Clarifications in the Stark Amendment regulations, published in the Jan. 4, 2001 Federal
Register (effective Jan. 4, 2002), spells out in great clarity how physicians can stock, as well
as bill, for orthotic braces and supports as long as they meet certain guidelines. In addition
to providing even wider treatment options, physicians can consider alternatives to sending
patients to other clinical specialists or using stock and bill programs. The exception that
allows physicians to bill for neck, arm, back and leg braces is the “In-office ancillary services
exception.” This exception specifies that for patient convenience, the designated health
service of orthotics may be provided in a physician office if the physicians act as their own
DMEPOS supplier (by applying for and receiving a DMEPOS supplier number). Physician
offices use L codes or E codes (subcategory of HCPCS device codes for orthotics) when billing.

A protocol program offers full support for treatment and reimbursement
DeRoyal’s Protocol Program is a value-added service intended to help you select the
most appropriate orthotic devices for the treatment of musculoskeletal pathologies.
This program supports your need for documentation by offering standardized
treatment for the most common indications seen in a physician’s office. This program
links to the most commonly used ICD-9 codes (and the appropriate diagnosis
terminology) to the most appropriate orthotic devices with their codes and descriptors.

To replenish inventory, use DOC (DeRoyal Order Connection)
Space and resources are always an issue when managing inventory.
DeRoyal has developed a perpetual online system that links your
office(s) directly to our customer service. Utilizing bar-code technology,
this system makes it easy to scan each item used and automatically
create replenishment orders. This not only saves time not counting and
placing multiple reorders, but by managing your par levels, prevents
expensive over-stocking of supplies, avoids running short on
product and effectively manages your storage space.
DOC (DeRoyal Order Connection and Inventory System) allows
you to view all your inventory details. You can filter by Supplier
and/or Inventory Location. You can follow every transaction for
an item through the system from usage, order and receipt of
replenishment stock. The system allows you to view transaction
history by product, by location, by patient, by physician, by type,
etc. (whatever you determine is the best way to document and
track your usage). It is a live system that you can update and
edit at any time – par levels, pricing changes, addition/deletion
products or suppliers and any account maintenance information that
needs to be documented. It also provides many reporting capabilities
that can be managed from an Acrobat PDF or Microsoft Excel format
that allows you to save, email or print reports.
Everything from orders and shipments, usage by date range or
location or physician, inventory valuation and expected reimbursement
amounts can be extracted from our Order Connection and Inventory
System to help you manage your business.
The Billing Index answers coding and reimbursement
questions
The web-based Billing Index helps answer all your coding and
reimbursement questions. It is an easy reference to the latest L code
assignments and corresponding Medicare reimbursement in your
state. A login and password protected site available only to our
physician billing customers, the Billing Index can be a useful tool
for your clinical and billing staff. It will help ensure the information
in the patient chart is accurate so that your reimbursement is
appropriate for the treatment provided. This site is the place to find
the latest breaking information and changes to product specific L
code recommendations and the most current fee schedule amounts
from the DMEPOS fee schedule.
