General Information
Sample Requirements
Sample requirements are listed showing the recommended collection tube.
In some instances alternative tubes may be used, eg the full range of tubes is
available for paediatric samples. Contact the relevant department to discuss:

The volumes indicated are to identify the correct tube only and do not imply
that more than one Monovette is required for a selection of tests. Although if
clotted blood is required in more than one department please send separate
samples. If in doubt about minimum volumes for given assays, please contact
the relevant department to discuss.

The minimum labelling criteria is surname plus two of the following: forename
or initial, date of birth, hospital number or NHS number.
For all blood transfusion samples the minimum criteria is full name (surname
and first name), hospital number or NHS number, date of birth, date and time of
collection and the sample must be signed by the person who took the sample.
Failure to meet these labelling criteria will result in the specimen being rejected.
Samples for Maternal Serum Screen must include either the hospital number or
NHS number as one of the three patient identifiers.
Samples for leukaemia immunophenotyping / cell markers (BCR / ABL, JAK2, PNH
Screen, EMA dye test and FISH), Genetics / Chromosomes (HFGE and HFE) and
tissue typing (HLA and HLA B27) must contain four points of reference, eg full
name (surname and first name), hospital number or NHS number and date of
birth.

Ipswich Hospital users:
Blood glucose meters: For replacement meters, QC logbooks and batteries
please bring complete blood glucose meter box to Clinical Biochemistry. Lost
meters may incur a small charge. QC solutions are obtained from the Pharmacy
Department.
POCT equipment training: Training is available for blood gas analysers, urine
test strip meters, pregnancy test kits, Coaguchek XS plus INR meters, DCA2000
HbA1c analysers and urinary drugs of abuse screening kits. Please call the
POCT coordinator for details. Blood glucose meter training session details are
on the intranet.
e-Learning: Online competency assessments are available for many of the
POCT devices in use. Please call the POCT coordinator for details.
External Quality Assurance schemes (EQA): We are inspected under ISO 15189
and ISO 22870 which require all POCT medical devices be enrolled on EQA
schemes. Samples are sent regularly and participation by all staff that test
patient samples is mandatory. For details please contact the POCT coordinator.
Blood gas analysers: Emergency Department blood gas analyser is available
for use by trained staff only. For training and barcodes please contact the
POCT coordinator.
Blood gas samples should be collected into electrolyte balanced heparinised
syringes. Samples MUST be free from air bubbles, capped and mixed for at
least a minute before analysis. Samples not analysed immediately should be
mixed for longer.
New equipment: Any department considering introducing new POCT
equipment must seek the advice of the POCT committee. For details please
contact the POCT coordinator.

Routine Clinical Biochemistry and
Endocrinology.
Laboratory Hours
The laboratory operates a 24-hour service. Full routine and emergency services
operate between 9.00 am and 5.30 pm, Monday to Friday, with a partial service
to 1.00 am. Only emergency services operate between 1.00 am and 9.00 am and
at weekends.
Urgent Requests
Urgent requests requiring immediate analysis, all blood gases and all
crossmatch requests must be notified by telephone to the department or, after
5.30 pm, by bleep 905 for Haematology and bleep 906 for Biochemistry.
All Emergency Department (A&E) requests where there is a likelihood of
4 hour breach must be notified by telephone to the relevant department.
Semi-urgent Requests
Samples received in the department between 5.30 pm and 1.00 am will be
analysed and the results available electronically within 2 hours of receipt.
There is no need to bleep for each request.
All requests from 1.00 am to 9.00 am must be notified by bleep 905 for
Haematology and bleep 906 for Biochemistry, unless the results are not
required within 2 hours.

Blood samples should be sent via the air tube, station 5709 for Haematology,
station 5705 for Biochemistry. CSF must be delivered by hand.
Web Browser
The introduction of a web browser within the hospital gives access to all
pathology results, even to GP-requested work. The web browser should be
available to all satellite and other hospitals in the future.
A full audit record of all enquiries including enquirer ID is logged automatically.
Any abuse of this system will be dealt with under the Trust disciplinary policy.

Collection of Fasting Samples
Fasting for plasma glucose is a minimum of 8 hours.
For lipids it is a minimum of 12 hours.
Anticoagulant Monitoring Service (AMS)
The Anticoagulant Monitoring Service must receive an AMS referral for both
new and existing anticoagulant patients on discharge from hospital.
To refer inpatients use Lorenzo iCM (select patient from patient list, enter
order and then select ACR or AMSR).
To refer outpatients use an AMS referral form or its PDF equivalent and send
by post or fax to 01473 703720. New outpatients can make appointments to
see the anticoagulant nurse specialist.
Please note that underfilled INR samples cannot be tested and the AMS will
request a repeat sample.
Laboratory Phlebotomist Opening Hours:
The laboratory open hours are Monday – Friday 7.30 am – 5.45 pm.
Saturday phlebotomy by strict appointment only, 9.00 am – 11.30 am,
telephone 01473 703721 for appointments.

OCM Ward Ordering
• One sample per request please.
• Completed results are available on microbiology screen.
• Urgent interim results are phoned.
Transport
All samples must be labelled and accompanied with a completed request form
with brief relevant clinical details.
Samples, except blood cultures and non-repeatable samples eg CSF, may be
sent by air tube to Microbiology.

Out-of-hours blood cultures should be placed in the incubator in the blood
bank room immediately after collection. All significant blood culture results
are phoned.
Non-repeatable samples should be delivered by hand to the laboratory.
All samples which are thought to contain high risk pathogens eg HIV, Hep B / C
and M tuberculosis must be labelled ‘Danger of Infection’ together with the
request form.

Histology and Cytology Specimen Requirements
Routine Histology – Please refer to Pathology Handbook for
comprehensive information. (Business Units Ô Clinical Support Services and
Cancer Ô Pathology Ô Pathology Handbook)
Routine histology specimens must be sent in formalin fixative in a container
appropriate to the size of the sample.
Frozen Sections
Frozen sections should be booked before the surgery is planned, this ensures
the relevant staff are available to process and analyse the sample. The sample
should be sent unfixed and handed to a member of the Histology staff.
Muscle Biopsies
Sample collection kit available, by request, from the Histology department.
Please allow 24 hours for preparation.
Nerve Biopsies
Sample collection kit available, by request, from the Histology department.
Please allow 24 hours for preparation.
Cytology
Gynae Cytology: Use Cytyc liquid-based cytology vial.
Non-gynae Cytology: Unfixed samples in a named universal with the exception
of urine samples that must be collected in a urine cytology container available
from the laboratory.
Post Vasectomy Semen Samples: Use semen analysis pack.
Semen Samples for Fertility Assessment:

Indoles are present in a number of foods and can give a false positive 5HIAA.
Suggest repeat urine collection avoiding the following foods for four days
before the collection and during the collection: bananas, avocados, red plums,
aubergines, pineapples, tomatoes, walnuts, blackcurrants, redcurrants and
chocolate.
Foodstuffs rich in capsaicin causes interference with the metanephrine assay so
must be avoided for four days before collection starts, and during collection.
These include: chilli peppers, chilli powder, curry powder and capsaicin paste.
If a test is not listed above, please contact the laboratory before starting the
collection.
If a combination of tests is requested requiring different preservatives, please
contact the laboratory for advice.

Microbiology Sample Requirements Edition 1
Urine Specimens
Urine containers are provided. These contain boric acid as a preservative. Urine
containers should be filled to the mark. The specimen will remain useful for up
to 48 hours at room temperature.
Small volumes of urine should be collected into plain universals and should
arrive at the laboratory within 4 hours or within 24 hours if refrigerated.
Collection of Mid-stream Urine
Instructions to the patients should be as follows:
1 Carefully wash the external genitalia and dry with a clean towel.
2Unscrew the top of the sterile container, avoiding touching the inside of
the bottle or lid.
3 Start passing urine, allowing the first part to flow into the pan.
4Collect the next part of the specimen straight into the container (women
should separate the labia with the fingers of the hand which is not holding
the sterile container). The bottle should be filled.
5Screw the lid on the container firmly. Wipe and dry the outside of the
container and write your name and the date on the label.
Collection of Urine from Infants
1 Bag Urine
External genitalia should be washed and dried before collection. Specimen
should be decanted into a sterile plain universal.
2 Supra Pubic Aspirate
These should be decanted into a sterile plain universal – these are treated
as sterile fluids and should be sent to the laboratory immediately.
Indwelling Catheter Samples
Urine should be aspirated with needle and syringe from the catheter sampling
port after disinfection of the port with alcohol.
Urine for Suspected Tuberculosis
At least 3 full early morning urine samples should be collected into large
containers (do not use small boric acid pots).
