Occupational Objective: The graduate should be able to acquire the mandatory skills in creating a professional portfolio as well as organizing and working in conjunction with various professionals (photographers, models, hair stylists etc.). Price: $800.00 (eight hundred US dollars). Occupational Objective: The graduate should be able to acquire Professional level skills in application of makeup artistry skills on male models; how to groom and style male models as well as proper interaction techniques. Educational Services: 7 Makeup and Fashion Couture Institute offers fully equipped facilities and classroom settings for students to study and practice their skills. A professional makeup kit is provided at the beginning of the program; it is included in our Platinum Package and is available for additional cost for all other students and graduates. 8 6 B 9 g 9 Entrance Requirements The school does not discriminate based on race, sex, religion, ethnic origin, or disability. Prospective students must provide government issued photo identification and/or Alien Identification Number (AIN) if applicable, a verifiable address and guardian information (when applicable) in order to be accepted for enrollment. Signed contract is mandatory prior to beginning of classes. The School does not administer an entry test, but will conduct a preliminary acceptance interview. Students must at least 16 years of age. No High School Diploma required. Enrollment Prospective students may enroll anytime. Late enrollments will be accepted only one week into the course; the complete program length is 160 hours, not to exceed 240 hours. Makeup and Fashion Couture Institute LLC Catalog 6 MFCI 10 Rev07.30.13 Placement Assistance Makeup and Fashion Couture Institute LLC does not offer employment assistance to graduates at this time. While assisting in your job search, we make no guarantee, expressed or implied, of future employment. Current law prohibits any school from guaranteeing job placement as an inducement to enroll students. 11 Payment Schedules The following payment schedules are available for all students: $200 Non-refundable Administrative fee upon signing of the contract; All tuitions must be paid prior to the beginning of classes; no payment plans are available. 12 Cancellation and Refund Policy: I. An applicant denied by The School for is entitled to a refund of all monies paid. Three-day Cancellation: An applicant who provides written notice of cancellation within three days (excluding Saturday, Sunday and federal and state holidays) of signing an enrolment agreement is entitled to a refund of all monies paid no later than 30 days of receiving the notice of cancellation, the School shall provide the 100% refund. Other cancellations: An applicant requesting cancellation more than three days after signing an enrollment agreement and making an initial payment, but prior to entering the School, is entitled to a refund of all monies paid, less the registration fee of $200. Refund after the commencement of classes: 1. Procedure for withdrawal/withdrawal date: A. A student choosing to withdraw from the School after the commencement of classes is to provide written notice to the Director of the School. The notice is to indicate the expected last date of attendance and to be signed and dated by the student. B. For a student who is on authorized Leave of Absence (LOA), the withdraw date is the date the student was scheduled to return from the Leave and failed to do so. C. A student will be determined to be withdrawn from the School if the student has not attended any class for 3 days. D. All refunds will be issued within 30 days of the determination of the withdrawal date. Makeup and Fashion Couture Institute LLC Catalog 7 MFCI Rev07.30.13 2. Tuition charges/refunds: A. Before the beginning of classes, the student is entitled to refund of 100% of the tuition (less the registration fee of $200) B. After the commencement of classes, the tuition refund (less the registration fee of $200) amount shall be determined as follows: % of the clock hours attempted: 10% or less More than 10% More than 20% More than 30% More than 40% More than 50% and and and and less less less less than than than than or or or or equal equal equal equal to to to to 20% 30% 40% 50% Tuition refund amount: 90% 80% 70% 60% 50% No Refund is required The percentage of the clock hours attempted is determined by dividing the total number of clock hours elapsed from the student’s last day of attendance by the total number of clock hours in the program. Makeup kit and fees: Once the School provided the student with the Professional Makeup Kit, the student may return that kit only in an unused and unpackaged condition to the School within 5 days following the date of the student’s notice of cancellation. Failure to do so will result in applied charges in the amount of $1000 (the cost of the Professional Makeup Kit issued by the School). Refund: refunds will issued within 30 days of the date of student notification, or the date of the School’s determination (withdrawn due to absences or other criteria as specified in the MFCI catalog), or in the case of a student not returning from the authorized Leave of Absence (LOA) within 30 days of the date the student was scheduled to return from the LOA Books and supplies: 13 14 The professional makeup kit is required and provided to each student which already contains all the necessary ingredients in order for a student to study and work while attending the program. Additional makeup supplies will be available for purchase. Student Services: To be determined. Makeup and Fashion Couture Institute LLC Catalog 8 MFCI 15 Rev07.30.13 Academic Policies: Attendance Requirements Students are expected to arrive on time for class with proper materials. An overall attendance rate of at least 90% is required. Instructors may request your withdrawal from a course or program if absences or tardiness exceed 3 or more occasions. Students who are unable to continue classes for medical reasons or severe personal problems will be required to take a Leave of Absence (LOA) until they are able to return to class. Proper documentation will be required to substantiate a student’s withdrawal. Postponement of Start Date Postponement of a starting date, whether at the request of the school or the student, requires a written agreement signed by the student and the school. The agreement must set forth: a. Whether the postponement is for the convenience of the school or the student, and; b. A deadline for the new start date, beyond which the start date will not be postponed. If the course is not commenced, or the student fails to attend by the new start date set forth in the agreement, the student will be entitled to an appropriate refund of prepaid tuition and fees within 30 days of the deadline of the new start date set forth in the agreement. Satisfactory Academic Progress: 16 17 As determined by the institution: Students must maintain an 85% Grade Point Average (GPA). Those who will not will be placed on probation for 3 classes. During the probation period students must raise their GPA to passing or higher. The student may be terminated if grades are not satisfactory at the end of the probationary period. Termination shall be at the school Director’s discretion. The Director has final authority and shall notify the student of the final decision. Grading System 96 – 100 = A 86 – 95 = B 76 – 85 = C 66 – 75 = D Under 66 = U Excellent Above Average Average Below Average Unsatisfactory Progress reports with grades will be issued to students by course throughout the program. Students enrolled in short-term courses will be issued grades upon completion of each course. Makeup and Fashion Couture Institute LLC Catalog 9 MFCI 18 Rev07.30.13 Conduct Policy All students are expected to act maturely and are required to respect other students and faculty members. Possession of weapons, illegal drugs, and alcohol of any kind are not allowed at any time on school property. All School regulations and policies are outlined in details below in Policies section of this catalog. Any violation of school policies may result in permanent dismissal from the School. 19 Dismissal Any student may be dismissed for violations of rules and regulations of the school, as set forth in school publications. A student also may be withdrawn from classes if he or she does not prepare sufficiently, neglects assignments, or makes unsatisfactory progress. The Director, after consultation with all parties involved, makes the final decision. The Director of Education may temporarily suspend students whose conduct is disruptive or unacceptable to the academic setting. After appropriate counseling, students who demonstrate a genuine desire to learn and conform to the School standards of conduct, may be allowed to resume attendance. The director will review each case and decide upon readmittance. 20 Previous Credits Makeup and Fashion Couture Institute LLC does not afford transferability of our credits to another institution unless there is a written agreement with another institution. Student Records 21 All student academic and financial records are maintained and filed in a secure and safe manner in perpetuity. Students are allowed to view their records, but the records must not leave the school. Official transcripts will be provided to the student at no charge, additional copies are available for $5.00. Should the institution cease operation, whether voluntarily or involuntarily, all educational records or legible true copies shall be filed with the Arizona State Board for Private Postsecondary Education within 15 days of ceasing educational operations. 22 School Policies 1. The School strongly enforces the Student’s professional appearance at all times during school hours, including the dress code. Casual business attire is the most desirable option; no shorts, baggy or too tight or revealing clothing; no open toe shoes or flip flops; no muscle shirt type clothing; No see-through clothing; no low cut jeans; Females must wear a full coverage brazier. In addition the Student will be held to the highest personal hygiene standards. Makeup and Fashion Couture Institute LLC Catalog 10 MFCI Rev07.30.13 2. The Student is required to conduct him/herself in a professional manner at all times. The Student must be respectful toward faculty and other students alike. 3. The Student must practice safety and sanitary rules at all times. The Student must keep their work area clean and free of obstructions. 4. The Student must cooperate in keeping the interior of The School clean, including the break room area and amenities. 5. Students must provide their own models for hands-on stage of the training. Students may work on each other only with a prior approval of an instructor. 6. The School enforces a “No Smoking” Policy within The School premises. 7. No weapons, drugs or alcohol allowed whatsoever (only medically prescribed treatments may be allowed, subject to the School Director’s approval). Any Student (s) found in possession of any of these items or being under the influence of any of these substances will be immediately terminated with no refund. In addition, The School reserves the right to prosecute criminally The Student (s) involved in any type of illegal activities. 8. The Student (s) shall not receive any monetary compensation during photo shoots, fashion shows or while working on a model at any time while attending The School (involves School functions only). 9. The student must be respectful of other students’ work areas, learning process and clients. That includes designated parking spots. Any interference with the above mentioned rules that is unwelcomed and disruptive will result in disciplinary action by the School. 10. The School’s phone and internet are for The School’s Personnel use only. No personal phone calls will be accepted for any of the students. 11. Although the school is committed to make the schedule reasonably convenient for The Student, The Student is responsible attending classes on time whenever he/she is assigned. The Student (s) who will fail to show up for the classes or on a test day in a timely manner will be held accountable and additional fees might apply for rescheduling those events. 12. All Students must attend their classes on time. Delayed arrivals of 10 minutes or more will result in suspension for that day; Excessive late arrivals and absences might result in termination of the Student and/or additional charges associated with rescheduling of those classes. Students who do not abide by the above rules and regulations will be subjected to suspension and/or termination without refund. Makeup and Fashion Couture Institute LLC Catalog 11 MFCI Rev07.30.13 Student Grievance Procedure 23 1. The following is the MFCI grievance procedure: (a) In the event where a Student will have a grievance, he/she must address their concern(s) verbally with the School Director within 10(ten) calendar days of occurrence. School Director must address the Student’s concerns and attempt to solve the issue at hand to the best interest of all parties involved and in accordance to the School Rules and Regulations Policy. (b) In the unlikely event when the issue remains unsolved, the Student may choose to submit a written grievance complaint to the School Director. Upon submission of the grievance the School must have a written response including an attempt to resolve the issue at hand within 30(thirty) calendar days. (c) In the event where the issue continues after the 30 (thirty) days expired, a mediation procedure must be offered as an option to all the involved parties during that time. (d) Mediation process must involve The Student and his/her guardian (if applicable) and the School authorized representative and any other party that has direct involvement in the complaint. (e) As is pertains to this document, A Grievance described as a complaint regarding a discrepancy which violates any written and published School Rules and Regulations Policy, not an arbitrary displeasure with an issue outside of a signed contract by which Makeup and Fashion Couture Institute LLC governs it’s internal affairs. “If the complaint cannot be resolved after exhausting the institution’s grievance procedure, the student may file a complaint with the Arizona State Board for Private Postsecondary Education. The student must contact the State Board for further details. The State Board address is 1400 W. Washington Street, Room 260, Phoenix, AZ 85007, phone # 602-542-5709, website address: www.azppse.gov 24 Class Schedule: As determined by Makeup and Fashion Couture Institute: All individual class and program schedules are tailored to a specific student’s needs; as such the dates and hours will vary depending on specific assignments. When an unexpected closure occurs due to extraordinary conditions such as inclement weather, students will be notified as soon as possible by phone and/or radio, and/or TV who provide closure information as a public service. Makeup and Fashion Couture Institute LLC Catalog 12 MFCI Rev07.30.13 Classes are not held on the following holidays: New Year’s Eve New Year’s Day Memorial Day Independence Day Labor Day Thanksgiving Day & the Friday following Christmas Eve Christmas Day Additional holidays might be observed based on individual schedules and are subject to prior notification. 25 Facilities The school is located at 7464 East Tierra Buena Lane Ste. 204, Scottsdale AZ 85260. Our classrooms are furnished with professional makeup stations; Separate classrooms for study and testing are fully equipped with desks and blackboard; Additional office space is designed to accommodate the School staff and faculty members.
