First Annual Delegation Trip Makes a World of Difference
A major earthquake struck near the Haitian capital city of Portau-Prince, home to nearly three million people, in early 2010.
The quake devastated the region, and Haiti — already one of
the poorest countries in the world — is still coping with the
devastating aftermath. Its people continue to struggle with a
lack of food, education, and basic health care.
The first annual Family Medicine Cares International Delegation
spent a week in early February providing much-needed health
care services to the people of Haiti, alongside in-country partner
Heart to Heart International (HHI). The 21-member Delegation
split into three distinct teams focused on patient care, medical
education, and service to children and youth.

• The AAFP Foundation’s humanitarian
program, Family Medicine Cares USA,
continued its support of new free health
clinics in communities across the United
States through grants and volunteer
efforts. Since April 2011, more than
$70,000 has been provided to help open
doors for four new clinics.
• The Foundation proudly introduced
Family Medicine Cares International,
the new international component of the
Family Medicine Cares program. This
humanitarian initiative provides patient
care, delivers education and training in
Family Medicine, and works to improve
the health and quality of life of people in
Haiti. See page 4 to learn more.
• Through the Family Medicine
Philanthropic Consortium (FMPC)
Grant Awards program $104,250 was
provided to support the work of AAFP
Chapters and Chapter Foundations
at the grassroots level. This is only
accomplished through your support of
the Dues Check-Off campaign each year.
• During the past 18 months, the
Foundation funded more than $306,000
in support of the Joint Grant Awards
Program (JGAP) and Research
Stimulation and PBRN Stimulation Grants.
• Diabetes management and prevention
received significant attention this year,
as more than $275,000 was provided
in grants to AAFP Constituent Chapters
for Highlight on Diabetes. The program
delivered physician and patient resources
for the management of diabetes. In
addition, Cities for Life, which was created
to facilitate healthy lifestyles and diabetes
management, launched in 2012 with a
kick-off in Birmingham, AL.

Dear Friends and Colleagues,
These volunteers, both medical and non-medical, shared the good
work of Family Medicine through medical education symposia
for Haitian residents, physicians, and medical students, clinic
improvement projects, and also helped to improve the health
of people throughout the country by attending to patients in
community clinics. The trip was a huge success and laid the
groundwork for the future of Family Medicine Cares International.
Each volunteer was able to see firsthand the value of what the
program can bring to the people of Haiti.
Another successful year has passed for the
AAFP Foundation – a year that was only
possible because of support from you and
your colleagues. Your generosity was evident
through the many humanitarian, scientific, and
educational initiatives the AAFP Foundation
has been able to support. Together, we were
able to showcase the true heart of Family Medicine.
The Foundation continues to support the future of Family
Medicine through the National Conference of Family Medicine
Residents and Medical Students Scholarship Program. We’re
proud to announce 200 students and residents will benefit
from the Foundation’s support in 2013. National Conference
scholarships are only a small part of what the Foundation is able
to do with your support.
Throughout 2012 the Foundation continued to develop Family
Medicine Cares USA, and also launched Family Medicine Cares
International — two initiatives that help support the health care
needs of underserved populations at home and abroad.
Four health clinics across the United States — launched with
help from Family Medicine Cares USA — have provided nearly
5,000 patients a total of 10,679 office visits worth $2.9 million
in services received since the program began in 2011. Without
Family Medicine Cares USA, those patients would not have
had access to the health care they so desperately needed.
It has been an honor to work with you as the AAFP Foundation
continues to impact the lives of many through our charitable
work. As always, thank you for your support of our Foundation
and our specialty.

Family Medicine Interest Group Funding Initiative
Reaching Medical Students Across the Country
More than 147 Family Medicine Interest Groups (FMIGs) were eligible to receive
funding in 2012 for the activities that reach out to and engage students at their
medical schools thanks to financial support from AAFP Foundation donors.
The FMIG Funding Initiative was able to reach more target schools (schools
without a department of Family Medicine) this year than ever before. Ten of the 11
existing target schools applied for funding — evidence that active FMIGs exist on
those campuses even in the absence of a medical school structure that supports
Family Medicine.
Yale University in New Haven, CT is one of those target schools, and was able
to reach 67 students through its qualifying FMIG event — a Family Medicine
Procedure Workshop.
“We talked about the specialty of Family Medicine and gave students an
opportunity to become members of the AAFP,” said Nyasha George, a fourth-year
medical student and an FMIG leader at Yale. “Then students had the opportunity
to learn several Family Medicine procedures.”
Clinical skills or procedure workshops are popular on medical school campuses,
particularly among first- and second-year students who are ready for hands-on
experience. Through support from the FMIG Funding Initiative, FMIGs are able
to host these popular events, reaching many students on campus and teaching
them the scope of Family Medicine.
The AAFP is able to provide support for Yale’s FMIG, and so many others,
through the AAFP Foundation and its donors. Your continued donations are both
appreciated and critical to future family physicians’ educational development.

FMPC Grant Application
Each year, the Family Medicine
Philanthropic Consortium (FMPC)
Grant Awards provide funding to
support programs of the AAFP
Constituent Chapters and Chapter
Foundations that help improve
the health of all people. In 2012,
the FMPC Grant Awards provided
$104,250 in funding for 29 projects
from 18 states and one multi-state
collaborative.
Apply by June 15 for the opportunity
to earn one of these competitive
grants in 2013, available only
to AAFP Constituent Chapters
and Chapter Foundations. Each
state may submit up to three
applications per year; applications
can be submitted by the state AAFP
Chapter, Chapter Foundation, or both.

and conferences for residents
and medical students, initiatives to
promote public nutrition and fitness,
leadership training meetings for

National Conference Scholarships: Your Support Matters
Thanks to your donations, the Foundation increased its funding for the second consecutive
year for the National Conference of Family Medicine Residents and Medical Students
Scholarship Program. The Foundation provided 125 scholarships in 2011, 169 in 2012, and
will provide 200 scholarships for the 2013 National Conference.
Your support truly does make a difference, as evidenced by this second-year medical
student who attended National Conference in 2012:
“Attending National Conference was a powerful experience for me. Meeting so many
other students and physicians who share my values and philosophy of medicine
affected me deeply. Thank you again for making my experience possible; the AAFP
really has shaped my life and my career in a tangible way by enabling me to attend the
conference.”
With your help, the Foundation can continue to make a difference for medical students,
Family Medicine residents, and for the future of Family Medicine through the National
Conference Scholarship Program. In addition, donors who contribute a full scholarship of
$600 will be matched with a scholarship recipient.

PATIENT CARE TEAM
As part of the Family Medicine Cares International Patient Care
program component, licensed physicians and residents (under
physician supervision) attended to patients at community clinics
during the Delegation trip. The Patient Care team, comprised of
seven family physicians, treated more than 600 patients (ranging
from two weeks to 92 years old) at five clinic locations throughout
the region. They also provided more than 100 well-child visits at
local orphanages — something incredibly rare for children in Haiti.
and with youth from the gang-controlled Bel Air community. They
were able to paint the exterior of the Rose Mina Orphanage and
also connect with more than 100 children at the facility. The team
also made improvements to the Bel Air Clinic, which included a
new triage area, improved bathroom facilities, and modifications
to ensure secure pharmacy operations.
“The kids are just so loving and so
caring, and they welcome us with
open arms,” said Karen Smith,
Service team volunteer. “It doesn’t
matter that we don’t speak the same
language, because we do speak the
same language — as human beings.”
“I’m so proud to be part of the
first Delegation trip,” said Julie
Anderson, MD, a family physician
volunteer from St. Cloud, MN. “I
realized we have so much in the
United States. Our patients have so
much…I guess if there’s anything I
can share with my colleagues, my
patients, and my family (from this
experience), it’s that we can’t take
what we have at home for granted.”
The team was also able to meet with the vo-tech students who
have received scholarships through the AAFP Foundation Family
Medicine Cares International program.
ONGOING VOLUNTEER OPPORTUNITIES
As part of Family Medicine Care International, two to three
volunteer physicians are needed each week throughout the
year to provide ongoing patient care in community clinics in
Haiti. These volunteer physicians generally serve one-week
deployments, working alongside Haitian medical personnel
to deliver the quality care the local population so desperately
needs. All volunteers are escorted by HHI in-country staff and
housed at HHI volunteer centers.
MEDICAL EDUCATION TEAM
The Medical Education team, made up of six family physicians
and one staff member, developed and presented two one-day
symposia for Family Medicine residents, physicians, and medical
students in Cap Haitien and Port-au-Prince. The symposia
included an overview of Family Medicine, cardiovascular risk,
and diabetes, which were specific medical topics identified by
our in-country residency program and medical school partners.

A critical part of the team’s schedule included meeting with the
medical schools, area physician leaders, and the Ministry of
Health in Port-au-Prince to discuss future symposia, curriculum,
and resource needs, plus a long-term role in supporting the
development of Family Medicine in Haiti.

Family Medicine Cares USA Makes an Impact
The road to
accessing
health care has
been filled with
challenges for
Andria Guidotti.
This 36-yearold mother
of two active
teenage girls
first became ill in
2000. Diagnosed
with diabetes, her condition became
much more difficult to manage when
her husband lost both his job, and their
health insurance, in 2009.
In the few years since losing her health
insurance, diabetes had increasingly
taken a toll on her health. Without
insurance, Andria couldn’t afford to
take a blood test to keep track of her
condition. And during the winter of
2011, access to programs and venues
to refill her medications finally ran out.
She applied to the State’s medical plan
lottery, but didn’t hear back quickly.
One day in early 2012, Andria’s friend
saw a Founders Clinic brochure at
the local Women’s Advocate Center.
Knowing of her significant health care
needs, her friend gave the brochure
to Andria, who quickly contacted the
clinic. Because of a grant from Family
Medicine Cares USA, the Founders
Clinic was able to open its doors and
serve the health needs of those like
Andria in Clackamas County in Oregon.
After Andria contacted the clinic, an
appointment with Dr. Michael Norris,
AAFP member and Clinic Director, was
scheduled for the next day.
As part of Andria’s examination,
Dr. Norris ordered lab tests. When
he received the results he called
Andria immediately because he was
concerned about her kidneys.
Dr. Norris arranged for her to see a
a nephrologist specializing in kidney
disease. Andria has since been under
his care.
Since June, Andria has had two shunts
implanted, and has begun dialysis —
three times a week, four hours a day.
“The Founders Clinic saved my life,”
Andria said. “Every person I have come
into contact with — nurses, doctors, front
desk volunteers — have worked hard to
help me. I wouldn’t be here without the
Founders Clinic.”
For Andria, and for many other
uninsured patients, the clinic has not
only provided primary health care
services, but is serving as the bridge to
obtaining critical care from specialists.
Andria still faces some challenging
days ahead. But her road to a hopeful
future has been paved by the care she
received at the Founders Clinic.

2013 Family Medicine Cares Resident Service Award
The Family Medicine Cares Resident Service Award was established to give Family
Medicine residents an opportunity to fulfill a desire to address health disparities of
the underserved in their communities. Nearly 30 proposals were submitted, and
the selection committee faced the difficult task of choosing a grant recipient. The
committee was impressed with the residents’ dedication to implement projects that
will expand health care services to vulnerable populations.

Family Medicine
Fellowship Winners
The joint winners of the 2012 Center
for the History of Family Medicine’s
annual fellowship award recently visited the Center to complete research
on their respective fellowship projects.
Dr. Rick Flinders of Santa Rosa, CA
conducted research for his project,
“The Santa Rosa Story: A Local Chapter in the Nation’s History of Family
Medicine,” while his fellow awardee,
Ms. Robin Gotler of Richfield, MN
researched her project, “The Roots of
Family Practice Research.”
Dr. Flinders’ work is a study in the history and development of the Santa Rosa
Family Medicine Residency Program
within the larger context of the history
and evolution of Family Medicine.
Ms. Gotler’s fellowship award funded a
study that aims to further understand
what shaped the priority given to
research as the specialty of family practice was established in its early years.
Flinders is a practicing family physician and serves as the Inpatient
Director of the Santa Rosa Family
