Attention Parents of Students Entering 12th Grade
NEW IMMUNIZATION REQUIREMENT - 2015/16
Meningococcal Vaccine (MCV4)
A new immunization requirement goes into effect in the fall of 2015. Students entering grade 12
in the State of Illinois must be immunized for meningitis. The immunization requirement consists
of two doses of the MCV4 vaccine for entry into 12th grade. Only one dose of the MCV4 is
required for students entering 12th grade if the first dose was received on/after the age of 16.
Proof of receiving the MCV4 vaccination(s) is required prior to the first day of school beginning
in the fall of 2015. According to District 303 policy students without proof of receiving the
MCV4 vaccination will be excluded from school. The only exceptions are students with an
approved medical or religious exemption on file with the school nurse or students with an
appointment to receive the MCV4 documented by the student’s healthcare provider and submitted
to the school nurse.
Verification of the immunization must be submitted to the school nurse prior to
the first day of school.

must specify the time frame of the temporary exemption.

All types of cars, bikes, and trucks will be displayed.
We would like a wide variety from
classic to custom cars and bikes.
All donations will be accepted and all proceeds go to St.
Charles North Auto Club to help restore a 1965 Mustang.
