If you are interested in applying to the LFCC
Practical Nursing program, you MUST complete the
college application to LFCC as well as the program
application.
The college application must be
submitted on-line, but your program application
must be completed manually and sent to Health
Professions Office, Room 107 in the Science and
Health Professions Building at 173 Skirmisher Lane,
Middletown, VA 22645.
Applications are not
accepted at the Fauquier Campus and will be
returned to the applicant.

Select Lord Fairfax Community College as the institution you
plan to attend and your plan of study as Pre-Practical Nursing.
Please indicate the semester you plan to start as Summer 2011
(or earlier if you plan or will need to enroll in classes at LFCC
prior to entry into the summer program). If you encounter any
difficulty submitting your information electronically, please call
the LFCC Admissions office for assistance at 540.868.7105.
If you are unable to download the Nursing application and need
a paper copy, please call the Health Professions Office at
540.868.7075 to request one.

As LFCC has two campus sites, Middletown and Fauquier, your
advisor will be made known to you when and if you are accepted
into the nursing program. The administrative assistant can be

WHEN SHOULD I APPLY? HOW DOES IT BENEFIT ME TO APPLY EARLY?
Applications for the 2011 class are accepted beginning February 1, 2011 and must be completed by no later than March 31, 2011 at 4 p.m. Current enrollment in
missing prerequisites is not required to apply, but must be completed by the end of Spring 2011 to be eligible for the 2011 program.
By applying now to LFCC and submitting official transcripts of high school and college coursework, we will be able to provide guidance and answer some of your
questions as you prepare for this major plan of study. It is necessary to review your transcripts/academic file to determine appropriate placement testing, do
transfer evaluation (if applicable), and suggest appropriate future courses. After your file is reviewed you are encouraged to meet with the advisor each semester
prior to open registration to review course selection and your progress. We can also keep you informed of any changes to the curriculum or transition policies.
WHAT DO I NEED TO DO TO APPLY?
We require all nursing application material(s) be submitted directly to the Office of Health Professions - if you fail to do so, be aware we will not be
responsible for any delays in processing your file. Complete the following and submit (if applicable) in one envelope to Office of Health Professions,
LFCC, 173 Skirmisher Lane, Middletown, VA 22645. If applying for acceptance to the nursing programs at the Fauquier campus, you must submit your
materials to the Middletown address. Materials sent to the Fauquier campus will not be accepted and will be returned to you.
1. Apply to LFCC by completing the college application, if not currently attending. You must complete the application on-line at https://apply.vccs.edu .
Please make sure you indicate your program/plan of study as Pre-Practical Nursing on the LFCC application and your term/semester to begin no later
than Summer ’11 (or earlier if you take classes sooner). If you have transcripts from another college/s that you would like evaluated, please submit
original transcripts to Admissions and Records and complete the “Request to Evaluate a Transcript” form.
2. Apply to the Practical Nursing Program by completing a 2011 Nursing Application form. An application is enclosed with this packet or you can pick
up an application at the Health Professions Office, Room, 107 in the Science and Health Professions Building or you can request one be mailed to you by
calling 540.868.7075. If you have MS Word you may access, fill out, and print the form from http://www.lfcc.edu/areas-of-study/areas-of-study-a-toz/health-professions-997/nursing/index.html, but you must submit it by mail or in person.
3. Include with your nursing application, all official sealed transcripts (high school and college) and a copy of GED scores (if applicable). Applicants
should submit all sealed transcripts with the application in an envelope addressed to Office of Health Professions. (When possible applicants should
request official transcripts from high school and all colleges attended/attempted to be mailed to your home address. Upon receipt of transcripts, do not
open the sealed official envelope(s) as they are no longer “official” to us when opened. Put all transcripts with your nursing application into one
large envelope and mail or bring to Office of Health Professions, Science and Health Professions Building, Room 107, LFCC, 173 Skirmisher
Lane, Middletown, VA 22645. The office is usually open Monday thru Thursday from 8 a.m. to 4 p.m. GED holders must submit a copy of their GED
scores. GED holders must also submit official high school transcripts if they completed at least through the 9th grade of high school. Transcripts must
be submitted from ALL colleges attended (except LFCC or any other Virginia Community College). If any school refuses to issue an official
transcript directly to student, please have it sent directly to Office of Health Professions at the address above. Home-schooled students must provide a
detailed curriculum progress/grade report from their school administrator.
4. Please note: The required GPA is calculated as of the end of the Fall 2010 semester. All applicants who attempt or complete college or high school
courses after their initial academic file is complete must submit updated transcripts through Fall ’10 prior to the March 31, 2011 deadline. High school
seniors must provide an updated HS transcript for the grading period that ends usually in late January (must be postmarked) before March 31st deadline.
All updated transcripts must be sent directly to the Health Professions Office at the address above.
WHAT ARE THE MINIMUM HIGH SCHOOL PREREQUISITES FOR ADMISSION TO THE PROGRAM?

Evidence of a high school diploma or GED with a grade point average of 2.5 or higher. High school graduates and GED holders who earned less than a
2.5 GPA during high school will be considered for admission if they have generated a college GPA of 2.5 or above based on at least 8 semester college credit
hours within a twelve-month period at one institution (developmental courses are not used in GPA calculations).
Appropriate developmental or college courses can be substituted for high school deficiencies. Must be completed by the end of Spring semester 2011, to be
eligible for the 2011 program.
WILL I NEED TO SUBMIT ANY OTHER MATERIALS OR TAKE ANY TESTS?
You will be required to take the TEAS test, available at the Learning Center and include the test results in your packet. Testing is available
November 8, 2010 until March 18, 2011. Contact the Learning Center for further information at 540.868.7088.
IS ADMISSION TO THE PROGRAM VERY COMPETITIVE? ARE THERE ADMISSION PRIORITIES?
Priority is given to applicants who have a GPA of 2.5 or higher and have either completed all high school prerequisites or anticipate completion of the missing
prerequisites by the end of Spring 2011 and who live in the LFCC service area.
WHO MAKES THE ADMISSION DECISIONS? WHEN ARE THE RESULTS GIVEN TO THE APPLICANTS?
Admission decisions are made by the Selection Committee, which meets mid-April.

Some applicants may need developmental courses for math or English prior to prerequisites or support courses, which can be determined by reviewing your
placement testing results with a counselor. A review of your transcripts after application will assist the advisor in determining if you lack the prerequisites .If you
need to establish a 2.5 GPA, please speak to the advisor to determine appropriate coursework prior to the fall semester. Some students prefer to take the required
general education courses such as English 111 before beginning the program, but it is not treated as a prerequisite.
CAN I BE A LPN IF I AM A CONVICTED FELON?
In the state of Virginia, if someone has been convicted of a felony or a misdemeanor they may not be allowed to take the LPN licensing exam. The question of
eligibility to take the LPN licensing exam cannot be determined until application for licensure is received by the State Board of Nursing. LFCC has no control
over whether or not the Board of Nursing in Virginia or other states will allow the student to take the NCLEX exam. If you wish to discuss this issue, you may call
the LPN Program Head at 540. 868.7076.
HOW DO I OBTAIN LICENSURE AS A LICENSED PRACTICAL NURSE?
You must successfully complete an approved Practical Nursing education program and pass a licensure exam, which is administered by the National Council of the
State Board of Nursing. You must apply to the regulatory board of the state in which you plan to practice. The general requirements include personal, character,
and criminal background information. Application for the licensure examination will be provided toward the end of the third semester in the Practical Nursing
program.
DOES LFCC HAVE A TRANSITION PROGRAM FOR LPN’S WHO WANT TO BECOME RN’S? WHAT WOULD BE REQUIRED?
Yes. You may request the informational application packet on LPN to RN Transition. The process is as follows: 1) complete a LFCC college application, 2)
complete the LPN to RN application, and 3) have all high school, GED scores (if applicable), and college transcripts sent to the Office of Health Professions
including your practical nursing school transcripts. You must take a STEP test, available at the Learning center. A copy of current LPN license is needed. The
application deadline is March 31, 2011 at 4 p.m. If you need more information, please call the Nursing office 540.868.7075. Meeting the curriculum admission
standards does not guarantee program admission.
PRACTICAL NURSING CHECKLIST
The following is intended as a general guideline for making sure you have taken the necessary steps to apply and be
considered for the Practical Nursing program. It is the applicant’s responsibility to insure that all items arrive prior
to the March 31, 2011, 4 p.m. deadline. Late application materials will not be considered and will make the
applicant ineligible to be considered for the 2011 program. Please understand an applicant may have unique
circumstances, which may require further documentation or verification, in which case the applicant may be asked to
furnish material not mentioned below. Every effort will be made to provide clear instructions to applicants needing to
provide such information. Please keep this checklist to refer to – you do NOT need to return it to us.

Official High School transcript (Required for all applicants, including those with previous
college degrees.) All transcripts must be sent to the Health Professions Office prior to March
31, 2011 at 4 p.m. (it must be in the original sealed envelope from the school that issues it,
please do not open it).
