ISO 9001:2008 accredited 501(c)(3) nonprofit
consumer credit counseling agency. FCF helps
financially-distressed individuals and couples move
from crisis to control by offering personal
confidential financial counseling, educational
initiatives promoting financial literacy, debt
management services, housing counseling services
and asset-building programs. FCF collaborates with
diverse partners at the national, state and local level

Purpose:
The mission of Mountainlands Family Health Center
(MCHC) is health professionals assuring the
provision of high quality primary health care for
everyone in our community - especially those who
experience economic, geographic, cultural and
language barriers to receiving care - regardless of
their ability to pay. Mountainlands Community
Health Center strives to eliminate all barriers to
receiving appropriate, quality health care, dental
care and mental health care. Our doors are open to
anyone with qualifying individuals and families
receiving discounted services based on their family
size and income. Eighty-five percent of our staff is
Spanish-English bilingual, and we offer culturally
appropriate health services. Mountainlands offers
case management for diabetic patients, patients who

(UPP)
UPP helps working individuals and families pay their monthly
health insurance premiums. If an employee's company offers health
insurance, qualified individuals and families can receive monthly
reimbursements for the cost of their employer-sponsored health
insurance coverage. Services: If qualified, UPP will pay up to $150
per adult every month and up to $100 per child every month. UPP
offers options for children's dental coverage.
