NEW GAS CONNECTION APPLICATION The Civil Supplies Department was originally only a regulatory Department. Subsequently, its activities have been diversified to include procurement of food grains under Minimum Support Price (MSP) , distribution of essential commodities viz. Rice, Wheat, Sugar, Kerosene, Palmolive Oil and Red gram dal through Fair Price Shops under the Public Distribution System at subsidized rates through issue of computerized cards, consumer affairs, monitoring of prices of essential commodities, distribution of LPG connections ections to BPL women (Deepam Scheme), Enrollment under UID (Aadhaar) etc., This Service is Used for Applying New Gas Connection Through Meeseva. Apply for New Gas Connection Application : This section contains instructio instructions for the operators of the Meeseva seva web application to apply for New Gas Connection Application . 1) In Meeseva home screen, under List of Services, select Civil Supplies Department as depicted in Figure 1. Request Screen will be displayed as depicted in Figure 3: New Gas Connection Application request screen 4) Enter LPG Distributor Details Such as LPG Gas Type, District, Location, LPG Distributor Company, Distributor Address, Distributor Pincode, Distributor Contact No and Distributor Email ID as depicted in Figure 4. Figure 4: Distributor Details in New Gas Connection Application request screen 5) Enter Applicant Details Such as Applicant Name, Age, Profession, Working Couple, No. of Family Members, Income Per year, Whether Used LPG Earlier, House No, Land Mark, District, Mandal, Village/Ward, Pin Code, Office Phone No, Residence Phone no, Mobile no and Email id as depicted in Figure 5. Figure 5: Applicant Details in New Gas Connection Application request screen 6)) Enter all the Informant details such as informant Name, Relation, Mobile No, No Delivery Type, Proof Document and Proof Document No as depicted in Figure 6. Figure 6: Informant details in Request Screen 7)) Collect the necessary supporting documents as indicated in the New Gas Connection Application request page. scan the documents and upload into the system, as depicted in Figure 7. Note:- Here Proof documents should be uploaded based on Selection. Figure 7 7: Documents upload list in the request Screen Note: All the mandatory fields have to be filled in; otherwise request will not be accepted by the system. 8) Click Show Payment. Another window appears requesting to Confirm Payment as depicted in figure 8. Figure 1 10: Payment Confirmation section On confirmation, a receipt will be generated as depicted in the following figure. The receipt will contain the Certificate delivery date. Note: Ensure that you have sufficient balance with your service provider (SCA), else the system will not accept the request. Figure 1 11: New Gas Connection Receipt Note: - On submission of the request, C Citizen Receives Message. Like your Request for New Gas Connection Application has been entered vide Application Number, Transaction Id and sent to Dept. 11)) On submission, the request will be sent to the work flow of the respective Department for further processing.
