Thank you for taking the time from your busy schedules to come to our conference. Often unknown to us we pass
people everyday who are searching for answers. Their life speaks of uncertainties and not possibilities. They plod
through life on a day to day basis. Many times downcast and resolute in their acceptance of the negative aspects of
life, they nonetheless on occasion see a glimmer of light in the darkness of their lives. To get through each day can be a
struggle. In this mire of thought and belief, the slightest penetration by a friend or family member or some
authoritative figure can give rise to the smallest of embers. There are those who rise up and move closer to this small
but penetrating light. As they get closer there is the slightest warmth that comes to their heart, a heart sadly cold
because there is no future, there are no answers. The soft whispers of encouragement from loved ones and others
begins to clear the mind of the web of questions and doubt.
This person has at one time or another come into your life. For some of you, it is an everyday occurrence. For
others it is a sometime thing. They come to us and look searchingly into our eyes for answers and dignity. People
yearn to have meaning in their life, but to those who carry great burdens, self-worth is fleeting and disappears easily
in the mist of endless questions. Those whose burdens are physical, mental, or emotional are attracted to integrative
medicine because there are new possibilities for answers. Integrative medicine gives them the platform to participate
in their own healing. They move from being passive to being active in their treatment. Even while the outcome is yet
undetermined, they come to the understanding that it is their body and their mind—it is their future. They are
encouraged to learn about not only their condition but more importantly about their body and how at the core of our
being we are designed and we are engineered to heal ourselves. Make no mistake, those who cross the threshold of
your office are in the midst of a battle. A battle so basic it is the battle for life and existence. As an integrative
practitioner, you have chosen to stand at the side of these people and do battle with them. You have chosen not to
believe the bad report, but to believe that each person has the innate ability built within them to be healed. While not
all will succeed in this quest, they can live every moment of life and live it to the fullest. This is what you can and do
give to those who are searching for answers, you give them the possibility of life and hope renewed.
The importance of this conference, and others like it, is not limited anymore to the dissemination of valuable
knowledge, but, maybe, more importantly the coming together of the true leaders of health care reform, those who
practice a type of medicine that is solely focused upon the client and the client’s needs while involving the client
directly in the healing process. We congratulate you on investing the time, energy, and money to be a part of not only

Jeff Camasi, On March 15, 2012 was a biscuit shy of 400 lbs. His triglycerides were
at 465. His cholesterol was 249. After drinking Green Smoothies for EXACTLY 30
DAYS his triglycerides went down to 170 and his cholesterol went down to 199. After

Thomas E. Levy, MD, JD is a board-certified cardiologist and a bar-certified attorney. After

The AMA establishes the criteria under which AMA PRA Category 1 Credits ™ can be earned. This criteria includes
requirements for event organizers, speakers, and participants. The organizers and speakers have met these criteria. As a participant you are required to meet the following requirements:
Sign in every day. A sign-in sheet will be available at the registration area and/or Academy CIM’s booth. You
cannot sign in once for the whole conference. You must sign in each day. These sign-in sheets are required to be
archived. Credits cannot be given if you have not signed in for the specific day in which you desire to earn CME
credits.
In order for you to earn a CME credit, you must submit an evaluation form for each speaker activity/
presentation. Compliance requires that you fill out completely the form associated with the presentation for which
you desire to receive CME credits. Evaluation forms will not be considered to be filled out if there is not a number
(1-5) in each small blank where you are requested to rate an activity. For those that are in attendance at the final
presentation of the Conference, the evaluation forms will be collected at the end of the presentation. For those
who are not in attendance at the end of the Conference, you must deliver your completed evaluation forms to Dr.
Lee Cowden before your departure from the event facility.

Upon review of the sign-up sheets and your evaluation forms, a certificate with the number of presentation
credits earned will be mailed to you. Generally this procedure takes between 14 and 21 days at which time the certificate will be mailed to you at the address provided to us on your evaluation forms.

Two hundred and thirty seven years ago a group of men gathered in Philadelphia to finalize the sentiments of the majority of residents of
the then American Colonies of the British Empire. The disgruntled masses that made up the burgeoning colony across the sea from its
distant king, George III. Having tried to right what they saw as deep and abiding injustices
through negotiation, they felt the need to declare freedom from what they considered to be
tyranny. So in July they gathered in this Pennsylvania city to lay the foundation for what would
be called “the great experiment”. Never before had such an idea been birthed that had survived
and there was considerable doubt as to the outcome of this birth. The colonies were no match
for the forces of the British Crown. Not everyone in America agreed with this growing rebellion
and many more did not want to fight even though they were oppressed. But a small select
group of men gathered to represent their states. They made a dramatic stand against tyranny.
They must have known that they were casting their futures into the wind, for their was no
certainty that they would prevail. The fifty six men who gathered to sign a declaration of

The war that ensued from the signing of the Declaration of Independence was devastating to many Americans who risked their fortunes
and lives for a cause greater than their personal needs. Many in the United States of today feel a similar need today where healthcare is
concerned. In many ways the things that are transpiring in this great country today are symbolic of war. There is an overwhelming
desire by the government to take over the lives of its citizens. Once an entity has control of a person’s body and can dictate what that
person can do in regards to their own body, that entity could be called tyranny. What should be more treasured by a person than their
own body? And yet people have been giving the right to dictate about what happens to their body to a disinterested third party we call
the Established Medical Complex. People have relinquished their responsibility for caring for their own person to this conglomerate of
self-interest entities because they have been convinced that they are too ignorant to make the important decisions about their own health
and wellness.
Join us as we build an alternative to the Established Medical Complex that puts the person at the center of their own health. Let this be a
peaceful determined revolution of ideas, practices, and beliefs that catapults us to new expressions of wellness. Many marvelous and
dedicated men and women have laid a sacrificial foundation on which to build. Just has enlightened visionaries over two centuries
