The goal of rehabilitation is to
help people achieve their highest
level of independence so they can
maintain a meaningful lifestyle.
The central theme of rehabilitation in ValleyCare’s Skilled Nursing Facility is
“Quality of Life.” Dignity, respect and encouragement of positive self-image is
afforded to all patients.

alleyCare’s Skilled Nursing Facility is a 26-bed unit on the third
floor of the Valley Memorial Center on the Livermore campus. The
unit offers skilled nursing care and individualized rehabilitation of
patients who are medically stable and no longer require acute hospital care.
The simulated home environment is designed to help patients make the
transition from hospital to home quickly and cost-effectively. Patients may
be transferred to ValleyCare’s Skilled Nursing Facility from ValleyCare
Medical Center in Pleasanton, or any acute hospital in the area.
The Rehabilitation Team
Patient care is managed by the family physician and coordinated by a multidisciplinary team consisting of:
• Nurses
• Physical Therapists
• Occupational Therapists
• Dietitians
• Respiratory Therapists
• Recreation Therapists
• Social Worker/Discharge Planner
• Resident Care Coordinator
• Pharmacist
• Speech Therapist (if required)
Each member of the team works with patients
to help them restore their maximum physical
capabilities and develop strategies for living
as independently as possible. The resident care
coordinator will work with the team to assess
each patient’s needs before being discharged
and makes arrangement for any needed home
care services.

Individualized Program
Treatment is intense and short-term. The average stay is 10 to 14
days. Some patients may have a skilled treatment plan that requires
a longer stay.
Patients generally are eligible for physical and occupational therapy.
Recreational therapy is also available. They learn how to use adaptive
equipment to make daily activities easier and safer. In the fully
equipped kitchen, patients are taught how to work around disabilities
and how to prepare meals safely and effectively to meet their needs.
Everything that takes place in the Skilled Nursing Facility has a
therapeutic purpose. Recreational therapy is meant to be fun, and at
the same time help people relearn cognitive and physical skills
diminished by their illnesses.
The program is tailored to each person’s specific needs and areas of
interest. For example, if a person loves to golf, they can practice
putting on the front lawn. Patients can also visit with their own pets
or participate in the Pet Therapy Program.

Family and Friends are crucial to recovery. They are encouraged to
participate in therapy sessions and in weekly meetings to monitor
progress and treatment goals.
