APPEARING IN THE MERIT LIST DISPLAYED IN THE NOTICE
BOARD OF STATE MEDICAL FACULTY OF WEST BENGAL
FOR
ADMISSION TO MULTI-SKILLED MALE HEALTH
WORKERS COURSE.
SESSION – 2014.
The following Rank holders are directed to report for counseling as per
under-mentioned date, time and venue:

B. Merit listed candidates are directed to report 1(One) hour
before their scheduled date & time for counseling.
COUNSELING WILL BE CLOSED IMMEDIATELY AFTER ALL SEATS HAVE BEEN FILLED UP

The above candidates or their authorized representatives who will be
absent or not respond immediately after announcing their name at the
time of counseling, will not get any chance thereafter or at later stage.
C. 1.
For the purpose of verification of application, candidates are advised to
bring the following documents in original as well as one attested Xerox
copy of each of the original. The candidates shall invariably bring these
documents for verification. However, those candidates who have
already submitted Xerox copies of the under mentioned documents
alongwith application forms are not required to submit the same again.
a) Admit Card ( Madhyamik / Higher
Examination ) for verification of the age.
Secondary

equivalent
b) Mark Sheet of the Higher Secondary or any other equivalent examination.
c) Caste certificate in case of SC / ST / OBC(A) & OBC(B) only.
d) Valid disability certificate in case of PH candidates with locomotive disorder
only.
e) A Medical Certificate of Fitness from a Medical Practitioner and
f) Character Certificate from two responsible person other than relatives.
g) Candidates should mention name & contact number on the back side of
Demand draft.
2.
A candidate allotted a seat on a particular counseling date will have to
deposit Rs. 6000/- after allotment of Seat at the time of
Counseling preferably by Demand Draft drawn in favour of State
Medical Faculty of West Bengal, payable at Kolkata, failing which
his/her seat will be cancelled.
3.
Those candidates, who are not in a position to appear for counseling
personally, may like to authorize their representative in the duly filledin and attested authorization certificate as per Annexure I available in
the Prospectus. Such authorized representative will have to report with
the authorization certificate as well as all the relevant documents, as
mentioned in SL. No. C1 (a+b+c+d+e+f+g) and the required fees by
Demand Draft for the counseling on behalf of concerned candidate on
the scheduled date, time and venue. Failure to report in time and with
the required documents & fees will make the candidature of the
concerned candidate liable to be summarily cancelled.
4.
Not more than one Guardian will be allowed with each candidate in the
Counseling Hall during counseling.
5.
In the matter of allocation / admission of a candidate in a particular course /
Institute, the decision of the President, State Medical Faculty of West Bengal
shall be final and binding on the candidate.
6.
Counseling will be closed immediately after all seats have been filled up.
7.
Mere Merit listing the candidates does not entitle them for any right or claim for
the admission to Multi-Skilled Male Health Workers Course in 2014.

8.
PROCEDURE FOR COUNSELING
i)
At the first instance, ST candidates be called for Counseling. Vacant seats
for ST candidates, if any, be converted to SC candidates subject to
concurrence of Backward Classes Welfare Department, Govt of West
Bengal
ii)
Then, SC candidates be called for Counseling. Vacant seats for SC
candidates, if any, be converted to General candidates subject to
concurrence of Backward Classes Welfare Department, Govt of West
Bengal
iii)
Thereafter, OBC(A) candidates be called for Counseling. Vacant Seats for
OBC(A) candidates, if any, subject to approval of Competent Authority,
may be converted to General candidates.
iv)
Then, OBC(B) candidates be called for Counseling. Vacant Seats for
OBC(B) candidates, if any, subject to approval of Competent Authority,
may be converted to General candidates.
v)
Thereafter, candidates be called from the Combined [General, ST, SC,
OBC(A) & OBC(B)] List for Counseling for Admission.
Conversion of vacant seats be made with the approval of the Competent
Authorities after agreeing the same by the observer of the Backward Classes
Welfare Department, Govt. of West Bengal.
Further Note for the candidates
Merit-list has been made on the following basis :
(a)
Candidates securing higher total
Chemistry & Physics ranked higher.
(b)
In case of a tie in highest marks, candidates securing higher marks in Biology
ranked higher.
(c)
In case of a tie in marks in Biology, candidates securing higher marks in
Chemistry ranked higher.
Even in case of a tie then, candidate senior in age ranked higher.
Marks in each subject calculated proportionately based on 100 Marks.
Incomplete application forms have not been considered.
Candidates should Report an hour before counseling time on the specified date.
Candidates shall bring all original documents.
