• If you choose one of the recommended product assortments (order form 1-3), determine the placement
of the samples on the grid, fill in your shipping information and fax to Customer Service.
• If you’re choosing the customized assortment (order form 4), please follow the instructions on the Order Form.
• Each individual product sample has a maximum point value:
• In-Line and “X” Display = 200 points
• “Y” Display = 136 points
• Customized assortment must add up to either 200 or 136 points (if you exceed
maximum point value, you will be charged $10.00 per point)
