Dear Friends,
This year we’ve worked diligently to provide new programs and services
that meet the needs of our aging population, such as our new home care
affiliate VNA Home, Inc., Concierge Nurse Service, an expanded Geriatric
Care Management Program and being accredited with the American
Association of Diabetes Educators. For the eighth consecutive year in a row
we have excelled as a home care agency, earning distinction as one of the
top 25% of almost 10,000 agencies nationwide. Our staff is the reason for
that achievement. New Milford VNA has an extremely low turnover rate of
employees, and our staff members live locally and are active and involved
members of our community.
The response of our patients and their families for our home care services
and community members for our preventive programs is gratifying and
truly makes me excited about going to work every day. As the pace of
our lives seems to get faster and more hectic each year, our clients still
manage to take the time to write us notes, letters and emails praising our
wonderful staff. I never cease to be humbled by this.

Three New Programs
Responding to the challenging home health care needs of our quickly expanding and aging population, New Milford VNA has instituted three exciting new
programs this year.

The first is VNA Home, Inc. a non-profit affiliate which provides non-medical care so clients can remain independent, safe and comfortable in their own homes. Homemaker/companions can assist with light housekeeping,
meal preparation, provide companionship and conversation, run errands and provide transportation to doctor’s
appointments. Home Health Aides provide personal care including bathing
assistance, diet monitoring and meal planning, medication reminders and
ambulation assistance. VNA Home can also provide 24 hour client monitoring
for Alzheimer’s and dementia patients by customizing care plans, providing
meaningful therapeutic activities and maintaining a positive environment.
Our second new program is a Geriatric Care Management service which blends medical, financial, legal, counseling,
insurance and social services in a cohesive way. Care plans are crafted by Licensed Clinical Social Workers together with
the patient and family. Counseling for depression, anxiety and adjustment to loss, resource finding and referrals, and
recommendations regarding housing needs, modifications, and alternatives are given by the professional staff.
Lastly, we have begun a private pay innovative program of Concierge Nursing in which a Registered Nurse works one on one
with clients and is available 24 hours a day, seven days a week to coordinate the patient’s care on a very personal level. She

can explain medical results, prescriptions and doctors’ orders. She monitors medical records and facilitates communication
between the patient, the physician or hospital/skilled nursing facility to ensure healthy transitions. Our decades of building relationships in the medical
community help our nurses to be pro-active, initiating care and services needed now as well as in the future.
New Milford VNA Achieves Diabetes Educator Accreditation
New Milford VNA & Hospice has always been responsive to the changing health needs of our community. From
our founding in 1918 in response to the world-wide influenza epidemic to today’s use of high tech at-home patient
monitoring through our Telehealth Program, we constantly seek to meet the community’s needs. Today our
community faces what can be described as a diabetes epidemic, with 9.3% of the U.S. population having this
condition, and 25.9% of persons 65 years or older are diabetic. Diabetes which is not controlled can lead to
increased hospitalizations and readmissions, strokes, cardiovascular disease, blindness, kidney failure and amputations.
In response, we hired Diabetes Certified Educator Dianne Cook RN, BSN to direct our
accreditation. For over twenty years Dianne was responsible for all in-patient and
out-patient diabetes education for New Milford Hospital. She will be joined by New
Milford VNA staff nurse Joanne Tiebout Douglas, RN, BS who received her Diabetes
Education certification in July of this year. Having an accredited program is important
as it ensures that we meet the National Standards for Diabetes Self-management
Education and Support. Together they will provide individual education to patients
referred to New Milford VNA by their physicians and will also be responsible for
providing guidance to our home care patients who are managing diabetes along

with other conditions, post-surgery or hospitalization. They are looking forward to
expanding New Milford VNA’s diabetic education program to include support groups and ongoing educational activities in local senior centers, clubs and
businesses.

This Year’s Hospice Innovations
Our Hospice Program was busy this year with a series of presentations and developing new literature to continue
to educate interested individuals and the medical community about what Hospice can offer to patients and
their families. We instituted what is called the “Hospice Hot Line” ([email protected]), an
email tool whereby people can pose questions to a Hospice staff member to help start a conversation about
the many services and benefits of Hospice care. We also developed a “Reference Guide for Clinicians” which
was distributed to local physicians, nurses and social workers. This booklet offers many strategies for clinicians
to begin talking about Hospice with appropriate patients. It explains eligibility criteria (including prognostic

criteria for specific life-threatening diseases), the Hospice Medicare Benefit, and
the many services provided by our Hospice. Perhaps most importantly, it includes “Talking Hospice,” a series of suggested
conversations which can assist the physician in helping the patient and their family members determine their personal plan
for managing their disease and making important decisions regarding care and quality of life.
Hospice Team Leader Kathy Leier RN, BSN, was busy this year giving a series of presentations on Hospice. She addressed fifty
students at Brookfield High School who have expressed an interest in entering the health care field. For many students it was
a first time introduction to the topic of Hospice and end of life issues, and their response was very positive and genuine. Kathy
also coordinated an evening dinner event at The Silo which addressed the topic of “Hospice and Spirituality.” She spoke about
the changing trends in Hospice care and introduced New Milford VNA’s new programs, Concierge Nursing, VNA Home, Inc., and
Geriatric Care Management. She was joined by Hospice Chaplain Rev. Chris Sakis and St. John’s Episcopal Church
pastor Rev. Jack Gilpin, who related personal stories and experiences they have encountered dealing
with patients and parishioners during their final journeys. Kathy also made two presentations
on the topic of “Hospice and Palliative Care” through the
New Milford Adult Education program.

Community Health Coordinator

Geri Rodda, RN, was named the Centers for
Disease Control and Prevention’s 2014
Connecticut Childhood Immunization Champion.
Geri was honored on April 30 during a ceremony at
the New Milford Town Hall during which representatives from the State of Connecticut and the Town of New
Milford acknowledged her contributions to the field of immunization education. Her book “Vaccines for Maxine” has been distributed worldwide and New Milford VNA’s Well Child Clinic, which she oversees, donated 1,000
copies to immunization coordinators in Connecticut. It has been exceptionally well received by the medical
community, including pediatricians and obstetricians. Ms. Rodda has been the community health nurse for
New Milford VNA and a mentor and leader in the promotion of childhood immunizations for the past 15 years.
While serving as a nurse consultant to area child care centers she observed that many parents do not understand the benefits of childhood immunizations. Without that knowledge some parents are reluctant to vaccinate their children. She decided to tackle this problem using her estimable skills as an author and poet.
Her first immunization book “The Flu and You” stressed the importance of receiving flu vaccine. “Vaccines for
Maxine” tells children about the history of vaccination and need for all ages to be immunized against preventable diseases. Congratulations, Geri, on all of your
achievements and on this great national honor!

We are happy to welcome a new member to our loyal team of New Milford
VNA Hospice volunteers, Nancy Steinman. A resident of Washington
Depot since 1985, Nancy may be new to our Hospice Program, but she
certainly isn’t new to volunteering! She has been involved in a host of
community activities, including Garden Club membership and writing
a newsletter for her church,
and has been interested and
involved in raising awareness of
environmental issues for many
years. An art history major
at the University of Southern
California, Nancy also worked
in the insurance business, for
an energy conglomerate and in
investment banking.
Jaime is excited to be here and has enthusiastically embraced all aspects of
fundraising. She said, “New Milford VNA & Hospice relies on the generosity
of its donors, and as the Fund Development Coordinator, it is my goal to
establish a relationship with all of our contributors.
I hope to continue to make NMVNA one of the top home health care
agencies in the country by ensuring that we have funds to better serve our
patients and families. One of the ways I hope to do this is by expansion of
our social networking, and by visiting past, present and future supporters.
It is so important to make sure our donors know that they are appreciated
and that they have a strong understanding of how important their
support is to our success. We provide many unreimbursed services to the
community, and fundraising is crucial to continuing these programs.”
Jaime is a Ridgefield resident, but no stranger to New Milford, as she is a
graduate of the Canterbury School. After her graduation from Wesleyan
University in 2012 she worked at Canterbury as Associate Director of
Development and Assistant Director of Admissions. While at Wesleyan she
was captain of the Women’s Ice Hockey team and coordinated fundraising
efforts to support financial aid scholarships. Jaime is working closely with
our Marketing Department to ensure efficiency and streamlining of our
events. We are so pleased to welcome her to our staff and wish her much
success!
Nowadays, however, Nancy
acknowledges her most important role is that of long-distance caregiver
for her mother who is 91 and lives in a senior residence in California.
She relates that this joy and responsibility is one of the things that drew
her to become a Family Support Volunteer in our Hospice Program. “So
many baby boomers are dealing with aging parents and the challenges
their care brings to the family. I have always been drawn to the elderly
population and have learned so much from their life experiences. The
world has changed so much during their lifetimes. I honor them and feel
that Hospice care is a noble calling. It is an honor to be in the presence of
someone who is entering their final life journey.”
We, at New Milford VNA, are honored to have Nancy join us as a member
of the Hospice team. She is a caring and compassionate person and we
hope she continues with our agency for many years!

Fundraising & Events 2014
Thanks to the generous donations of our loyal
supporters, our fundraising efforts were a great
success this year! Events such as the Hospice Tree of
Life, Community Breakfast and Tea for Two Hundred
generated more participants than ever before. New
Milford VNA & Hospice takes pride in being able to
provide services to members of our community
without regard to race, creed or ability to pay, and
it is because of you and your donations that we
are able to continue our mission. Fundraising is
imperative to our organization’s future, and we look
forward to making next year’s events even bigger!

Henderson, friend and
supporter of New Milford
VNA & Hospice who died on
February 25th at the age of
84. Ruth always said “Yes!”
when we asked for her
guidance and help, whether
it be for event coordination,
fundraising, or providing a
name from her seemingly
unending list of friends and
resources. She hosted our
Artists for Hospice fundraiser at The Silo for many years, and was a guest
speaker at our Tree of Life Lighting ceremony, where she spoke of her high
regard for the concept of Hospice care and for New Milford VNA’s Hospice
Program in particular. Ruth and her husband Skitch donated regularly
and generously to New Milford VNA’s many events and fundraisers, right
up until the time of her death. A co-chair of the New Milford Hospital
Foundation for two decades, she was instrumental in helping set up a
dedicated room for Hospice patients at New Milford Hospital which was
funded in part by what became an annual holiday fundraising event at
the Silo. Many of our Hospice patients received end of life care in that
beautiful, warm and homelike atmosphere. Ruth, simply put, lived a life
of service to her community, family and friends. She will remain a role
model for all of us.

death this past year of their loyal
supporter, volunteer and contributor,
Madeline “Mal” Jacobsen. Madeline
was a resident of Kent for twentyfour years, where she devoted many
hours of her time and considerable
talents to community organizations and activities. One of her favorite
activities was helping run FISH (Friends in Service Here), a service for people
needing transportation to doctor or hospital appointments. For many
years she organized the calendar, scheduled message takers and drivers,
did transportation runs and would fill in for cancellations. For “Mal”, as
friends and family called her, this voluntary avocation was nearly a full
time job. She was also active in the Kent Garden Club serving as President,
the Town of Kent, serving as Deputy Registrar of Voters, and at Sacred Heart
Church where she was always available for fundraising or other projects.
An avid horse rider, she became involved in H.O.R.S.E. of Connecticut and
eventually sheltered and cared for horses who “retired” on her property. At
New Milford VNA & Hospice we remember Mal for her many years of work
on the Kent Tree of Life Committee, helping with the mailing and using her
many contacts in Kent to raise awareness of the program. We were also
privileged to work with her through the Kent Nursing Association which
helps raise funds every year to underwrite New Milford VNA services for
Kent residents. Mal is very much missed by her husband George, her sons
and her grandchildren, as well as all of us at New Milford VNA & Hospice.
We appreciate all of her work on our behalf.

Camp Jonathan Guardian & Donor
Dear Mary,
I would like to take this opportunity to thank you and all
the members of the VNA-Hospice involved with Camp
Jonathan.
My three grandchildren were able to attend this summer’s
session. It was a positive experience for all of them. The
creative approach to bereavement helped them work
through their grief, sometimes with tears and even more
often with smiles. The stuffed animals and quilts have
become permanent fixtures on their beds, providing
them with wonderful memories and immeasurable
comfort.
As I stated at the family memorial dinner, one of the
best aspects of Camp Jonathan were the teen volunteer
counselors. They were especially impressive as positive
role models for Marie, Michael and Thomas.
I have made a donation online through the VNA website
and requested that it be used towards Camp Jonathan.
Please express my appreciation to Mary Lee, Ruthellen,
Rosemary, Catherine and Donna, as well as anyone I may
not have mentioned.
