZ In today’s health care environment, leaders have an opportunity to significantly reduce their insurance costs by creating a culture of health. But the benefits don’t stop there. In this practical, interactive session, you’ll hear from industry experts and practitioners on how a healthy employee base will help you to better serve your patients. You’ll gain knowledge around best practices that are effective in any hospital or health care setting. Discover how to embrace a culture of health in your organization as you: • Examine the findings and recommendations from the American Hospital Association’s 2011 report A Call to Action: Creating a Culture of Health • Learn to create partnerships with your employees around the importance of wellness both within individual organizations and across the field • Understand why the business case for wellness initiatives is becoming more compelling for health care leaders • Explore models of wellness excellence that can be integrated within your organization, whether small or large, and benefit from the experiences of others to avoid “reinventing the wheel” The recent passage of the Patient Protection and Affordable Care Act (PPACA) has created uncertainty about the future of the nation’s health care delivery system. Regardless of how PPACA is implemented, or funded or modified, PPACA and other government initiatives e.g., value-based purchasing, have signaled to providers that that they will have to offer care that is more coordinated, efficient and of high measureable quality. Health systems will have to begin today to invest in new competencies in order to be positioned for the health care system of the future. These competencies include: employing physicians without breaking the bank, clinically integrating to make care more predictable and coordinated, and redesign to eliminate waste and unnecessary variation of care. In order to accomplish this, the individual providers within a health system must trade their autonomy for a team culture. As we learned in the early 90’s, investing in the organizational structure de jour, can be costly. Without proper leadership, culture and execution, efforts to employ physicians, clinically integrate and create ACO’s could actually be worse for an organization than doing nothing! That is, do it right or don’t do it at all. This workshop will provide participants with a likely scenario of what the health care system will look like in 2016, and the critical competencies necessary for success. For each competency, there will be a discussion, using specific examples of the underlying factors that will determine whether the strategy will succeed or fail. With the reality of the health care reform legislation taking hold, health care organizations must seriously and systematically look at their delivery processes and make sure any inefficiency is driven from their processes. Cutting costs without adversely impacting quality is dependent on finding the sources of waste and driving them out. Participants in this highly interactive workshop will thoroughly explore the tools, techniques and outcomes of Lean Principles, which enable all health care organizations to immediately improve flow by removing the bottlenecks caused by waste—whether they be structural, procedural or human. Workshop participants will: • Understand key differences between Lean and Six Sigma, and how each method is applied in health care • Participate in “proof of concept” exercises to witness the application and potential of the principles in real time • Examine actual case studies from organizations that have employed the principles • Discuss alternatives to deployment, senior management considerations, and success criteria The program has been approved for 3 Category I (ACHE Education) credits. • Learn to create compensation, rewards, and performance evaluation processes that align with wellness initiatives 7 Sunday, July 17th Summit Opens — Sunday, July 17th 7:00 am–6:00 pm 2:30–2:45 pm Registration Douglas Pavilion Foyer Break Manchester Ballroom Foyer 1:00–2:30 pm 2:45–4:00 pm Presentation of the AHA-McKesson Quest for Quality Prize® and Keynote Presentation Presentation of the Dick Davidson Quality Milestone Award for Allied Association Leadership and Keynote Presentation The Voice of a Generation Manchester Ballroom Tom Brokaw, Legendary Newsman, NBC News and Author Tom Brokaw draws on a rich career in network news, throughout which he has traveled the world to cover elections, summits, wars, political turmoil and major news events of every description. CORPORATE UNDERWRITER Track 1 Excelling in Quality Outcomes and the Patient Experience Setting Priorities, Measuring, Benchmarking, and Excelling… Lessons from the Cutting Edge of Quality Elizabeth A Zeff Ross, Senior Vice President & Chief Executive Officer, Memorial Regional Hospital, Hollywood, FL; Jay Anderson, Vice President of Quality and Operations, Northwestern Memorial Hospital, Chicago, IL; Joan Brennan, DNP, Vice President Quality and Performance Excellence, AtlantiCare Regional Medical Center, Atlantic City, NJ. Moderated by Jeffrey D. Selberg, Executive Vice President & COO, Institute for Healthcare Improvement, Cambridge, MA and Chairman of the AHA-McKesson Quest for Quality Prize Committee The Institute of Medicine envisions a transformed, high-quality health care system that is safe, patient-centered, effective, efficient, timely, and equitable. The AHA-McKesson Quest for Quality Prize honors hospitals that are leading the way in developing integrated, sustained, and highly reliable systems and approaches to achieving these six aims. Come hear from the 2011 honorees how they are pursuing excellence in care delivery. CORPORATE UNDERWRITER The Dick Davidson Quality Milestone Award for Allied Association Leadership The Dick Davidson Quality Milestone Award for Allied Association Leadership is presented annually to a state, regional or metropolitan hospital association which 8 demonstrates exceptional organizational leadership and innovation in quality improvement and has made significant contributions to the measurable improvement of quality within its geographic area. Sunday, July 17th Track 2 Strategies for Efficient and Effective Resource Utilization Found Money Case Studies: How Lean Practices Helped Sacred Heart Hospital’s Leaders Dramatically Reduce Controllable Expenses Elizabeth B Faye L. Deich, RN, MS, NEA-BC, Chief Operating Officer, Sacred Heart Hospital and Senior Vice President for Hospital Operations, HSHS Division (Western Wisconsin), Eau Claire, WI This session will focus on the Lean best practices and responsible stewardship principles that have empowered Sacred Heart’s leaders to reduce controllable expenses by over $5,000,000—without layoffs and without sacrificing quality or harming physician relationships. Ms. Deich will explore how the successful initiatives began with the formation of crossfunctional teams, each with a financial target based on the expense amounts managed by that group’s departments, and describe the deliberative process that led to final decisions. She will illustrate the importance of vigilance and continual improvements for sustainable savings and demonstrate how an innovative stewardship model for resource utilization can serve as a tangible witness of a hospital’s constant connection to mission. In this highly practical presentation, participants will learn how strategic performance improvement can positively impact an organization when access to capital is problematic and bad debt and charity care rise. CORPORATE UNDERWRITER Track 3 Emerging Models of Physician Engagement How to Have a Margin Discussion with Your Doctors Elizabeth C Nicholas Sears, MD, FACS, Chief Medical Officer, MedAssets, Centennial, CO Hospital executives struggle daily with razor-thin operating margins. Clinical practice patterns, especially by physicians in key specialty areas such as orthopedic surgery, neurosurgery, cardiac services and high-volume medical admissions, make the difference between profitable or unprofitable service lines. Whether these physicians are partners or adversaries can depend on how well and how often hospital management keeps them “in the loop” on hospital financial matters. This session will prepare participants to conduct productive discussions with their physicians that will enhance physicians’ understanding and appreciation of the challenges and how they can help. You’ll understand the rationale of involving physicians in operating margin discussions, gain ready-to-use slides on national trends that introduce and frame the topic for physician discussions, and hear examples of data as the lingua franca between administrators and clinicians. In addition, Dr. Sears will identify specific “call-to-action” items for key physicians, strategies to deal with problem physicians, a checklist of areas to address with physicians in improving margins and a communication plan for ongoing interactions. CORPORATE UNDERWRITER Track 4 Creating a Culture of Health and Healing Improving Quality of Life for Employees and Patients by Creating an Optimal Healing Environment Elizabeth D Wayne Jonas, MD, President and CEO, Samueli Institute, Alexandria, VA; Diane Gage Lofgren, Senior Vice President, Brand Strategy, Communications and Public Relations, Kaiser Foundation Hospitals and Health Plan, Oakland, CA; and Todd Linden, President & CEO, Grinnell Regional Medical Center, Grinnell, IA In this session, you will learn why creating an “optimal healing environment” (OHE) in your hospital can not only help create a culture that not only supports health and promotes safety and quality but can provide demonstrable cost savings, improved health and quality of life for employees along with a reduction in employee turnover. Creating an OHE is a new way of thinking about how health care is delivered—by taking into consideration all factors that influence the optimal healing of the patient—from the outer environment (built environment, organizational culture) to the inner environment (intention, relationships between providers and patients). CORPORATE UNDERWRITERS Track 5 Integrating Care Delivery Chasing Unicorns: A Leadership Conversation around the Future of ACOs Elizabeth F Chris Van Gorder, President and CEO, Scripps Health, San Diego, CA; Nancy M. Schlichting, President and CEO, Henry Ford Health System, Detroit, MI; Deborah Proctor, President and CEO, St. Joseph Health System, Orange, CA; and Frederick Hessler, Managing Director, Health Care Group, Citigroup, New York, NY. Moderated by Ian Morrison, Author, Consultant and Health Care Futurist, Menlo Park, CA Ian Morrison writes that at their very best, ACOs could be a powerful, successful rework of Alain Enthoven’s managed competition notion, which many thought was a pretty decent reform compromise the first time around. At its worst, the ACO could be a badly defined mishmash of half-baked ideas and experiments that is an orgy of excess for lawyers and consultants. Mr. Morrison counsels that before we embark on this path to accountable care, we should all start with a little self-awareness and good old-fashioned humility about core competencies to manage the risk for and outcomes of care for a defined population. This panel of leaders will do just that, sharing their perspectives on ACOs, and more broadly on integrating care delivery. In this engaging conversation, you’ll learn how they are preparing for the changes in payment and care delivery poised to roll out in the coming years. What does it take to balance a focus on the structural efforts to link providers, with a redesign of clinical care for greater value and improved population health? Learn what your organization should do now to be ready for the new ways that you will be held more accountable, whether as part of a formal ACO, or not. CORPORATE UNDERWRITERS 9 Sunday, July 17th Track 6 Managing Populations and Care Under New Payment Models Medical Home: A System of Care to Achieve Improvements in Cost, Service, and Quality… the “Triple Aim” Elizabeth G Nancy McClure, Senior Vice President, HealthPartners Medical Group, Minneapolis, MN; Beth Averbeck, MD, Associate Medical Director, Primary Care, Health Partners Medical Group, Minneapolis, MN; and Melissa McCain, Principal, The Chartis Group, Boston, MA While most leading health care delivery systems have begun to assemble many of the components required to deliver more consistent, higher quality, integrated care, few have yet to realize that vision. In 2004, HealthPartners Medical Group introduced an innovative program that successfully integrates care delivery around a Medical Home model. This program is based on four key design principles: consistency (in care processes), customization (to the individual), convenience (of access to care and information), and coordination (of care across sites, specialties, conditions and time using a Medical Home model). HPMG has applied this innovative approach to its entire patient population, across all payers and conditions, achieving significant system-wide improvements in health outcomes, patient satisfaction and cost of care—the Triple Aim. Participants will learn about implementation approaches to integrating the care delivery model and managing population health to simultaneously achieve superior health outcomes, outstanding patient experience, and improved affordability of care. PLATINUM UNDERWRITER Track 7 Leveraging Clinical Information Systems for Improved Performance at the same time; and to use a complex EHR in a mid-sized hospital setting, with rollout in less than 6 months. Participants will learn how FRHS ensured the success of this bold deployment by obtaining physician buy-in of the new CPOE system; populating the EHR with visit histories, a master patient index, historicalto-current visit results, transcribed reports, allergies and order catalogs; and making the solution work with a modestly-sized IT staff. Additional discussion will focus on how the hospital was able to assess its nursing staff, which was reorganized in conjunction with the EHR launch. CORPORATE UNDERWRITERS Mid-Sized Hospital EHR Deployment: A Realistic Approach to Managing Unrealistic Expectations Elizabeth H Jim Sinek, President and CEO, Faith Regional Health Services, Norfolk, NE and Becky Quammen, CIO, Faith Regional Health Services, Norfolk, NE With the HITECH provisions of the ARRA poised to infuse incentive dollars into health care organizations, those facilities without these systems are scrambling to find ways to go live. Faith Regional Health Services made the commitment to buck three major industry trends in regards to the IT deployment: to operationalize a new patient bed tower while simultaneously taking an EHR live; to go live with CPOE 5:30–7:00 pm Welcome Reception Exhibit Hall (Douglas Pavilion) Renew acquaintances, network and get a look at the products and services offered by the exhibiting companies that support the Summit. Beverages and light hors d’oeuvres will be served. CORPORATE UNDERWRITER Monday, July 18th 7:00 am–6:00 pm Registration Douglas Pavilion Foyer 7:00–8:15 am Continental Breakfast Exhibit Hall (Douglas Pavilion) 7:00–8:15 am Sunrise Sessions Continental breakfast provided. Charting a Path toward Meaningful Use: Lessons from Most Wired Hospitals Elizabeth F Martin Fattig, CEO, Nemaha County Hospital, Auburn, NE; Suzanna Hoppszallern, Senior Editor, Data and Research, Hospitals & Health Networks, Chicago, IL; Andrew Mellin, MD, MBA, VP, Predictive Care, McKesson Corp., Roseville, MN; and Betsy L. Thompson, MD, DrPH, Chief Medical Officer, Centers for Medicare & Medicaid Services, Region IX, San Francisco, CA. Moderated by Matthew Weinstock, Senior Editor, Hospitals & Health Networks, Chicago, IL Health information technology has been thrust into the national spotlight thanks to the release last year of so-called meaningful use regulations. Hospitals big and small are trying to figure out how— really if,—they’ll be ready to apply for their share of the $27.3 billion the federal government has authorized to help foster the use of electronic health records. But officials at leading IT hospitals know that this isn't just about deploying technology for technology's sake; it's about using the technology wisely and becoming highperforming, high-quality organizations. Meaningful use can't be viewed in isolation. Health reform will force hospitals to change the way they operate and information technology is a key component of that transformation. In this interactive Monday, July 18th session, you'll hear the result of two AHA surveys measuring the adoption rate of health IT. Executives from hospitals appearing on H&HN’s Most Wired list will offer insight into how they are deploying IT. Improving Health Care in America Insidious Intimidation: The Underestimated Threat to Patient Safety As a practicing surgeon and an accomplished writer, teacher and speaker, Dr. Atul Gawande offers a unique perspective on the joys and uncertainties that lie at the heart of modern medicine and a thoughtful, sensible approach to U.S. health care reform. His 2009 New Yorker article, “The Cost Conundrum” was called “the most influential health care story in recent memory,” “the must-read story of the year,” and “required reading” by almost every influential person in health care, including President Obama. His previous books, the National Book Award finalist Complications, and the bestselling Better: A Surgeon’s Notes on Performance, captured the public’s attention on the need for improvement. Sharing insights of his latest book, The Checklist Manifesto: How to Get Things Right, Dr. Gawande speaks eloquently and with passion about how to improve quality, reduce errors, manage risk, and measure performance in health care and beyond. Elizabeth G Daniel Zuhlke, Vice President, Human Resources, Intermountain Healthcare, Inc., Salt Lake City, UT and Theresa Zimmerman, Theresa Zimmerman Consultants, LLC, Stilwell, KS Health care leaders have an opportunity and an obligation to reduce the threat of insidious intimidation to patient safety. By working collaboratively to compare and contrast different unit perspectives, data sources, and shared knowledge, organizational leaders have an opportunity to powerfully demonstrate and then influence the cause and effect link between personal behaviors and patient outcomes. Learn how to eradicate insidious intimidation and its threat to patient safety through a “no-tolerance” approach that involves talent management as well as training and equipping all hospital personnel with the knowledge, tools, and resources needed to recognize, respond to, and eradicate intimidating behaviors. CORPORATE UNDERWRITER Track 2 Strategies for Efficient and Effective Resource Utilization “The Amazing Race” for Patient Throughput Improves Hospital Efficiency Elizabeth B Vicki LoPachin, MD, Medical Director, North Shore University Hospital, Manhasset, NY Success under health care reform will require a firm grasp of processes that effect efficacy and improve operational performance. Dr. LoPachin will demonstrate how North Shore University Hospital employed a structured team approach to streamline patient throughput, promote the use of evidence-based practice protocols and minimize variability in care processes, thus dramatically improving operational efficiency. She will describe the methodology used in planning the program, how baseline length of stay, excess days, and denial data was prioritized to identify the DRGs where the greatest opportunity for improvement existed. You’ll see how combining operational, clinical, financial processes will improve hospital outcomes and better align organizations with a common goal to improve quality and create capacity while controlling cost. And better yet, the organizational structure used in the “Amazing Race” model can be replicated by any hospital for addressing length of stay/excess day issues or any other complex interdisciplinary issue—all without any additional resources. CORPORATE UNDERWRITER 11 Monday, July 18th Track 3 Emerging Models of Physician Engagement Advanced Approaches to Primary Care in an Era of Reform Elizabeth C Craig Holm, FACHE, Senior Vice President, Health Strategies & Solutions, Inc., Philadelphia, PA and Martin Lipsky, MD, Regional Dean, University of Illinois College of Medicine at Rockford, Rockford, IL As the debates about health care reform dominated political discourse in 2010, the elephant in the room was that the foundation for successful reform must be a robust primary care infrastructure. Health care organizations are facing the daunting challenge of building a robust network of primary care physicians under very dire circumstances, including a rapidly evolving commercial, non-government payor environment that is in some ways far more advanced than Medicare initiatives embedded in reform legislation. While a major component of the primary care challenge for health care organizations relates to recruitment, equally important will be taking proactive and innovative measures to create a new role for primary care within provider organizations and establishing and executing new standards for patient care interactions, care management, accountability for outcomes, quality, and health status as never before. In this session, successful approaches for primary care physician recruitment and retention will be discussed along with approaches for proactively managing the changing role of primary care providers. Case studies will be used to illustrate real world approaches for redefining the role of primary care providers for optimal positioning under health care reform. CORPORATE UNDERWRITER Track 4 Creating a Culture of Health and Healing The Human and Organizational Impact of Creating an Authentic Caring Culture Elizabeth D Mina Ubbing, President and CEO, Fairfield Medical Center, Lancaster, OH; Ann Foss-Durant, MSN, RN, Director of 12 Caring Services Integration, Kaiser Northern California Regional Patient Care Services, Oakland, CA; and Bruce Cryer, President and CEO, HeartMath, Boulder Creek, CA. Moderated by Todd Linden, President and CEO, Grinnell Regional Medical Center, Grinnell, IA This panel will discuss the scientific research, organizational benefits, and bottom line impact of creating an authentic caring environment within health care. Creating such an environment is key to creating a culture of health and healing for both patients and caregivers. This alignment has a vital impact on the delivery of care and the overall healing experience. Results, experience and practical applications will be discussed looking at the daily operations and impact of integrating HeartMath tools and technologies into the clinical setting. This information will be furthered anchored by return of investment data on health care costs savings, lost time and staff and patient satisfaction. CORPORATE UNDERWRITERS Track 5 Integrating Care Delivery Keys for Success—Clinical Integration and Positioning for Accountable Care Elizabeth F Hans G. Wiik, FACHE, MPH, MHA, RPh, President and CEO, Centura integrated Physician Network, Louisville, CO and David Richard Ehrenberger, MD, CMO, Avista Adventist Hospital and Centura integrated Physician Network, Louisville, CO This session will outline the structural model and success factors for a clinically integrated network of 21 independent multi-specialty medical practices and providers, including a large Federally Qualified Health Center, which has significantly improved patient outcomes, while also improving financial performance with ACO positioning for new payment models on the horizon. The model focuses on the missing ingredients for population health management by placing primary care at the forefront, and integrating specialty care in new models of health care delivery. Participants will understand the importance of validated clinical data at both the network and provider level with insights as to why a robust clinical data warehouse is essential. Additionally, the presenters will describe how quality is managed at the network, practice and provider level and how quality metrics for ACO contracting for payers and employers are used for documenting results and value. Finally, the presenters will explore the leadership and governance needed for long term success in pursuit of the IHI’s triple aim. CORPORATE UNDERWRITERS Track 6 Managing Populations and Care Under New Payment Models Navigating to Value-based Accountable Care: A Unique GPS for Destination Success Elizabeth G Valinda Rutledge, Chief Executive Officer, CaroMont Health, Gastonia, NC; Jan Mathews, RN, MPHA, CPHQ, NEA, BC, AVP, Quality Management, CaroMont Health, Gastonia, NC; and Mary Hassett, President, Integrations, Inc., Greenville, SC This session focuses on one successful organization’s itinerary for achieving valuebased, accountable care. CaroMont Health has revved up the engine by using its size, initiative, agility and innovation to out maneuver competitors on an accelerated journey toward value-based care. You’ll learn about CaroMont’s unique approach to identify the key coordinates, locate the right partners, and navigate the multiple terrains that stand between the rhetoric and actionable success. Patterning their approach after today’s GPS receivers, CaroMont is engaging multi-channel relationships, payment and delivery models to reach their desired destination. You’ll understand why it’s critical to include ‘competency-enhancing’ partners, and how to do it in mutually beneficial ways— leveraging new proprietary systems to enhance clinical care coordination, optimize resource utilization, manage population health, quantify projections, and offer systematic approaches and technologies that go beyond the competencies (and available capital) in most health systems. This session provides a driver’s seat view of the milestones and potholes including garnering one of the few bond rating upgrades in the industry. PLATINUM UNDERWRITER Monday, July 18th Track 7 Leveraging Clinical Information Systems for Improved Performance Leveraging Information Technology and Engaging Physicians for Improved Clinical Quality, Patient Safety and a Superior Patient Experience Elizabeth H Thomas C. CORPORATE UNDERWRITER Track 3 Emerging Models of Physician Engagement Strategies and Processes for Creating Successful Physician Engagement Models: A Case Study Approach Elizabeth C Track 2 Strategies for Efficient and Effective Resource Utilization Improving Quality of Care and Financial Performance in a Complex Economic Climate Elizabeth B Marty Bonick, CEO, Jewish Hospital and SVP, Operations, Jewish Hospital & St. Mary’s Healthcare Louisville, KY and Chris Drummond, Managing Director, Wellspring+Stockamp, Huron Healthcare, Chicago, IL Hospitals must manage the impact of the economic downturn while preparing for reform’s changing payment models. With the current state of the industry, it is increasingly important that providers ensure resources are utilized in the most effective and efficient manner possible. Jewish Hospital & St. Mary’s Healthcare was experiencing years of operating losses, increasing numbers of uninsured and charity care patients, a deteriorating commercial paying base, and an increase in bad debt. In response, JHSMH implemented a turnaround through targeted expense reductions, supply chain initiatives and long-term strategies to improve operational standardization and quality at the same time. This session will demonstrate how to build out a strategy to ensure optimal resource utilization in the face of decreasing patient volumes. Methods to ensure collaboration among clinical staff regarding changes to supply utilization and staffing patterns will also be addressed. The session will also outline the most effective ways to communicate such change, while ensuring that longterm performance improvement initiatives are measured and tracked consistently. CORPORATE UNDERWRITER Peggy Naas, MD, MBA, Vice President, Physician Strategies, VHA Inc., Chanhassen, MN The challenges posed by health care reform legislation have spawned a renewed focus on the need for physicians and hospitals to work together collaboratively and seamlessly to deliver a culture of care continuum defined by costeffective clinical excellence, evidencedbased medicine and patient satisfaction. More than ever before, the “we’re all in this together” attitude must prevail as the industry shifts to a pay-for-performance, bundled payment model that transverses multiple care sites and requires increased coordination of care to deliver high-quality clinical outcomes and improved patient satisfaction. The presenters will review various emerging models of physician engagement including service line comanagement, shared risk – ACOs and employment, and clinical integration. Participants will learn to evaluate their organizations’ culture and identify which payment model(s) would best fit within their current care delivery model; understand how to overcome potential barriers to implementing a new model and how to structure a model to suit their specific needs; and create a leading practice blueprint for a physician engagement plan that will engage physicians in meaningful collaborative conversations. CORPORATE UNDERWRITER Track 5 Integrating Care Delivery Integrating Patient Care Delivery Elizabeth F William F. Jessee, MD, FACMPE, President and CEO, Medical Group Management Association, Englewood, CO Integrating care delivery requires more than just creating an organizational structure that includes hospitals, physicians and other clinicians. It requires re-engineering the clinical work of that enterprise around the patient. A number of tools—such as the patient-centered medical home, chronic disease management, lean management methods, home-based patient monitoring, e-visits, and others— have proven to be useful in improving care coordination, reducing the likelihood of errors, and enhancing patient outcomes. And yet, much more attention is currently being focused on the organizational structure of ACOs than on the operation of those entities in achieving “accountable care.” This session will provide actionable information to help senior leaders move beyond a focus on structure, to one in which the function of the ACO is primary. Dr. Jessee will analyze barriers to the effective integration of clinical care delivery and share specific tools for improving the coordination and integration of clinical services. CORPORATE UNDERWRITERS Track 6 Managing Populations and Care Under New Payment Models Bundled Payment: The Next Step in Improving Quality and Reducing Cost Elizabeth G Rita E. Numerof, PhD, President, Numerof & Associates, Inc., Saint Louis, MO Promising to deliver a standardized set of services for a fixed price with specific quality guarantees will finally force care providers to align their efforts to contain underlying cost drivers. Attention to such issues as overutilization, technology choices, and inadequate coordination across the care continuum can drive meaningful change in cost and quality—but only if providers are uniformly focused on these outcomes, without the distortions caused by the current fee-for-service approach. A concurrent focus on development of a differentiated clinical value case will ensure a balanced approach that doesn’t forgo quality and safety for the sake of economic efficacy. This session will help you prepare for bundled pricing with strategies and approaches to: • Examine current economic and clinical value proposition, identifying strengths and gaps • Identify cost drivers in key services, including clinical, technical, and process factors, and create the infrastructure to address these 15 Tuesday, July 19th • Map out predictive care paths and implement these across the continuum of care to reduce variability in cost and quality • Collaborate with payers to move reimbursement away from episodic care PLATINUM UNDERWRITER Track 7 Leveraging Clinical Information Systems for Improved Performance From Accountable Care to Meaningful Use, Why Health Information Exchange Is the Linchpin across the continuum, their records need to follow. In this panel discussion, you’ll hear how health systems are putting aside competitive differences in order to exchange patient data and improve community health. In Baton Rouge, Ochsner Health System developed a secure web-based health information exchange to connect with thousands of physicians and ensure better care coordination. Meanwhile, in Michigan, several competing health systems have come together to form Michigan Health Connect. You’ll also gain a national overview of HIE activity from CHIME.
