Because we know your time is valuable and because
we appreciate having you as a customer, we at
CTB/McGraw-Hill are working to make the scoring
process as fast and efficient as possible.
As part of our effort, we have designed this guide
to help you organize and assemble the CTB test
materials that will be sent to us for scoring. We
have described in sequential order the steps in the
checking and assembly process that you should
follow and the information that should be included
to ensure that your testing program is successful
for students, teachers, parents, and schools.
If you feel the guide is not clear or if there is
additional information you need, please call us and
we will help in any way we can.

Q: Why do I need a Group Information Sheet (GIS)?
A: The GIS enables CTB to summarize and report students’ scores as a group, such as a class. For example,
if two GIS forms are submitted, one for Ms. Smith’s class of 15 students and one for Mr. Parker’s class of
20 students, a class report can be generated for both Ms. Smith’s class and Mr. Parker’s class.
Q: If I choose to separate students into special population groups, such as Special Education,
English as a Second Language, or Title I, what procedure do I follow?
A: Although federal legislation mandates the inclusion of assessment data for all students, the GIS can
be used to separate special population groups from regular populations of students. Each population
should be separated by using a GIS. On the GIS, indicate the appropriate special population in
the “Special Population Category” box located on the middle right side of the GIS. If the special
population is not listed, use “Special Population 1, 2, or 3.” An example follows.
Teacher A has a class of 25 students—15 regular
education, 5 Special Education, and 5 ESL students.
First, pack one completed GIS followed by the 15
regular education students’ answer documents. Next,
directly underneath the regular population students,
pack another completed GIS with the special
population filled in and followed by the 5 Special
Education students’ answer documents. Last, directly
underneath the Special Education students, pack
another completed GIS (with ESL filled in) followed
by the 5 ESL students’ answer documents.

Q: What company do we use to ship our materials
back to CTB?
A: CTB recommends using Federal Express or UPS for shipping. If you do not use these companies,
please use a company that can track your materials. Shipping the materials back to CTB is the
customer’s responsibility. If U.S. mail is used, please certify.
Q: Where do we put the School/Group List (SGL)?
A: Please put the SGL on top of the materials in the first box. The customer should have a GIS for every
name listed on the SGL. Please refer to Pages 18 and 19 for further assistance regarding the completion
of the SGL.

Q: What method of payment is required?
A: The CTB Scoring Process Team must have a Purchase Order before generating reports. If your school
or district does not use Purchase Orders, please provide a Letter of Authorization stating that the
school/district will pay the invoice without a Purchase Order.
Q: How do I know what scoring services were ordered by my school?
A: Please read the Scoring Services Confirmation Sheets located inside the Test Coordinator’s Kit. This
confirmation states exactly what the customer has ordered and what the customer will receive after
reports are processed.
Q: What can I do to ensure prompt processing of reports?
A: Be sure to carefully read and follow the instructions in this document. The turnaround time for testing
results will be delayed if materials are packed incorrectly.
Q: What if students used loose, handwritten sheets of paper for their responses for TerraNovaTM
Multiple Assessments?
A: CTB cannot score loose, handwritten sheets of paper.
Q: What if students have key entered and printed out their responses on loose sheets of paper for
TerraNova Multiple Assessments?
A: Students who used a computer to generate their responses to items for TerraNova Multiple Assessments
must be identified on the School/Group List. In addition, “computer generated” must be written in
the student’s test book next to each item, as appropriate, to ensure that evaluators are aware that a
computer-generated response exists.
Q: If we administer separate TerraNova Plus test books along with the TerraNova Complete Battery,
Survey, or Multiple Assessments, are there special requirements?
A: Yes. Check to be sure that the form and level of the Plus test books are the same as the form and level
of the target TerraNova test book—Complete Battery, Survey, or Multiple Assessments.
Q: What if tests are administered to students who use assessment accommodations?
A: TerraNova, The Second Edition and TerraNova, Third Edition answer documents have been modified
to record assessment accommodations. Please see the Assessment Accommodations Supplements
and the Guidelines for Inclusive Test Administration. If you need a copy of either publication, please
contact your Scoring Process Team.
Q: If a student uses a Large Print or Braille test book or answer sheet, may I send it back to CTB
for scoring?
A: In most cases, no. If a student marks answers on a nonscannable document such as a Large Print or
Braille test book or answer sheet, you must transfer all marks to a scannable document before it is
shipped to CTB for scoring. However, if a student marks answers in a Multiple Assessments Large
Print test book, fill in the student-identifying information on a regular, scannable test book and insert
the regular test book in the Large Print test book. Ship both books to CTB.

Preparation
for Testing
..............................................
 Announce the test dates in advance. Have the teachers prepare students
by explaining the purpose of the test.
 Review the Test Directions for Teachers/Examiner’s Manuals for all tests
and levels and become thoroughly familiar with their contents. Conduct a
workshop with test administrators to familiarize them with the test.
 Help teachers prepare the testing schedule. Pace the schedule to avoid
rushed sessions and fatigue.
 Have teachers administer the appropriate practice test prior to the actual
testing. Careful checking of the practice tests will identify those students
who may need extra help with the mechanics of taking tests.
 Help teachers gather materials:
• several No. 2 pencils with erasers per student
• a sufficient number of test books
• a sufficient number of answer sheets if students are using reusable test
books
• appropriate Test Directions/Examiner’s Manuals
• Group Information Sheet(s) and answer document envelope(s)
• a Do Not Disturb sign is helpful but not included with the test
materials. Have teachers prepare their own Do Not Disturb signs.
• a watch or clock with second hand (a stopwatch is recommended)
• scratch paper
 If students are able to complete the student-identifying information,
have teachers draw a model of the student-identifying grid of the answer
document on the chalkboard to guide them.
 If students are not able to complete the student-identifying information,
have teachers complete all the information, including correct birth dates,
on the answer documents prior to distributing them.
 We recommend that your aptitude tests (InViewTM, TCS®/2, or PTCS) be
administered before your achievement tests (TerraNova, CATTM/5).

Shortly before the testing date, distribute the test books, the answer sheets
(if they are to be used), the Test Directions for Teachers/Examiner’s Manuals,
the Group Information Sheets, the School/Group Lists, and the answer
document envelopes to the schools and examiners who will be administering
the test.

The School/Group List is CTB’s way of double-checking that we have received all your groups of answer documents. Every Group
Information Sheet (GIS) completed should have an entry on the appropriate School/Group List.
NOTE: Ungraded groups cannot receive National Percentile, National Stanine, Normal Curve Equivalent, or Anticipated Achievement scores.
Please list each group in order by grade. DO NOT list more than one school on a School/Group List.
SPECIAL POPULATION INSTRUCTIONS (e.g. Groups to be Scored and Reported Separately)
If you chose to use the “Special Populations” option on a group’s GIS:
1) Enter a ✓ in the Excluded Group column for that group. The GIS must be bubbled accordingly.
2) In the Excluded Group Title column for that group, write the title of the Special Population as you bubbled it on the Group Information
Sheet (GIS). There are 10 possible titles, e.g., “Spec. Ed.,” “Chapter 1,” “SDC.”

Check
Answer Documents

Immediately after the testing session, the examiner collects the test
documents from the students and checks the physical condition, including
dark, clear marking. Then the documents are checked for accurate studentidentifying information, and invalid tests are identified.
Physical Condition
There are some conditions that interfere with the electronic scoring process.
Please check documents that are to be machine-scored for the following:

If you find this...

paper clips, punch-out tools
(rulers, blocks, etc.)
•Remove them.

• creases in documents
• paper damaged by erasures
• ripped or torn sheets
• documents with spills or
other damage
• responses marked in pen
•Ensure that stray marks or light marks
are completely erased or darkened, as
necessary.
•Students must write ONLY in the
designated areas and ONLY on the
lines and spaces provided. Pencil
marks in the margins will delay
processing of the test books.
•Transfer responses onto a clean,
unused document.
•Use a No. 2 pencil only.
•Use a soft eraser.
•Ensure that filled-in circles are solid
and dark.
•If you have questions regarding the
condition of a document, call your
CTB Scoring Process Team.
Note: Please carefully count answer documents to ensure that a document
has been collected from each student who was tested.

Student-Identifying Information for TerraNova,
The Second Edition
Check the student-identifying information on all answer documents. This
information should already be present, having been filled in by hand prior to
the test administration or by the CTB Precoding Service. Review the data for
accuracy, and check all handwritten entries for legibility. All marks should
be solid and dark. Incorrect or incomplete information will result in
inaccurate student data and can also cause a delay in the processing of
your test documents.
Below is a sample student-identifying information grid from a TerraNova,
The Second Edition answer document. Use it and the explanations on
the following page to check the TerraNova, The Second Edition answer
documents.

Check the student-identifying information on all answer documents. This
information should already be present, having been filled in by hand prior to
the test administration or by the CTB Precoding Service. Review the data for
accuracy, and check all handwritten entries for legibility. All marks should
be solid and dark. Incorrect or incomplete information will result in
inaccurate student data and can also cause a delay in the processing of
your test documents.

Invalid Tests
A test section is considered invalid if a student has lost time during
testing, marked answers randomly, or had other problems as described in
the Test Directions for Teachers/Examiner’s Manual. If an examiner has
determined that a test section is invalid, follow these steps to ensure that
the test section is not scored:

Organize
Answer Documents

It is essential that all test books or answer sheets for a single grouping
of students (“population”) be sent together with a completed GIS.
Therefore, the answer documents must be sorted into designated groups.
Before grouping the answer documents, consider how many population
categories you have—for example, regular education, Special Education, .
ESL, Resource, etc. This configuration will determine how you organize .
the documents to be returned to CTB for scoring. Refer to the following
scenarios.
Single-Group Populations
Typically, a “single group” is a classroom or a group of students in a special
population category, such as ESL, who are all in the same grade. A single
group can be as large as an entire grade or as small as a single student on
home study. Put all the answer documents for each group in their own stack,
and complete one GIS for each group.
Combination Classrooms
When a class includes students in more than one grade, sort the answer
documents into separate stacks, one for each grade. Complete a GIS for
each stack or grade.
Different Test Sections at Different Levels
If students need to take different test sections (such as reading and
mathematics) at different levels, they will mark their responses on separate
answer documents. Therefore, the documents may need to be stacked
separately, depending on the configuration of levels and test sections and
the types of reports ordered. If you have a special testing situation, call your
CTB Scoring Process Team for assistance in grouping your test documents.
Ungraded Groups
Group and designate a population as “ungraded” only when you do
not need national scores such as National Percentile, Normal Curve
Equivalent, or National Stanine, or when a grade level cannot be
determined.

When you check this... look for this:
1 TEACHER NAME
The teacher’s last name must be printed in the boxes, and then the
first name or initial, if needed. Under each box, the circle with the
corresponding letter should be filled in.
2 NUMBER STUDENTS
TESTING
The number of students whose answer documents are related to this
Group Information Sheet should be printed in the boxes and the
corresponding circles filled in.
3 GRADE
The circle that shows the grade of the students being tested should be
filled in. If this group of students is to be treated as “ungraded,” that
circle should be filled in.
NOTE: Group and designate a population as “ungraded” only when
you do not need national scores such as National Percentile, Normal
Curve Equivalent, or National Stanine, or when a grade level cannot
be determined.
4 SPECIAL CODES
Numbers in columns A through V should not be marked unless an
arrangement was previously made with your CTB Scoring Process
Team.
If special codes are to be marked, only one circle per column should
be filled in. Marking more than one circle in each column will
invalidate the coding.

In order of grade, list each group by the name
shown on the GIS—typically the classroom

STUDENTS TESTED
For each group, write the number of students
tested, which should be the same number that
has been filled in on the corresponding GIS.

TITLE
Only one special population group can

Prepare Answer Documents for Shipping

Below are the steps that must be taken to prepare answer documents for
shipment to the CTB Scoring Center. To ensure that your test reports give
you the full range of information they are designed to provide, proper
completion of these steps is essential. Failure to follow these directions
may cause a delay in processing your answer documents.

Package and Ship Test

When all of the test materials have been returned to and checked by the
School Test Coordinator, store nonscorable materials that will not be
returned to CTB in a secure location. The scorable materials should be
packaged according to the following guidelines to ensure safe shipment.
Follow these guidelines . . .

Obtain shipping cartons.
Use the corrugated cardboard cartons that your test materials
from CTB arrived in or use boxes of comparable construction to
minimize the possibility of loss or damage to your documents
while in transit.
