If you need help, the following options may be possible subject to our approval. Not all borrowers will qualify for the listed
options. We cannot guarantee that you will receive any or a particular type of assistance. Options may include refinancing

appraisal. We will promptly give you a copy of any appraisal, even if your application does
not close. You can pay for an additional appraisal for your own use at your own cost.

Financial Analysis Packet Instructions
GET STARTED – use this checklist to ensure you have completed all required forms and have the right information.
Step 1
Complete, sign, and date the enclosed Financial Analysis Form and be sure to follow the detailed
instructions. All parties on the mortgage must submit their financial information to determine if assistance is
available.
Step 2
Provide required documentation including income documentation. This documentation will be used to verify
your hardship. (Notice: Alimony, child support or separate maintenance income need not be revealed
if you do not choose to have it considered for repaying this loan).
1. Provide copies of the two most recent monthly statements for the following:
 Your bank statements,
 Your monthly mortgage or home equity loan(s), and condominium dues (if applicable)
2. Provide a copy of your most recent annual property tax bill;
3. Provide copies of your federal tax returns together with all schedules for the past 2 years;
4. Provide copies of the two most recent pay stubs or other income verification for all parties on the
Mortgage, if applicable. If self-employed provide a three month profit and loss statement for the
most recent quarter;
5. Provide evidence of hazard and flood (if applicable) insurance coverage and amount of annual
premium(s);
6. For short sale applications include a copy of the purchase and sale agreement, listing agreement, the
proposed HUD settlement statement, and the short sale affidavit.
Step 3
Complete and sign a dated copy of the enclosed IRS Form 4506-T Request for Transcript of Tax Return
 For each borrower, please submit a signed, dated copy of IRS Form 4506T-EZ (Short Form
Request for Individual Tax Return Transcript)
 Borrowers who filed their tax returns jointly may send in one IRS Form 4506T-EZ signed and dated
by both joint filers
Additional documentation of income or assets not reflected on tax returns may be requested and/or if the
borrower(s) have requested tax filing extension(s).

Gather the completed, signed, and dated Financial Analysis Form along with required documentation and
mail to:

Section I. Detailed Hardship Questionnaire
 Answer all questions for both the borrower and co-borrower. Please explain the reason for the delinquency and why
you are requesting assistance. Be as specific as possible in describing any hardship event(s) affecting your financial
situation.
Section II. Monthly Income Data
 Enter the amount of income in separate columns for each mortgagor. Proper documentation to support the income is
required.
 Include any other income received which does not fit into any of the listed categories under "Other - Specify" and
indicate the source of that income.
 Note: copies of your last two Federal tax returns with W-2(s) and all schedules must be submitted with the
Financial Analysis.
 Under the "Less Taxes" section, report the taxes withheld or paid. Include any other deductions withheld from your
paycheck under "Other Deductions".
 Note: Please submit copies of the two most recent check stubs or other income verification for each wage earner.
 Subtract deductions from income to arrive at net income for each column. Enter the total under "Net Income".
Section III. Assets/Liabilities
 Record assets owned under the appropriate category. Determine the current estimated value. If a balance is owed
on the asset, enter that amount under "Amount Owed". Subtract the amount owed from the estimated value to arrive
at the net value of each asset.
 Note: You must submit copies of recent bank statements, IRA/Keough or 401K/ESOP retirement accounts, etc.

Section IV. Expenses
 Record your monthly expenses in the appropriate area. Enter the remaining amount owed under "Balance Due" for
such items as mortgage, credit cards, and loans, etc.
 Add all expenses under the "Monthly Payment" column and enter the sum in the "Total" space.
 Enclose copies of recent bills to substantiate monthly expenses.

 Calculate the difference between total income and total expenses.
