Page 2 project experience VISN 1 VA New England Healthcare System, Bedford, MA (2008-2010) Statement of Conditions Surveys Support was provided to client, EOC Solutions, to conduct campus-wide Statement of Conditions (SOC) surveys in VISN 1. The campuses, totaling more than 4.7 million-square feet of various healthcare occupancies, have included: • • • • • • • • • • VA Connecticut Healthcare System – West Haven Campus, CT Providence VA Medical Center – Providence, RI VA Boston Healthcare System – Jamaica Plain Campus, MA VA Boston Healthcare System – West Roxbury Campus, MA VA Boston Healthcare System – Brockton Campus, MA Edith Nourse Rogers Memorial Veterans Hospital – Bedford, MA Northampton VA Medical Center – Leeds, MA Manchester VA Medical Center – Manchester, NH White River Junction VA Medical Center – White River Junction, VT Togus VA Medical Center – Augusta, ME Edith Hourse Rogers Memorial Veterans Hospital Wright Patterson Air Force Base Medical Center, WPAFB OH (2007-2008) Fire Protection & Life Safety Systems Conditions / Joint Commission Hughes prepared an assessment and Joint Commission on Accreditation of Healthcare Organizations Statement of Conditions for the six-story, 750,000 square foot, Air Force Medical Center. The systems evaluated during this project were the fire alarm, fire sprinkler, passive smoke control, active smoke control, heating and ventilation, and mass notification. The building Statement of Conditions included the Basic Building Information (BBI), Life Safety Analysis (LSA), and Plan for Improvements (PFI) sections as based on the NFPA 101 2000 Edition requirements for healthcare occupancies and business occupancies. Deficiency documentation and resolutions, resolution cost estimates, life safety plans, and SOC report services were provided. VISN 4 VA Medical Facilities, Pittsburgh, PA (2006-2007) Statement of Conditions Surveys Surveys were conducted of 256 buildings, totaling approximately eight million square feet,and evaluated for compliance with NFPA 101. The surveys include hospitals, nursing homes, hotels, administration, and utility buildings. A Statement of Conditions (SOC) report was developed for each campus within the VISN 4 network. The SOC forms for The Joint Commission were prepared for the healthcare and other applicable occupancies. Johnson VA Medical Center – Clarksburg, WV Page 3 St. Johns Medical Center, Detroit, MI (2007-2008) Statement of Conditions A full Joint Commission on Accreditation of Healthcare Organizations SOC was developed for this high-rise multi-building hospital. The building Statement of Conditions included the Basic Building Information (BBI), Life Safety Analysis (LSA), and Plan for Improvements (PFI) sections as based on the NFPA 101, 2000 Edition requirements for healthcare occupancies, ambulatory care occupancies, and business occupancies. Saint Peter’s University Hospital, New Brunswick, NJ (2009) Joint Commission Compliance Support Hughes provided fire and life safety code consulting to update the SOC for Saint Peter’s University Hospital. St. Johns Medical Center Cleveland Clinic, Cleveland, OH (2006) Joint Commission Consulting SOC’s were developed for a number of buildings on this 2 million-square foot campus. VA Medical Center, North Hampton, MA (2006) Joint Commission SOC Upgrades Hughes conducted a site investigation and reviewed The Joint Commission Statement of Conditions previously prepared. Fire safety consulting services were provided to address deficiencies identified in the report and found during site inspection. MetroWest Medical Center Facilities, Framingham, MA (2006-2008) Joint Commission Site Visits & Support SOC support was provided for Joint Commission compliance. Fire safety consulting services were also provided for renovations as needed. Projects included Leonard Morse Hospital and Framingham Union Hospital. The alternative life safety plan included submission to The Joint Commission for an equivalency request. Jacobi Medical Center, Bronx, NY (2008-2011) Life Safety Survey Inspections Hughes provided quarterly inspections of fire and smoke doors, annual inspections of corridor doors, and quarterly inspections of fire and smoke walls for penetrations that are not properly fire stopped. All findings were documented in written reports. Office of Mental Health, New York State Office of General Services, New York, NY (2004-2008) Code and Fire Protection Consulting Hughes provided code and fire protection consulting services under contract to the Office of General Services on a task order basis for Office of Mental Health facilities.
