The Alliance’s mission is to promote mental well-being, prevent mental illness, advocate for the
rights of and provide services to persons who are mentally ill, and persons who are mentally
retarded.
Program Name: Advocacy for Adults & Children

Advocates serve as spokespersons and problem-solvers for persons in the community with mental
illness in various life situations such as mental health services, housing, and employment.
Advocates work on behalf of children and adolescents with mental illness in asserting and
protecting their rights in the mental health and other various child-serving systems of care.

Advocacy Alliance offers a safe, supportive environment for persons in the community with
mental illness or emotional problems to come together.

“The Doorway,” consumer driven drop-in centers, provides an opportunity for persons receiving
mental health services to come together in an atmosphere of acceptance and mental support.
Program Name: Financial Management
The representative payee service is a comprehensive system of financial and budgetary
management for persons who are mentally ill, mentally retarded, or elderly who are unable to
manage their monthly Social Security benefits and financial affairs.
Program Name: Community Education
The Alliance promotes public awareness of mental health/mental retardation issues, problems, and
service/treatment. The Alliance advocates on behalf of the consumers by keeping local, state, and
federal elected officials informed of mental health/mental retardation issues and legislation.
Program Name: Direct Client Services
Assisting mentally ill clients with basic life sustaining needs, such as food, clothing, and shelter for
persons in the communities. Advocacy Alliance also helps organize and sponsor self-help groups
for persons in the community coping with specific aspects of mental illness.
Program Name: Health Care Quality Units
The Alliance also serves as facilitator of the Health Care Quality Units (HCQUs), the responsible
entities to the County Mental Health/Mental Retardation Programs for monitoring the overall
health status of individuals with mental retardation who are receiving services. The HCQUs work
to support and improve the mental retardation community service systems by building capacity and
competency to meet the physical and behavioral health care needs of persons with mental
retardation.
Program Name: Consumer Satisfaction Teams
To facilitate teams of mental health services consumers and their families whose expressed
purpose is to visit both adult and children consumers and assess their level of satisfaction with the
mental health services they receive, to inquire as to their wants and needs, and to learn what they
think would help in the delivery of services to them as individuals.
Program Name: Independent Monitoring Teams

Facilitate teams of consumers of mental retardation services, their family members, and
community volunteers who are dedicated to the continuous improvement of the quality of service
and support for persons with mental retardation. The teams conduct surveys of consumers,
facilitate self-advocacy groups, speak with consumers and family members to determine their
levels of satisfaction with services, and educate the community.

skills could include budgeting, preparing meals, shopping, using public transportation or job
hunting.

An ARCIL peer helper is a person with a disability who is living independently and is able to share
their experiences and offer their support to others. They work one on one with individuals, helping
them make adjustments to live with their disability, learn problem solving skills, explore options
and use community services more effectively.

Wheelchair accessible transportation services are available on a limited, first come first serve basis
for special trips that can not be met by local transportation services, i.e. crossing county lines,

Avenues is a private non-profit corporation that serves Schuylkill, Carbon and Northumberland
counties. The purpose of this corporation is to promote the general welfare of persons, with

Program Name: Celebrate Recovery
Celebrate Recovery is a recovery program based on the eight principles from the Beatitudes and
the twelve steps and their biblical comparisons. Celebrate Recovery® addresses all types of
habits, hurts and hang-ups. Some recovery programs deal only with alcohol or drugs or another
single problem. But Celebrate Recovery® is a “large umbrella” program under which a limitless
number of issues can be dealt with such as overspending, anger, overeating, need to control,
fear/anxiety and more!
The purpose of Celebrate Recovery® is to encourage fellowship and to celebrate God’s healing
power in our lives as we work our way along the road to recovery. We are changed as we share our
experiences, strengths, and hopes with one another. In addition, we become willing to accept
God’s grace and forgiveness in solving our life’s problems.
Meetings will be held every Friday from 6:30 pm to 8:30 pm in the Library at the Church.

Program Name: GriefShare
It may be hard for you to feel optimistic about the future right now. If you’ve lost a spouse, child,
family member or friend, you’ve probably found there are not many people who understand the
deep hurt you feel. This can be a confusing time when you feel isolated and have many questions
about things you’ve never faced before.
GriefShare groups meet bi-weekly to help you face these challenges and move toward rebuilding
your life. Each GriefShare session has three distinct elements: Video Seminar with Experts,
Support Group Discussion with Focus, and Workbook-based Personal Study and Reflection.

The Pennsylvania Department of Public Welfare through its Child Care Information Services
Agencies, administers the Subsidized Child Day Care Program for low-income families and makes
resource and referral services available to all citizens of Pennsylvania.
Child Care Information Services Agencies are dedicated to provide all services courteously, to
help families receive all of the services for which they are eligible, and to help families access all
needed resources in a community.

The Schuylkill County Child Care Information Service is the funding agency for subsidized
childcare in Schuylkill County. CCIS will provide information about childcare in the county, act
as a resource to childcare providers, and can also make available a listing of childcare, both
licensed and registered. All of these services can be accessed through, CCMIS, a web based
referral service, which can help families find appropriate, affordable childcare. Information and
referral services are available regardless of family income.
The Child Care Information Service provides assistance with childcare costs to eligible families
residing in Schuylkill County. This subsidy for childcare cost is available to families currently
using state licensed or registered providers. Assistance is also available to families using
relative/neighbor care. This is care provided in an unlicensed facility (someone’s home) for not
more than three (3) children.
If you are using a licensed setting for child care, or your friend, neighbor or relative is caring for
your child, you may be eligible for help with child care costs. A minimum number if working
and/or training hours must be met to be eligible for help with these costs. Certain special
circumstances are allowable under the Child Care regulations (e.g. disabled spouse, shared
custody, foster parent, Head Start and others); please contact the CCIS directly to see if you
qualify.
Schuylkill County Child Care Information Services also administers a small amount of funds,
which are “set aside” to provide care for the children of teenaged parents, so they can continue to
attend school.

This program is designed to meet the needs of parents who are working or are in training. There
are three (3) state-licensed centers located throughout the county which provide service Monday

through Friday. This twelve (12) month program is open to children six (6) weeks old to
kindergarten age, including children with disabilities.

This program is designed to meet the needs of children age three (3) to five (5), including children
with disabilities and families who meet the income guidelines.
Locations throughout the county that provide service Monday through Friday, in a nine (9) month
program (September to May) for a minimum of four and one half (4.5) hours per day.
Center-Based – Full Year
State-licensed centers located throughout the county provide service Monday through Friday, in a
twelve (12) month program for a minimum of four and one half (4.5) hours per day, for eligible
children whose parents are working or are enrolled in a training program.
Home-Based
Home visitors throughout the county provide service Monday through Friday in a nine (9) month
program (September through May), by one and one half (1.5) hour weekly home visits and biweekly group socializations.

month, providing transportation for seniors to medical and/or critical social service appointments,
clerical and providing health insurance counseling to people with Medicare.

An interfaith outreach ministry offering assistance to seniors of all faiths through the support of
volunteers. DVHC assists seniors who are temporarily or intermittently in need of help in their
home. Services Include: companionship, telephone reassurance, transportation to medical or

are free and donations are gratefully accepted.

Meals on Wheels delivers five (5) noon-day meals per week to homebound clients helping them to
remain independent and in their own homes. Deliveries are made on Mondays, Wednesdays and
Fridays. Volunteers make re-assurance calls on Tuesdays and Thursdays. Meals on Wheels is
funded in part by the Office of Senior Services.

Gaudenzia at Fountain Springs is a residential drug and alcohol treatment program for
pregnant/parenting women and their children. Treatment, based on the therapeutic community
model, is done in a homelike environment with mothers as the principal caregivers for their
children. Fountain Springs provides unique services to pregnant/parenting women and their
children through a combination of diverse therapeutic activities and supportive services. Women
who enter treatment may bring two (2) children with them. Their stay at Fountain Springs is from

The Literacy Council for Schuylkill County is a state-funded program that provides one-on-one

The purpose of the Make-A-Wish Foundation is to grant the wish of each child who has
reached the age of two and one half (2 ½) and is under the age of eighteen (18) who has a
life-threatening medical condition, i.e., a progressive, degenerative or malignant
condition that has placed the child’s life in jeopardy.
This service provides a wish to children ages two and one half (2 ½) to eighteen (18) who suffer a

The Schuylkill County Office of Senior Services is a public social service agency which involves
itself with all issues concerning persons sixty (60) and older who are residents of Schuylkill
County. The agency carries out a wide range of functions related to advocacy, planning,
coordination, interagency linkages, information sharing, monitoring, service provision and
evaluation. These activities are designed to promote the development and enhancement of
comprehensive and coordinated community-based geriatric social service systems in/or serving

care. It is called a “Waiver” program because services are funded through a special waiver of
certain Medicaid restrictions, allowing payments typically used for nursing home care to be used
for home-care services.

Evergreen is contracted through The Office of Senior Services and is licensed by the
Commonwealth of Pennsylvania Department of Aging.
The mission of Evergreen Senior Services Adult Day Centers is to deliver a planned program of
therapeutic and recreational activities and the opportunity for socialization in a supervised, safe yet
home-like setting to an impaired older adult population. The centers strive to keep participants
functioning at their optimal level and to keep them in their own homes for as long as possible.
Services include activities, personal care, social services, health monitoring, nutrition, and
emergency care.
The target population is an adult 60 years of age or older who is mildly to moderately physically
and/or mentally disabled and in need of activity, socialization and/or health monitoring. Adults 18
to 59 years of age may be eligible for services if they meet the criteria for admission into an adult
day program. Funding is available through various resources. Evaluations can be done at the
center during a free visit or at an individual’s home.

Mortgage (HECM) Counseling

HECM is a loan program that works in the “reverse” of a typical mortgage. Instead of paying a
monthly payment you will “receive” a monthly payment or lump sum from the equity in your
home. A reverse mortgage is a loan against your home that requires no repayment for as long as
you live there. Applicant must show that the home is the principal residence, be sixty-two (62)
years of age or older and attend a counseling session at Schuylkill Community Action.
Program Name: Rent and Mortgage Assistance
Schuylkill Community Action can provide rent or mortgage assistance to individuals or families
facing a hardship and/or eviction. Funds are distributed when available. The applicant can receive
the service once per year, must meet certain income guidelines, and agree to budget counseling at
SCA.
Program Name: Homeless Assistance
Short-term housing as well as case management services is provided to individuals and/or families
who are homeless or residing in unfit residences when funds are available. Service may be one (1)
week in a boarding house or possibly two (2) nights in a hotel.
Program Name: Homeowners Emergency
Mortgage Assistance Program (HEMAP)

Offers emergency mortgage assistance to homeowners facing foreclosure. Applicant must have
received an Act 91 notice (legal notice of “Intent to Foreclose”) from their mortgage holder.
Provides short-term temporary funding to cure loan delinquencies. Makes mortgage payments to
lenders on behalf of qualifying homeowners for up to twenty-four (24) months. Recipients must
meet eligibility guidelines defined by state law. Repayment of assistance is required.
Program Name: Weatherization

This program works to stop infiltration of cold air through homes and apartment buildings in an
effort to help cut down the cost of heating the home or apartment building. Must be a Schuylkill
County resident, fall within the income guidelines (60% of the area median income) and be a

homeowner or renter. If the home is rented, the landlord must sign an agreement before any work
is started.
Program Name: Volunteer Income Tax Assistance Program
(VITA)

SCA provides assistance to low-to moderate income families of Schuylkill County, which qualify
for the Earned Income Tax Credit (EITC), with their state and federal tax returns through
electronic filing free of charge. Partnered with the IRS, SCA is regulated by income guideline and
household living status for this program. The VITA Program runs from the end of January to
April 15, of the tax year.

Housing Rehabilitation Program

Provides housing assistance grants to very-low income households where a child six (6) years of
age and younger resides. Housing rehabilitation includes the elimination of health, safety and code
violations; accessibility and VisitAbility modifications: and lead-based paint remediation.
Application must be owner and occupant of the property to be assisted, fall within Program income
guidelines, has a child under the age six (6) residing (or spending a significant amount of time) in
the assisted property and provide proof of current real estate tax status and homeowner insurance
coverage.

participants in the home buying process. Topics covered at the two (2)-hour sessions include

A grant program that provides housing repair assistance (major systems) to person(s) over sixtytwo (62) years of age. This program also provides grants to person(s) with disabilities to make
modifications to their home which will increase their accessibility.
Major system
repair/replacement is limited to the following systems: roof/drainage, heating, electrical,
plumbing/fixtures, and fire/smoke protection. “Accessibility modifications” include elimination of
barriers and improvements to the home that provide greater accessibility and “visitability.”
To be eligible for assistance, the applicant must be a Schuylkill County resident, must own and
reside in his/her home, must have current status with real estate taxes, must have homeowners
insurance, and must meet income eligibility requirements.
Program Name: Keystone and Renovate and Repair Program

Schuylkill Community Action is the Local Program Administrator for the Keystone Renovate and
Repair Loan Program (R&R) offered by the Pennsylvania Housing Finance Agency (PHFA). The
purpose of the R&R Program is to fill a gap in the home repair lending market and to help prevent
homeowners from becoming victims of predatory lending practices. The R&R loan provides an
attractive interest rate and also provides the homeowner with assistance regarding repairs and/ or
improvements to be completed. The Program is intended to finance renovations to improve the
basic livability of the home, as well as repairs to items affecting health, energy efficiency,
accessibility, and code violations.

Program Name: Road to Economic Self-Sufficiency
through Employment and Training (RESET)

RESET is designed to enable recipients of Cash Assistance, within the constraints of available
funds, to secure permanent full-time jobs that can establish a work history with wages and benefits
that lead to economic independence and self-sufficiency. As a condition of eligibility for cash
assistance every individual who is not exempt must seek employment, accept any bona fide offer
of employment, and maintain employment and earnings. Those who do not comply with RESET
without good cause may be sanctioned.
Special Allowances for supportive services are available to clients who are enrolled in RESET,
have an approved Agreement of Mutual Responsibility Form, and are actively
participating in an approved activity in order to enable them to accept or continue employment.
Special Allowances may be authorized for items such as child care, transportation, automobilerelated expenses, tools, equipment, books and supplies as well as test fees and registration fees.
Limited allowances are also provided to SNAP recipients under certain circumstances.

Through the CHIPP project Schuylkill County Mental Health/Mental Retardation program
together providers, consumers, families and staff will build the most effective community service
system for the most seriously ill mentally ill adults residing in or returning in Schuylkill County.
The system will depend upon existing services and implement new services addressing behavioral
and treatment issues. Housing and community supports will be specialized and tailored to meet the
varied needs of all individuals currently in the state hospital system as well as the individuals who
are at risk of requiring long-term outpatient treatment because of the lack of adequate and

Schuylkill Medical Center - East offers assistance with payment for medically necessary, nonelective, hospital services (both inpatient and outpatient) for patients who are uninsured or underinsured. Uninsured or under-insured patients who wish to participate in our Community Care

Speak to our financial counselor regarding the criteria necessary to apply for Medical
Assistance.
Applications must provide proof of income. This can be accomplished by providing a copy
for the patient’s last W-2, tax return, last two pay stubs, or bank statement reflecting the
most recent two deposits. Eligibility will be based on household income and family size.
Please note that Schuylkill Medical Center – East’s Community Care Program applies to the
hospital component of a patient’s medical bills, and does not apply to any bills from a physician’s

The Senior Behavioral Health Inpatient Unit at the Schuylkill Medical Center - East provides
inpatient psychiatric services exclusively to patients aged 62 and older. We treat a wide range of

multi-disciplinary care provided by trained and experienced Nurses, Social Workers, Activity

STS WELCOMES YOU ABOARD
The Schuylkill Transportation System offers shared ride van services under several funded
programs that are available to many Schuylkill County residents. The most commonly known
program is the Shared Ride Program for Senior Citizens, which is sponsored by the PennDOT
Lottery Program and subsidized by the Schuylkill County Office of Senior Services. All senior
citizens who are 65 (sixty-five) years of age or older regardless of income are eligible to use this
service. For a small co-pay of one dollar ($1.00) per one- way trip seniors can travel to the
following destinations: any type of medical appointments, social services agencies, local grocery
shopping, senior resource centers, and adult day care centers. Transportation to other destinations
such as the hairdresser, barbershop, retail shops, malls and restaurants cost two dollars and ten
cents ($2.10) to two dollars and twenty-five cents ($2.25) per one-way trip is collected upon
boarding the van.
Another shared ride program includes the Rural Transportation for Persons with Disabilities
Program. This program, which is also sponsored by PennDOT, provides shared ride can serve to
any disabled resident of Schuylkill County between the ages of eighteen (18) and sixty-four (64)
regardless of income. A minimal co-pay of two dollars and ten cents ($2.10) to two dollars and
twenty-five cents ($2.25) per one-way trip is collected upon boarding the van.

Any Schuylkill County resident who is a medical assistance ACCESS cardholder may be eligible

In addition to the shared ride van programs that are available in Schuylkill County, STS also offers
fixed route bus services. Fixed route bus services have been provided for over twenty-five (25)

(65) years of age or older ride FREE. All other individuals can ride the STS fixed route bus
system by paying a fare of one dollar and twenty-five cents ($1.25) to three dollars ($3.00) to get

To advance the economic security of the nation’s people through compassionate and vigilant

The following programs are administered by Social Security (SSA):
Retirement, Disability, Survivor, Supplemental Security Income (SSI) and Medicare. In addition
to the above, Social Security will accept Black Lung Claims for the Department of Labor as well
as process post-entitlement actions for them. Social Security Numbers are obtained through SSA,
both the original number as well as replacement cards. For disability benefits, an individual must
have a condition that prevents him/her from performing Substantial Gainful Activity and that
condition must be expected to last at least twelve (12) months. SSI applicants must be either at
least sixty-five (65) years of age or disabled. Retirement age is sixty-two (62) for early retirement.
Age sixty-five (65) is the earliest age for full retirement benefits however people born after 1938
will have their full retirement age gradually increased. Benefits are also provided to the families of
workers who die.
