plans are also entitled to receive certain documents, such as summary plan descriptions and notices. These documents are important in

Notification was made 08/30/2016 of the distribution of the 2016 Plan Year documents by electronic means to THC employees and

Employees identified have been notified by previous newsletter communication and Transamerica
Enrollment notification that unless deferral is elected at a different amount (or employee chooses to opt out of the
automatic enrollment feature prior to September 30th) employees identified below will be added to the plan for an

September Healthy Recipe: Blissful Banana Mousse
Nutrition Facts: Calories: 94, Fat: 1g , Saturated Fat: 1g
Ingredients:
2 T low-fat (1 percent) milk
4 tsp sugar
1 tsp vanilla
1 medium banana, cut in quarters
1 cup plain low-fat yogurt
8 banana slices (1/4-inch)
Directions: Place milk, sugar, vanilla and banana in blender. Blend for 15 seconds on high until smooth. Pour mixture into a small bowl. Fold in yogurt. Chill.
Spoon into dessert dishes. Garnish each with 2 banana slices just before serving.

This is a summary of the annual report of the Texas Health Care PLLC Cafeteria Plan (Plan
Number 510), a health plan sponsored by Texas Health Care, P.L.L.C. (Employer Identification

Texas Health Care PLLC Welfare Benefits Plan

sponsored by Texas Health Care, P.L.L.C. (employer identification number 71-0892430), for the plan year 01/01/2015
through 12/31/2015. The annual report has been filed with the Employee Benefits Security Administration, as required
under the Employee Retirement Income Security Act of 1974 (ERISA).

the terms of the plan.

The total premiums paid for the plan year ending 12/31/2015 were $5,155,470.

The plan has a contract with Aetna Life Insurance Company to pay certain dental claims incurred under the terms of the plan. The
total premiums paid for the plan year ending 12/31/2015 were $369,782.
Component Plan: Texas Health Care PLLC Group Vision Plan
The plan has a contract with UnitedHealthcare Insurance Company to pay certain vision claims incurred under the terms of the plan.
The total premiums paid for the plan year ending 12/31/2015 were $52,502.
Component Plan: Texas Health Care PLLC Basic Life Plan
The plan has a contract with UnitedHealthcare Insurance Company to pay certain life insurance claims incurred under the terms of the
plan. The total premiums paid for the plan year ending 12/31/2015 were $27,591.
Component Plan: Texas Health Care PLLC Supplemental Life Plan
The plan has a contract with The Lincoln National Life Insurance Company to pay certain life insurance claims incurred under the
terms of the plan. The total premiums paid for the plan year ending 12/31/2015 were $95,122.
Component Plan: Texas Health Care PLLC Long Term Disability Plan
The plan has a contract with The Lincoln National Life Insurance Company to pay certain long term disability claims incurred under
the terms of the plan. The total premiums paid for the plan year ending 12/31/2015 were $66,661.
Component Plan: Texas Health Care PLLC Short Term Disability Plan
The plan has a contract with The Lincoln National Life Insurance Company to pay certain short-term disability claims incurred under
the terms of the plan. The total premiums paid for the plan year ending 12/31/2015 were $82,653.
