RIGHT TO INSPECT - The Owner and or his Agent has had the opportunity to inspect the facility and agrees that
all aspects of the facility are acceptable and in acceptable condition. All objections as to the conditions of the
facility must be provided in writing before Horse arrival to ETF /Marilou N. de Azúa
OWNER’S EXPENSES - Any additional expenses incurred by ETF on behalf of the Owner and in the event veterinary
services and/or farrier services are provided for Horse or miscellaneous expenses were incurred for Horse, will
be the sole responsibility of Owner and shall be paid prior to the release of the Horse from ETF quarantine.
SECURITY INTEREST - In accordance with Chapter 713 of the Florida Statutes, Owner hereby grants to ETF and/
or its operators a security in Horse for any unpaid Owner’s expenses.
INSURANCE - Owner shall maintain and pay for life and casualty insurance and extended coverage insurance
on Horse in such amounts to fully compensate Owner in the event there is any loss, damage or death to
Horse. Owner hereby waives any and all rights of recovery, claims, actions, or causes of action against ETF,
Horse by reason
Owner, regardless of cause or
origin, including negligence, gross negligence or willful misconduct of ETF
employees.
ASSUMPTION OF RISK - Owner expressly assumes all liability for or on account of any such injury, loss or damage
to Horse, during the time Horse, is in the care and custody of ETF; and will at all times indemnify and hold
ETF
of any nature, including attorneys’ fees, including those arising from ETF
employees’ negligence, gross negligence and/or willful misconduct incurred in or caused by or arising out of
any such injury, loss or damage to Horse.

PERSONAL GUARANTY - In the event the registered Owner is an entity, the agent, by signing below, personally
and unconditionally guarantees the faithful, prompt and complete compliance by Owner with all terms and
conditions of this Agreement and agrees to pay all costs, expenses, charges and other expenditures required
to be made by Owner under the terms and provisions of this Agreement.
COLLECTION - In the event any Owner’s expenses are not paid pursuant to the terms of this Agreement within
ten (10) days after the same becomes due and payable, Owner shall be deemed in default of this Agreement
and all outstanding sums owed by Owner to ETF, shall thereafter bear interest at the maximum rate permitted
by law. Owner shall be responsible to ETF for any reasonable attorney’s fees and costs incurred by ETF in
attempting to collect payment or enforcing the terms of this Agreement, through and including any appellate
costs and fees.
GOVERNING LAW AND VENUE - This agreement shall be constructed in accordance with and shall be governed by
the laws of the state of Florida. Any legal action must be brought in the appropriate jurisdictional court of Palm
Beach County, Florida.

• Complimentary use of our 90 x 220 lighted, covered arena from 7:00 AM to 9:00 PM. Use of the arena between
7:00 PM to 9:00 PM requires advance notice.
• Complimentary use of our horse treadmill. While on the treadmill, the Owner or someone authorized by the
Owner must supervise the horse at all times. Eagle Tree can provide supervision for an added charge. of $25
per session.
• Complimentary administration of medication and/or nutritional supplements provided by owner except those
mentioned in the Additional Veterinary Services section.

EagleTree C.E.M. Quarantine (Eagle Tree Farm, Inc.) Is a limited access facility. Visitation is permitted at the
discretion of the Department of Agriculture and Consumer Services of the State of Florida, the United States
Department of Agriculture and the management of Eagle Tree Farm, Inc. Below are the guidelines we follow to
allow visitors in our installations.
• ALL VISITORS MUST FIRST REGISTER at office and sign a release form. To avoid any delays we
recommend that you call or text Marilou de Azúa in advance.
• REGULAR HOURS OF OPERATION ARE FROM 7:00 AM TO 7:00 PM, TUESDAY THROUGH
SUNDAY. Mondays and after hours use of the facility can be arranged upon request.
• USE OF FOOT AND HAND WASH IS MANDATORY UPON ENTERING OR LEAVING QUARANTINE
AREAS. This applies to all visitors including veterinarians, trainer, farriers and non-staff grooms.
• NO PETS are allowed in any of the quarantine areas.
• MEDICATION, SUPPLEMENTS OR TOPICAL TREATMENTS shall not be administered to any
quarantined horses without prior consent.
• ADMINISTRATION OR USE ANTIBIOTICS AND ANTIBACTERIAL PRODUCTS IS NOT ALLOWED
while a horse is undergoing C.E.M. testing.
• SMOKING IS STRICTLY FORBIDDEN in all the quarantine areas
• QUARANTINED HORSES MAY BE EXERCISED BY HAND WALKING, LUNGING OR RIDING. Turn
out is available.
• VETERINARIANS AND FARRIERS CAN WORK ON QUARANTINED HORSES SUBJECT TO PRIOR
NOTIFICATION AND APPROVAL. The signing of a release of liability form is also required.
• TIPPING OF GROOMS IS WELCOME. Nonetheless, all service requests and special instructions
must be made through our General Manager or Assistant Manager.
