It offers relatively rapid access to cardiac arrhythmia specialists using a shared model for patient care. The nurses work under direct supervision of the clinic physicians, who are responsible for patient care within the AF clinic. The AF clinic is not a general cardiology clinic. The directory is a searchable, centralized, web-enabled tool allowing referral requestors (e.g. family physicians, referral coordinators, medical office assistants) to access referral information for consultants (including specialists and family physicians who accept consultations for areas of interest or expertise). Healthcare providers can access the directory in a variety of ways: o Netcare on the ‘Resources’ section o AHS Insite website on the ‘Employee Tools’ section o External website address www.albertareferraldirectory.ca. This site requires a log in using a physician’s CPSA number, first and last name to ensure the directory information is secure from public view. This free website is secure and searchable. It is a streamlined, efficient way of accessing consultant referral information to assist in getting patients to the right provider at the right time. Consultants create profiles on the directory which informs primary care providers of the consultant’s area of interest, what documentation and tests should be done prior to the consultation as well as an approximate time to routine referral appointment. Through automated email reminders to consultants entered in the ARD and with the support of ARD staff, content will be regularly reviewed and updated in order to stay current and accurate.
