WMEA/VITERBO UNIVERSITY GRADUATE CREDIT VALIDATION FORM This form will document your attendance at the 2014 Wisconsin State Music Conference to receive one (1) graduate credit through Viterbo University. Please indicate and report on your attendance at the various events of the conference. The form is divided into sections based on performances, workshops, and other sessions for each day. There is also a section to record your impressions of the exhibits, as well as any other events you attend while at the conference. You do not need to complete every section, but the sections you complete must reflect attendance at two full days or equivalent of conference activities. Please note that Wednesday, Saturday, and evening sessions, including the WSMA Honors concerts, can all be included in your session attendance. Please address the following in each of your responses: • Describe the ways in which you will apply what you have learned during the conference. How will it affect the way you do things? • How will this conference help you to reflect on your own practice? • How will it help you to assist students and parents in your district? • Feel free to attach additional pages if necessary to elaborate on any of your responses or to provide more information. All forms are due by November 14, 2014 to receive credit. You are encouraged to complete this form during the conference if possible. Forms may be turned in at the Conference Headquarters on the main floor of the Monona Terrace before 4pm on Friday, and also at the ticket sales table at the Middle Level Honors concert on Saturday. Grades will be issued within 4-6 weeks, and after that time you may request a transcript.
