Less Invasive Procedures Give Vascular
Patients New Options

Less invasive procedures have become more common for the
treatment of peripheral arterial disease (PAD), abdominal
aortic aneurysm (AAA) and varicose veins. Minimally invasive
procedures tend to have lower risk and easier recoveries
than larger open operations. The impact has given patients
new options:

3. Varicose vein patients are now almost exclusively treated in the office setting.
Large lumpy veins on the surface of the legs are often associated
with pain, aching, heaviness of the legs and swelling. Varicose
veins and venous insufficiency affects more individuals than most
arterial problems combined. The treatment of varicose veins has
undergone a drastic revolution to minimally invasive therapies.
Peripheral Arterial Disease or PAD
PAD occurs when the arteries to the legs develop restrictions
to blood flow due to atherosclerotic plaque. The most common
risk factors of Atherosclerosis are related to cigarette smoking,
high blood pressure, and high cholesterol. Restriction of blood
flow to the legs can cause muscle pain that occurs with walking.
When the restriction is severe, foot and toe pain can occur at
rest or wounds can develop on the feet that do not heal.

Since the aorta is deep in our abdomens, it is not easy to feel so
many patients do not know that they have an AAA. People at
risk should ask their doctors to order a simple ultrasound.Some
patients with AAA may need to have a major operation but many
aortic aneurysms can now be repaired with a much less invasive
procedure called endovascular aneurysm repair.

The most important point about PAD is that if
atherosclerosis is present in the legs it may also
be present in the arteries to the brain or heart,
thus indicating a risk for stroke or heart attack.

Men and women over the age of 60 who have
smoked cigarettes, have a history of heart disease
or a family member who has had an aortic
aneurysm are at higher risk.

Once associated with vein stripping operations, most
procedures are now performed in the office with local
anesthesia. Endovenous closure with small catheters, minimal
phlebectomy and sclerotherapy have replaced larger, more
painful stripping procedures.

Most patients with PAD need to only modify their lifestyles
and begin an exercise program. For patients who need invasive
therapy, newer technologies have allowed more patients to be
treated with minimally invasive or endovascular procedures such
as balloon angioplasty, stent placement or atherectomy. Some
patients may require more extensive surgical procedures like leg
artery bypass.
Abdominal Aortic Aneurysm or AAA
The aorta artery is our main artery. It starts at the heart and ends
in the abdomen, giving rise to all the arteries in our bodies. In
the lower abdomen, it splits in two supplying the main arteries to
the legs. Some people develop a weakness in the elastic layer of
the artery causing it to slowly expand over time. Like a balloon, if it
gets too large it can rupture, causing internal bleeding and death.

In the past five years, more than 300 men have undergone
new cancer treatment with none of the damaging side
effects of previous treatments. Innovative radiation therapy
treatments at MIMA Cancer Center have achieved a 98
percent success rate, with no return of cancer.
Dennis French is one such patient. When Dennis was
diagnosed with an aggressive form of prostate cancer, he
was able to find hope close to home. MIMA Cancer Center
is an integrated cancer center that offers board-certified
surgeons, medical oncologists, radiation oncologists and
some of Florida’s most advanced medical technology.
“Prostate cancer surprised me. During a standard physical
for my 50th birthday, I was diagnosed with an aggressive
form – a Gleason 8,” said French. “Usually, that diagnosis
comes without much hope. I heard about how toxic
treatments could be and I was nervous about what I
was facing.”
French did what most people would do; he got a second
opinion. “I did my research and visited cancer centers.
It became clear that MIMA Cancer Center was the right
place for me: the technology was more advanced, I felt
confident in the expertise of the physicians, and I wouldn’t
have to be out of town and away from my support system,”
says French.
Dennis was impressed with the 3D radiation capabilities of
the Varian Trilogy™ - the technology used at MIMA Cancer
Center that enables image-guided radiation therapy (IGRT).
Physicians have the ability to precision-target the exact area
of a tumor and not only provide treatment, but treatment
with no damage to surrounding tissues. It is so precise that
it can even accommodate for a patient’s breathing during
treatment so that it always targets just the disease.
“IGRT has given us incredible versatility in cancer treatment,
particularly prostate cancer,” says Dr. Todd Scarbrough,
medical director of the MIMA Cancer Center. “It’ s exciting
to give patients an option that virtually eliminates any side
effects – particularly those that men with prostate cancer
fear the most such as ED, incontinence and rectal damage.
We’re curing men everyday and they are remaining cancer
free, going to work, and playing golf all during treatment!”

Dennis French agrees. “It’s amazing what modern radiation
can do. I continued to participate in all the activities of my
life,” says French. “It took only 30 minutes a treatment for 9
weeks and I was done! ” he said. As one of the first patients
to be treated at MIMA Cancer Center, Dennis is today active,
working, and happily married, with no side effects from
treatment and most importantly, cancer free.

The “Softer Side” of Cancer –
Case Managers Provide Personalized Care
Today more and more people are surviving a cancer diagnosis. With this in mind,
care of cancer patients has moved beyond just the medical to encompass not only
the whole patient but also the family.
As a leader in cancer care in Brevard County, MIMA Cancer Center recognized
this trend and developed Oncology Case Managers who serve multiple roles that
include providing emotional support to patients and families, facilitating support
groups, assisting the under-insured, and helping patients navigate the complexities of cancer care.
As oncology case managers, MIMA Cancer Center has developed a program called
the “Softer Side of Cancer” that highlights the psycho-social care of the patient
that can often be missed in other centers. “We believe that cancer is difficult
enough to cope with on its own so we try to ease as many other stressors as
possible so the patient and family can focus on healing,” said Jane Lyons, MIMA
Cancer Center case manager. “Paul and I assist in the coordination of efficient
scheduling for treatment plans and work with patients and their loved ones in
assessing their needs in order to make necessary referrals.” This includes helping patients obtain assistance from community, pharmaceutical, and professional
agencies, as well as helping patients and their families cope with cancer through
a variety of support groups.
“It’s almost hard to imagine that something like cancer could have a ‘soft’ side,
but I know from experience that it exists,” said Walt M., whose wife Pat underwent
treatment at MIMA’s Cancer Center. “The care and advice of nurses who have a
genuine calling to what they do; the reassuring touch and concern; the confidence
of great doctors; all combine to soften something that is otherwise so harsh. I
think this combination is why Pat is alive today. Without this type of care, I can see
why some patients would do poorly.“

This summer, MIMA Cancer Center will unveil Novalis Tx ®

Healthcare in Brevard County
In 1968, subspecialty medicine was well
recognized in the US and the founding
MIMA physicians were practicing
subspecialty consultants who wanted
to provide top-quality care at a local
level. In the 40 years since that first
three-physician group was created,
the same notion has carried MIMA
into a modern era of quality care
unsurpassed in the county.
MIMA has grown into a multi-specialty group of 123 boardcertified physicians, over a dozen nurse practitioners/PA’s,
and over 800 nurses, technicians and other employees. We
know that our biggest strength is in our employees, and
we recognize their value. They reward us with loyalty and
commitment in the care they take for the people we serve.
With the largest growth in county occurring in south
Brevard, MIMA has limited geographic coverage to this
area - a strategy we feel best serves our patients.
We recruit physicians among the best in the nation and
give them the tools to do their jobs well. We have the
largest collection of primary care physicians in the county,
nationally recognized for quality initiatives and use of our
electronic medical record. With national and international
reputations in their fields, many of our consulting physicians
are called to lecture worldwide. MIMA also plays host to
visiting physicians and other professionals from across the
nation and from around the world who come here to learn
in our high-tech environment.
MIMA has recognized the necessity of investing in
technology. Our ancillary services departments contain
some of the most advanced diagnostic and therapeutic
tools available anywhere in the world, and we employ
individuals with the skills necessary to operate them.
Now preparing to meet the future needs of the residents of
Brevard, we have three major building projects in progress,
are renovating many of our current facilities (to make sure
we change as medicine does) and have pioneered the
clinic style practice. This puts primary care physicians in
close proximity to sophisticated diagnostic equipment - a
convenience well received by our patients. All of our new
facilities will follow this model.
As we grow, we take on new responsibilities and roles. This
includes the creation of The MIMA Foundation, a not-forprofit subsidiary with growing activities in the community.
We have distributed over $100,000 in scholarships to local
area residents, and expanded activities to allow us to directly
help less fortunate patients. The annual MIMA Classic Tennis
tournament, a partnership with the Kiwi Tennis Club, is a
major stop on the USTA women’s professional tour. All of
the money distributed by the MIMA Foundation goes to
the citizens of South Brevard County, and none of it is
used to pay us for medical care or facilities. Additionally,
our physicians donate more free medical care to the
citizens of South Brevard than all other physicians in the
area combined.
Another developing role for MIMA is community outreach.
In recent years we have added social workers and taken
on many of the community programs traditionally run
by hospitals, including the American Cancer Society
Community Closet, “For Ladies Only” support groups,
“Cancer 101” education and resource group, and sponsorship
of “Look Good, Feel Better.”
This is MIMA in the modern era. We provide full spectrum
medical care with top-level physicians and staff, and are
committed to our local community.
MIMA opens new medical
facility in malabar
MIMA’s newest medical facility in Malabar next to
Palm Bay Community Hospital will give new access for
residents in south Brevard to physicians, laboratory
services, and diagnostic services. “We are committed
to more efficient, comprehensive and convenient access
of services to more of the residents in Brevard,” says
Joseph McClure, M.D., chief executive officer and MIMA’s
chairman of the board. Its proximity to the greater Palm
Bay and Malabar area, along with being next door to Palm
Bay Community Hospital, gives physicians and patients
alike, added convenience and access to cutting-edge healthcare.
The 36,000-square-foot facility will feature a two-story centralized
reception room, retail kiosk, 42 exam rooms, up to 12 physicians, four
procedure rooms, and state-of-the-art diagnostic services. Patients can
now access medical services in south Brevard including laboratory tests,
Cardiology testing, Nuclear Cardiology, CT (including Cardiac CT), Open
MRI, general x-ray, and Ultrasound.

MIMA has once again met all criteria for laboratory accreditation by COLA, a national healthcare accreditation
organization. Accreditation is given only to laboratories that apply rigid standards of quality in day-to-day
operations, demonstrate continued accuracy in the performance of proficiency testing, and pass a rigorous
on-site laboratory survey.
Mary Lawrence, Director of the MIMA Laboratory says accreditation is independent validation of MIMA’s
overall commitment to quality healthcare. The MIMA lab combines state-of-the-art instrumentation and
computerization with licensed laboratory personnel to achieve a level of quality and patient satisfaction

Advanced Pulmonary Function
Test Services at MIMA
Over the past 30 years, pulmonary function testing has
come into widespread use. Pulmonary function tests (PFT)
are breathing tests that find out how well you move air in
and out of your lungs, your lung capacity, and how well
oxygen enters your body.
Michael Ott, M.D., board-certified specialist in pulmonary
diseases, critical care, and undersea and hyperbaric medicine, along
with Drs. Prakash Reddy, Theodore Amgott, and Jerry Hung have
created one of Central Florida’s most comprehensive PFT Laboratories.
Complete with two PFT testing stations, two smaller fixed stations,
and a portable station, the lab performs spirometry (blowing hard into
a tube connected to a machine), diffusion studies (a series of breaths
into a tube and holding your breath) and plethysmography (measuring
how much air remains in your lungs when breathing in and out). For
patients unable to enter the testing chamber for plethysmography, an
alternative method is also available involving estimates of lung capacity
from breathing inert gas.
In order to provide more expanded care, and to meet the needs of scuba
divers (a keen interest of his), Dr. Ott has built an advanced diagnostic
PFT test lab that can only be found at MIMA.
One such test offered is the HAST – High Altitude Simulation Test.
Designed to simulate the conditions in higher altitudes during airplane
travel, patients are tested to measure their tolerance to increased altitudes
to find out if they need additional oxygen. Patients with heart or lung
disease can now more easily check their oxygen levels and if needed,
utilize portable, battery-operated oxygen concentrators that have made
travel easier and less expensive in recent years.
Dr. Ott has also put together a shunt study, which checks blood oxygen
levels under different conditions to assess whether oxygen may be
by-passing the normal pathway. Using such tests as the methacholine
challenge, exercise challenge, saline challenge, and cold air challenge, the
lab can simulate many different conditions that might lead to asthma and
reduced lung function.
This advanced level PFT laboratory is a valuable service for the
community by giving area patients and physicians a unique range of
services not available anywhere else in the area. For more information,

A Mission to Honduras
Along with half a dozen other
central Florida physicians, Drs.
Peter Marzano and Jim Ronaldson
traveled to San Pedro Sula
Honduras for a medical mission trip.
Sponsored through Rotary Club
of Orlando, physicians get an opportunity to serve
residents in Honduras with much needed medical
care by highly qualified doctors.
The second trip for both Dr. Marzano and Dr.
Ronaldson, they cared for patients of all ages,
most often dealing with conditions such as scabies,
intestinal parasites, and upper respiratory infections.
“Seeing the tremendous need for health care in this
little village, as well as how much help we could provide
in such a short period of time, reinforced why I became
a physician in the first place,” said Dr. Marzano.

days, four physicians cared
for more than 574 patients.

electricity or running water.
“These people have absolutely
no access to healthcare. They
live in such depressed conditions, and bring their
whole families…walking a day to get there,” said Dr.
Marzano. “It is very rewarding to help people who are
unable to help themselves.” Dr. Marzano adds that he
and his wife Linda, a nurse, plan to return every year.
“We are proud of the many MIMA physicians, like
Dr. Marzano and Dr. Ronaldson, that serve our local
community and travel abroad to participate in
programs such as this,” said Al O’Connell, MIMA’s chief
administrative officer.
