❑ DESIGNATE: Each group or organization
needs to designate ONE Cash Back Coordinator.
❑ REGISTER:
ANNUAL REGISTRATION IS
REQUIRED. The Cash Back Coordinator should
complete the Registration Form even if you participated last year. You will receive your membership
number in the mail within 2-3 weeks.
❑ SAVE: Shop at Acme Fresh Market and save
your register receipts dated September 21,
2013 through February 8, 2014. Ask your
family, friends, co-workers and neighbors to shop
at Acme Fresh Market and save their receipts, too!
R.S.V.P. stores not included in the program.

SUBMIT: Pick up and complete a
REBATE FORM from the Customer Service
desk at any Acme Fresh Market. Submit the form
along with your receipts to any Acme Fresh Market
Customer Service Desk (do not mail to corporate
office) by March 1, 2014. No tapes will be
accepted after the deadline.
❑ RECEIVE: Acme Fresh Market will issue a
rebate check equal to FIVE PERCENT (5%) of the
Cash Back amount printed at the bottom of the
receipt. The Cash Back amount includes all Acme
Fresh Market store brand merchandise purchases.
These include: Acme Fresh Market, Food Club, Top
Care, Full Circle, Valu Time, Paws, World Classics,
Academix, Domestix, and Electrix products.

❑ MINIMUM OF $1,500.00: A minimum in qualifying product purchases is required to participate. This
is the Cash Back amount listed on the bottom of the
register receipt.
❑ PRINT DATES: September 21, 2013 to
February 8, 2014 Receipts must have print date
on them.
❑ BUNDLED RECEIPTS: You must have at least
one bundle of $1,500.00 to qualify. EXAMPLE: If
your organization collects $3,500 in tapes, you
would submit TWO Bundles marked $1,500
each, and ONE Bundle marked $500. If available, include a calculator tape or computer print
out with each bundle for easy auditing. (You
only need to use one rebate form for multiple
bundles turned in on the same day).
❑ SUBMIT RECEIPTS: Receipts can be submitted anytime during the program ($1,500.00
MINIMUM).
❑ CHECKS WILL BE ISSUED: Checks
are mailed to the organization name, contact
name, and address provided on the registration form. If coordinator changes contact
Mimi Locker at the number below. Checks
for this year’s program will be issued starting
in January. Please allow up to eight weeks.
Reissuing any lost or misplaced rebate
checks will incur a $12 fee.
