The owner may operate on his or her own or may employ others. The owner of the business has total and unlimited personal liability of the debts incurred by the business. Corporate body A body incorporated under the Companies Act 1965 [Act 125]. Partnership A business entity in which two or more people operate for the common goal of making profit. Each partner has total and unlimited personal liability of the debts incurred by the partnership. Society An association, club or body that function based on membership consisting of seven (7) members or more in addition to having certain purposes, whether temporary or permanent, which is registered under the Societies Act 1966 [Act 335] (considering under the Private Healthcare Facilities and Services Act 1998 [Act 586]) Type of establishment Hospital An institution with primary function to provide inpatient diagnostic and therapeutic services for a variety of medical conditions both surgical and non surgical. Non Specialist Hospital The non specialist hospitals were mainly seen in the public sector. These are hospitals without resident/permanent specialists. Specialist hospital Hospitals that provide secondary and tertiary services. Specialised medical institutions/specialist hospitals are hospitals providing services in specific disciplines such as cardiology, ophthalmology, respiratory etc. Maternity centre A hospital that is used or intended to be used for the provision of obstetric and/or gynaecological care that extends beyond care in a normal pregnancy/labour/post-partum period/normal newborns and may involve the conduct of deliveries besides spontaneous normal vaginal deliveries. Inpatient beds A bed used for patients admitted for diagnostic and/or therapeutic purposes in a hospital. The figures should include cots or bassinets set aside for babies requiring special care that should exclude the following: • Labour room, post–anaesthethic or post-operative beds which are not maintained for continual use by patients. • Cots for normal newborn infants in obstetric wards. Inpatient A person who undergoes a formal process whereby he/she is accepted by a hospital for the purpose of hospital treatment. 131 Appx 1 List of Definitions • Observation or recovery beds in out-patient department. NATIONAL HEALTHCARE ESTABLISHMENTS & WORKFORCE STATISTICS 2010 HOSPITALS Admission Admission (or inpatient admission) is the formal acceptance by a hospital of a patient who will occupy a hospital bed, crib or bassinet for observation, care, diagnosis or treatment and will have a medical record maintained for him. Infants born in the hospital that need medical care and for whom separate medical records are maintained are counted as admissions, (Foetal deaths or still births are not counted as admissions) Average length of stay (ALOS) (in days) The average number of days that inpatients (excluding of newborn) remained in the hospital. Length of Stay = (Sum of Length of Stay of discharged patients for a given period/Total number of discharged patients in the same period) Bed Occupancy Rate (BOR) (%) The percentage of inpatient beds occupied for a given period of time. Also known as the percentage of occupancy. BOR = (Inpatient Days of Care/ Bed Days Available) x 100 Where: Inpatient Days of Care: Sum of each daily inpatient census for the given period. Beds denote inpatient beds that are set up, staffed and available for care. Bed Days Available: The maximum number of inpatient days of care that would have been provided if all beds were filled during the given period and takes into account changes in the number of beds available for use during the same period. For example: Hospital X had 300 beds from July 1 to Feb 28. The number of beds in services was increased to 350 beds from March 1 to June 30. The bed days available are calculated as: 300 beds x 243 days 72,900 (July 1- February 28) = 350 beds x 122 days = 42,700 (March 1-June 30) Total bed days available = 72,900 + 42,700 = 115,600. Turnover interval (TOI) (in days) Average period in days that an available bed remains empty between the discharge of one inpatient and the admission of the next. Total number of inpatients bed X 365.25 TOI = - ALOS Total number of inpatient admissions Nurses with post basic training A registered nurse with a degree/diploma and has post basic training. Nurses without post basic training A registered nurse with a degree/diploma but without post basic training. Community nurse A registered nurse with a certificate in nursing. Assistant medical officer A registered assistant medical officer with the Medical Assistant Board Specialist A registered medical practitioner with post-graduate qualification recognised by both the Ministry of Health and the Public Service Department Malaysia; and has the necessary core clinical competence and core procedural skills as expected of doctors with the responding recognised postgraduate qualification. 132 NATIONAL HEALTHCARE ESTABLISHMENTS & WORKFORCE STATISTICS 2010 HOSPITALS Resident An inhouse medical practitioner/specialist who is available to diagnose and treat patients at your hospital; and is a permanent staff in your establishment. Visiting A medical practitioner/specialist who is available to diagnose and treat patients at your hospital; but he/she is not a permanent staff at your hospital. The medical practitioner may make regular visits (eg. monthly/biweekly/weekly etc) to see patients in your hospital or is accessible on an ad-hoc or emergency basis only. Computed Tomography Scanner (CT Scan) Imaging equipment that uses a combination of x-rays and computer technology to produce detailed cross-sectional images (slices), both horizontally and vertically, of any part of the body. Magnetic Resonance Imaging (MRI) unit Imaging equipment that uses a combination of large magnets, radio frequencies and computer technology to produce detailed images of organs and structures within the body. Positron Emission Tomography (PET) Imaging equipment that comprises of multiple rings of detectors which record the emission of energy from radiotracers given to patients and are in turn, used to develop biologic images that reflect metabolic activities of cells. CHAPTER OBSTETRICS & GYNAECOLOGY Delivery Beds Any bed on which a delivery is conducted. It is not used for inpatient care. It would be possible to put a patient in lithotomy position on such a bed Operating Theatre (OT) A facility within a hospital that is designed and equipped with facilities to perform sterile surgical operations and has facilities for specialised care for post-operative patients. Dedicated OT An infrastructure within a hospital that is designed and equipped with facilities to perform sterile surgical operations exclusively for obstetric patients and has facilities for specialised care for such patients. Obstetric HDU Beds A unit that provides monitoring facilities for obstetric patients that would not normally be found in the wards. Ventilation of such patients would not be carried out in this facility. All types of Deliveries Any type of birth of a foetus or foetuses alive or dead from a pregnant mother, vaginally or abdominally. Lower Segment Caesarean Section (LSCS) A surgical procedure done via a laparotomy and an incision or incisions in the lower segment of the uterus to deliver one or more foetuses that may be alive or dead. 133 Appx 1 It occurs when a pregnant female goes into labour without the use of drugs or techniques to induce labour, and delivers her baby in the normal manner, without forceps, vacuum extraction, or a Caesarean section. List of Definitions Spontaneous Vaginal Delivery (SVD) NATIONAL HEALTHCARE ESTABLISHMENTS & WORKFORCE STATISTICS 2010 HOSPITALS Vaginal breech deliveries Delivery of the foetus or foetuses by the buttocks or feet first through the vagina Instrumental deliveries using Vacuum A vaginal delivery that is completed by using the vacuum extractor Caesarean Hysterectomies A surgical operation in which the uterus is removed at the time of Caesarean section. Medical Officer Registered medical practitioners who possess an annual practicing certificate issued by the MMC and working in the obstetric and labour wards. Nurses with midwifery training in Obstetric Wards Nurses who are registered midwives with the Midwives Board Malaysia working in obstetric wards. Nurses with midwifery training in Labour Wards Nurses who are registered midwives with the Midwives Board Malaysia working in labour wards. Obstetric ward A ward that provides care to antenatal and postnatal patients. There will be probate facilities where other patients share the ward with postnatal patients. Labour ward A ward that allows the monitoring of a pregnant mother in labour and provides facilities for the conduct of normal and instrumental deliveries as well as care of the newborn infant(s). Maternity Homes Any premises, used or intended to be used for the reception of, and the provision of nursing care and midwifery in labour, or of women immediately after childbirth. CHAPTER PAEDIATRICS Paediatrics medical wards Wards for admission of children older than 28 days with medical illnesses. It does not include PICU/PHDW or paediatrics surgical ward. Note: Where the same facility caters for both medical and surgical cases, the facility is considered shared. Paediatrics medical beds Beds in the paediatrics medical ward. Neonatal Intensive Care Unit (NICU) A facility or a unit of a hospital dedicated and equipped to provide specialised care of ill or premature newborn infants. The NICU is distinct from the special care nursery (SCN) in providing a high level of intensive care to premature and ill newborn infants while the SCN provides specialised care for infants with less severe medical problems. It consists of ventilator supported beds for neonates, e.g. Level III (intensive care) beds, but may have Level IIb (semi-intensive) care too. Special Care Nursery (SCN) A unit for care of neonates who do not require ventilator support. 134 NATIONAL HEALTHCARE ESTABLISHMENTS & WORKFORCE STATISTICS 2010 HOSPITALS Paediatric Intensive Care Unit (PICU) A facility within a hospital dedicated and equipped to provide specialised of critically ill infants, children and teenagers. A PICU is typically directed by one or more paediatrics intensivists or PICU consultants; and staffed by doctors and nurses who are specially trained and experienced in paediatric intensive care. Note: Where the same facility caters for both adults and paediatric cases, then the facility is considered as an ICU only. Paediatric High Dependency Ward (PHDW) A facility within a hospital for step-down care of ill infants, children and adolescents from PICU and step-up care for ill infants children, and adolescents. CHAPTER EMERGENCY Red zone cubicle A red zone cubicle is one that possesses all of the following descriptions: • Located in an area of the emergency department designated for management of the acute critically ill or injured patient or described often as the critical care zone. • Dimensions conform to or exceed standards for the emergency department of a MOH hospital. A dimension between 16m2 – 18m2 excluding the storage area is deemed minimally appropriate. • Has a specialised resuscitation bed that is appropriate to manage a critically ill or severe trauma patient, medical grade overhead lighting. • Space that allows complete 360° access to patient for procedures. • Space to position patient monitors, resuscitation equipments, ventilator, equipments for oxygen delivery, suction, irrigation. • A dedicated physiological monitor that at minimum carries the following capabilities: NIBP, IBP, SPO2, RR, Temperature, ETCO2, ECG • All medical gases that are internally piped in. • Bed-head service panel that include at a minimum: 3 uninterruptible power supply sockets on either side of patient (total 6),2 piped in oxygen outlets ,2 vacuum/ suction outlet, 2 medical air outlet • External service panel that include at a minimum: 5 uninterruptible power supply sockets on either side of patient (total 10),3 piped in oxygen outlets ,2 vacuum/ suction outlet,2 medical air outlet • Medical grade hanging pendants are appropriate to meet the above needs. • Organizer system that allows systematic, easy storage or access to essential items for delivery of emergency care to patients. • Appropriate functionalities such as IV drip poles or monitor/defibrillator tiered racks, internet network outlets. • Emergency call system, X-ray viewing box for at least 4 films, funduscopy and auroscopy diagnostic set, Clock and resuscitation-timer. Patient privacy is ensured visually and audibly whilst not compromising critical care monitoring requirements. Such provisions of emergency care when shared, does not reduce or affect level of care provided to other patient of similar category (critical) at any given time. Yellow zone cubicle A yellow zone cubicle is one that possesses all of the following descriptions: • Dimensions conform to or exceed standards for the emergency department of a MOH hospital. • A dimension between 12m2 – 14m2 excluding the storage area is deemed minimally appropriate. • Has a specialised patient bed that is appropriate to manage a semi-critically ill or moderately injured trauma patient. 135 Appx 1 • Occupies a physically distinct care area that allows care for a patient to be uninterrupted while in close proximity and rapid access to the critical care zone. List of Definitions • Located in an area of the emergency department designated for management of the semi-critically ill or injured patient or described often as the semi-critical care zone. NATIONAL HEALTHCARE ESTABLISHMENTS & WORKFORCE STATISTICS 2010 HOSPITALS • A dedicated physiological monitor that at minimum carries the following capabilities: NIBP, SPO2, RR, Rhythm ECG • Bed-head service panel that include at a minimum: • 2 uninterruptible power supply sockets on either side of patient (total 4),2 piped in oxygen outlets ,1 vacuum/ suction outlet,1 medical air outlet • X-ray viewing box for at least 2 films. Such provisions of emergency care when shared, does not significantly affect level of care provided to other patient of similar category (semicritical) at any given time. Other devices such as funduscope, organiser system are as for red cubicle. Asthma cubicles A dedicated area within the asthma area/zone where patient can be treated for mild to moderate exacerbation of asthma or COAD. Within this cubicle there should be dedicated structures to manage asthma patient such as oxygen outlet or oxygen access, suction outlet and asthma chair. Paediatrics resuscitation cubicle Paediatric resuscitation cubicle for child under one year or baby includes radiant warmer with resuscitation cot. Observation ward An area where patients can be observed or have early investigation/management within the Emergency and Trauma Department. Patients are admitted to this area with an expectation of discharge within 24 hours. ‘Functioning’ ambulance Functioning ambulance means dedicated ambulance that is ready and well equipped for the purpose of emergency response. This will rule out those ambulances that are under beyond economic repair (BER) status, repair and not readily available. OSCC room A room dedicated for the management of the survivors of violence against women, children and the elderly. It is inclusive of small rest area, examination room, specimen handling area and attached toilet/bathroom. This room should away from the main wait area of the Emergency and Trauma Department. Psychiatric evaluation room An area/room dedicated for the assessment and management of suspected non organic cause of altered behaviour/psychosis or exhibiting severe symptoms of mental illness. Parenteral thrombolytics therapy Thrombolytic therapy administered by means other than through the alimentary tract (as by intramuscular or intravenous injection). Poisoning A condition that occurs when any substance interferes with normal body functions after it is swallowed, inhaled, injected, or absorbed. Emergency primary response Ambulance that responds or is dispatched to the scene of incident in response to emergency calls from the public/victims. These ambulances are readily available (dedicated) for such services. This differs from Secondary Response where the response is only carried out after the first responders arrive at the scene and request back up ambulances. Interfacility ambulance run Interfacility ambulance run is the process of patient transport system between health care facilities and, though not an emergency situation, transportation of critically ill patient could be done. This is for the purpose of either upward or downward patient care. This transport service is a large component of ambulance service which connect the different healthcare facilities; between clinic to hospital, hospital to hospital and hospital to specialised service centres and also to homes. 136 NATIONAL HEALTHCARE ESTABLISHMENTS & WORKFORCE STATISTICS 2010 HOSPITALS CHAPTER GENERAL SURGERY Operating theatre An infrastructure within a hospital that is designed and equipped with facilities to perform sterile surgical operations and has facilities to monitor patients in the immediate post-operative period. Operating rooms A room within an operating theater that is designed and equipped with facilities to perform sterile surgical operations. One operating theater (OT) usually has more than one operating room (OR). Emergency/Trauma Theatres A 24-hour operating theatre dedicated for urgent patient interventions. Urgent is defined as acute onset or deterioration of conditions that threaten life, limb or organ survival. CHAPTER ANAESTHESIOLOGY GA (General Anaesthesia) Cases refer to cases where anaesthesia is administered by an anaesthetist or anaesthesia team.This includes regional or plexus blocks. LA (Local Anaesthesia) Cases refer to cases where anaesthesia is administered under sedation, local anaesthesia or by nerve block. Anaesthetic Clinic A clinic managed by the Anaesthesiology Department, dedicated to pre-operative assessment of patients booked for surgery. Pain Clinic A clinic that manages patients suffering from pain; consultants who see the patients are pain specialists, anaesthesiologists or other specialists trained in pain management. Acute Pain Service A service provided by the anaesthesiology department to manage post-operative and other forms of acute pain including labour pain; members of the service include doctors and nurses. ICU A facility within a hospital dedicated and equipped to provide specialised care to the critically ill patients; the unit is directed and staffed by medical and nursing personnel with special training in intensive care. Note: Where the HDU and ICU shares the same facility, then the facility is considered as an ICU only. Where the same facility caters for both adults and paediatric cases, then the facility is considered as an ICU only. For the purpose of this survey, paediatric (PICU) and neonatal ICU (NICU) are excluded. Level 2 Intensive Care Refers to care as per level 1 with the addition of advanced respiratory system monitoring/support e.g. invasive mechanical ventilation. 137 Appx 1 Refers to high dependency care which includes basic respiratory system monitoring/support e.g oxygen therapy and non-invasive ventilation OR cardiovascular system monitoring/support e.g vasoactive drugs List of Definitions Level 1 Intensive Care NATIONAL HEALTHCARE ESTABLISHMENTS & WORKFORCE STATISTICS 2010 HOSPITALS Level 3 Intensive Care Refers to care as per level 2 with the addition of any other organ system monitoring/support e.g renal/liver replacement therapy, invasive neurological monitoring, extra-corporeal respiratory support and invasive cardiovascular support. General ICU An ICU that admits both medical and surgical patients and caters to multiple disciplines. Organ Specific ICU An ICU that only admits patients of a specific discipline. e.g. post-cardiac surgical patient, neuro-surgical patients. Functional bed A bed which is readily available to take inpatient and provide the level of care according to the equipment and nurses available for that bed. Trained anaesthetic assistant Medical personnel, usually a nurse or medical assistant who is dedicated and trained to assist the anaesthesiologist in the provision of anaesthesia. Ventilators ICU (invasive ventilators) A ventilator that assist ventilation with the use of an endotracheal tube. Transport ventilator A ventilator used during transportation of critically ill patients. Non-invasive positive pressure ventilator Non-invasive positive ventilator (NIPPV) provides ventilatory support through the patient’s upper airway using a mask or similar device; patients on NIPPV do not require an endotracheal tube. High frequency ventilator A mechanical ventilator that employs very high respiratory rates and very small tidal volumes (usually below anatomical dead space) and is used to reduce ventilator-associated lung injury (VALI), especially in the context of ARDS and acute lung injury. Patient-controlled analgesia pump Patient-controlled analgesia (PCA) pump is a computerised pump that enables the patient to self-administer pain medications intravenously determined by the level of pain experienced; the PCA pump enables the patient to achieve a pain free state. CHAPTER ONCOLOGY Cytotoxic Drug Reconstitution (CDR) The process of preparing the cytotoxic drugs into a form ready for use. Cytotoxic safety cabinet The equipment used in the reconstitution of cytotoxic drugs that provide protection to personnel from the hazardous drugs and protection to the product as well. Cytotoxic drug reconstitution (CDR) isolator CDR isolator is considered alternative equipment to cytotoxic safety cabinet for the preparation of the cytotoxic drugs. 138 NATIONAL HEALTHCARE ESTABLISHMENTS & WORKFORCE STATISTICS 2010 HOSPITALS Radiation therapist An individual who is trained and registered to perform radiotherapy imaging, develop, evaluate and verify treatment plans; to deliver the planned and prescribed treatment using accurate and safe radiation therapy; and to monitor and assess radiation side effects, with the aim of cure and palliation of diseases treated with radiotherapy. Physicist An individual who is registered and trained as a medical physicist to conduct quality control of imaging and/or therapeutic radiation facilities and perform duties in nuclear medicine, radiotherapy, radiology or cyclotron facilities. Physics equipment All types of measuring tools used in testing the performance of radiation equipment and data commissioning. Examples: • Water-phantom/solid water • Ionization chambers, electrometer • QA devices (for linac, HDR and imaging systems) • Survey meter • In vivo dosimetry equipment Treatment planning system A treatment planning system that consists of a hardware package and a software package. It enables the input of patient data, the anatomy definition, beam set-up, the dose distribution calculation, the plan evaluation in terms of dose, volume, biological effect and the output for documentation and transfer to other units (simulator, record and verify system, treatment machine). Examples: • Full-capacity external beam/brachytherapyRT planning system • DICOM connections to receive images from the inhouse CT, linac’s CBCT, and from outside imaging centres, to directly transfer planning parameters to the record/ verification electronic charting system • Remote planning capability, allowing physicians, physicists and dosimetrists to review and participate in the planning process remotely. CHAPTER PSYCHIATRY Methadone clinic Outpatient programs that give opioid dependent individuals controlled access to methadone for the purpose of treating their dependence and/or for harm reduction. Consultation-liaison Service where the psychiatrist/psychiatric medical officer see patients currently admitted as general medical inpatients at the request of the treating medical or surgical team. Structured sessions conducted using modules that help to understand specific mental illness and develop coping skills needed to avoid relapses. Family link sessions (8 modules) Appx 1 Carers, relatives or volunteers who attend structured sessions using modules to assist in better understanding on managing patients with mental disorder.
